An elevation in skin temperature by a mean of 0.6 degrees C (p less than 0.05) and increase in intraarticular temperature by a mean of 0.8 degrees C (p less than 0.01) was noted following the electrical stimulation. Intraarticular pressure in the stimulated joints has only a trend to increase, but not significantly. In four other rabbits, synovial tissue of the stimulated joint showed blood vessels congestion and interstitial edema. {"title":"Severely mobility-disabled people assess the quality of their lives.","authors":"R. They were asked to rank 30 different abilities involving physical and mental functions, interpersonal and social relationships, and to rate their overall quality of life (QOL) on a 0-10 point scale. Among the severely mobility-disabled subjects the mean value of self-reported QOL was 8.0, which differs only slightly from 8.3 among the controls. The mean QOL among the disabled showed no significant difference regarding congenital/acquired and progressive/permanent disability. The 'abilities' ranked 1-9 were not directly related to mobility and corresponded among the disabled and non-handicapped. The functions lacked by the severely mobility-disabled persons were rated as less important by the disabled. Enrolled nurses assessed independently 100 aged patients. Scalability and interobserver variability were tested with Guttman scale-analysis. The results indicated that the activities are ranked according to a cumulative scale and that the index is reliable. Patients independent in ADL had shorter hospitalization and were discharged home more often than were the dependent patients. This indicates that the scale is valid. One year later most dependent patients were either dead or living in institutions: thus the index has a predictive potential. The systematic errors of scale were used to refine the index for use in short-term care. Previous trials, however, have failed to increase the IAP by abdominal muscle training. Twenty healthy subjects, 20 low-back patients and 10 weight-lifters, were tested with various breathing techniques in order to elucidate the causal factors of the IAP rise during lifting and the effects respiration. Those with high IAP and low IAP as well as those with great variations in IAP underwent an extended program. The intra-abdominal and intrathoracic pressures and the EMG of the oblique abdominal, the erector spinae and--in some cases--the puborectalis muscles, were recorded. The transdiaphragmatic pressure was calculated both during lifting and during the Mueller manoeuvre. The IAP rise during lifting seems to be correlated to a good coordination between the muscles surrounding the abdominal cavity. Of these, the diaphragm seems to be the most important for the IAP level. The effect of cross-education upon the contralateral muscle was also examined. EMS with a frequency of 50 hertz, a duration of 0.2 miliseconds, and a rectangular wave was applied for ten seconds with a ten-second interval and repeated ten times per day for six weeks. The ST group gained significantly 9.4%, 15.5%, and 16.4% after two, four, and six weeks of stimulation, respectively, while the SH group also showed a significant gain of 5.1%, 8.3%, and 3.0%. When comparing the two groups at the end of the six-week period the ST group's gain was significantly greater. The increase of power production of the unstimulated TA was 5.5%, 8.0%, and 4.3% in the ST group, which was significant at the end of the second and fourth week of stimulation. The SH group, however, registered a non-significant increase of -2.7%, 1.8%, and -1.5%. Eighty-five persons, mainly elderly, who had consecutively consulted a district occupational therapist, were assessed in their homes in order to study the reliability, the scalability, and the validity of the expanded index. The inter-observer reliability was high. The coefficient of scalability was well above the acceptance level, indicating construct validity. No person was dependent in personal ADL and totally independent in instrumental ADL. Persons who were dependent in both personal and instrumental ADL were older and lived in sheltered accommodation more often than persons who were dependent only in instrumental ADL, indicating external validity. This study shows that there was a cumulative relationship between certain well-defined instrumental activities and between personal and instrumental activities. The predicting variables describing the treatment group (n = 95) and the control group (n = 93) at the initiation of study included sociodemographic factors, variables related to work, severity of low back pain, and a number of clinical measurements and evaluations. The Oswestry Low Back Pain Disability Questionnaire was used for judging recovery factors. Analysis of the results provides statistically significant parameters for characterization of the oedema. Mobility of the fluid in pitting oedematous tissue was significantly greater than that of normal tissue and decreased after pneumatic compression treatment. Oedematous fluid at the distal sites of the arm showed a greater mobility than that of the proximal sites. Improvement in the oedema due to pneumatic compression treatment was found to be greater at the distal sites than at the proximal sites. In addition, oedema recurred between the treatments and repeated treatments produced the best results.
