[Comparison of heart rate to lactate threshold and to Conconi's threshold in children]. [儿童心率与乳酸阈值和Conconi阈值的比较]。

{"title":"[Comparison of heart rate to lactate threshold and to Conconi's threshold in children].","authors":"G Gaisl,&nbsp;G Wiesspeiner","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The heart rate (HR) at the lactate threshold was determined on a treadmill in 59 children with an average age of 11.8 years. 45 minutes later, they had to go through a vita maxima test on a bicycle ergometer. The load was increased stepwise by 10 Watt/min. During the tests HR was registered continually and stored every 5 seconds. In 52 children the characteristic break point was evident; in 7 children no break point could be found - in 2 children, because they had stopped the exercise before reaching the lactate threshold, and in 5 children because their HR increased linearly from the beginning until the end of the load application. HR at the break point correlated very strongly with HR at the lactate threshold (r = 0.99). {"title":"[Conconi test on the bicycle ergometer].","authors":"H Probst,&nbsp;C Comminot,&nbsp;J Rojas","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Several recent papers on the Conconi procedure tested on an ergometric bicycle have shown that the deflection of the heart rate is rarely seen if the procedure follows the usual protocol where the work duration remains the same at each work level. According to the di Prampero formula, work can be expressed as the energy cost of running (C), multiplied with covered distance. Therefore it is important, as in the current 400 m-track Conconi test, to keep work and not time constant at each work level. Consequently we examined heart rate and lactate profile in four groups of athletes, in following an experimental design respecting this condition. We regularly measured the level of blood lactate during training in order to estimate as precisely as possible the anaerobic threshold in swimming. As is already known sportsmen improve more rapidly their physical capacity (especially endurance) when they swim at a threshold speed corresponding to 4 Mmol of lactate. We used the following parameters: blood lactate concentration in relation to swimming time, and blood lactate accumulation in relation to covered distance for successive days of training at the same swimming speed. These two complementary procedures allowed us to follow the state of training of each athlete using simple graphs and to better advise him so that he may attain his threshold swimming speed. We have often noticed an improvement of the physical condition: either as an increase in swimming speed for the same accumulation of lactate, or as a decrease in the level of blood lactate for a same swimming speed. The aim was to measure the aerobic performance capacity of these athletes. The results give us on one hand informations on the actual performance potential of these sportsmen. On the other hand, they enable us to design an optimal training schedule. With such data in hand, it becomes possible to determine which intensity of endurance training is optimally adapted to an individual's progress of performance. It is thus easier to develop the sought after basic fitness in youth players. 120 ice hockey players of several leagues had to go through either a Conconi test, or our modified, more sports specific Probst test. 54 of these were junior players (age 15-16). Young gymnasts are submitted to intensive training, hence to an increase of the risk of injuries. In spite of this evolution, present medical assistance does not appear to be readily accessible to swiss coaches and gymnasts. By necessity, coaches endorse medical functions which have so far not been evaluated. This aspect was therefore investigated among coaches from the french speaking community of our country. The results show that the present medical network hinders regular follow-up of gymnasts and valuable discussions between coaches and doctors with, as a result, inappropriate medical teaching. Coaches distrust advice given by doctors of whom most underestimate the technical aspects of this sport. If this event gives rise to immediate symptoms, recompression is remarkably effective. This end-point is characteristic of joint pain, that is, Type 1 decompression sickness. Unfortunately the onset of serious Type 2 decompression sickness may be insidious and the delay may be associated with blood-brain barrier dysfunction. Pressure is less effective in the resolution of this problem than a raised partial pressure of oxygen. Standard therapy using oxygen may be associated with worsening of symptoms and air tables with recurrence. Recompression to 4 ata and the use of a mixture of 50% oxygen and 50% helium offers a good working compromise in the treatment of both serious decompression sickness and gas embolism arising in air diving, avoiding the need for a differential diagnosis. Only oxygen or helium and oxygen mixtures should be used in the therapy of decompression sickness in helium and oxygen diving. However, the subsequent evolution of such an event depends mainly on how fast the first aid is given to the patient. Therefore, the scientific and medical departments of our company developed a medical handbook to be used by chiefs of working platforms. The text which has to be easily understandable, mentions: a cursory description of the clinical signs of the different decompression accidents the measures which have to be taken in each case, depending on: the moment of the emergency: after or during decompression, the presence of an insufficient decompression, or a \"blow-up\". The handbook contains several recompression tables, first aid treatment recommendations and drugs. It has to be stressed that these procedures are only emergency steps.
