Patient mood in 37 stroke patients was related to the critical attitude of a key relative. Patient depression may also have been associated with severity of dysphasia, but no link was found between patient mood and the other measures of cognitive or physical deficit used in this study. Psychiatric distress in the key relative had a rather different causal basis. Relatives' psychiatric disorders were associated with physical burden, cognitive deficit, and severity of dysphasia. The Type-A sum score varied with age, sex and work status, but not with basic education. Respondents were further assigned to two extreme groups according to the Type-A sum score: the Type A's in the highest and -B's in the lowest quartile. Of those aged 40 years or more (n = 1,460), the persons with the Type-A behaviour pattern reported typical severe angina pectoris symptoms more frequently than the Type B's. There were no differences, however, in the frequency of typical mild angina pectoris symptoms between the Type A's and B's. The functional capacity assessed by the NYHA-classification improved in about 40% of the patients. About 80% considered their symptoms and well-being to have improved after the operation. At the re-examination, heart size was most often enlarged in patients with mitral valve replacement. The average work load measured in bicycle ergometer test was higher in patients with aortic valve replacement compared to those with mitral valve replacement. Patients with aortic valve replacements were working more often (54%) than those with mitral valve replacements (37%). The mean age of patients who were working was significantly lower than in patients who were retired. There was a statistically significant relation between the physical working capacity and the working status. Psychological and social data were covered by means of questionnaires and a brief interview and somatic data by a standardized medical examination. New concepts were introduced after factor analysis. The degree of preoccupation varied very much and was considered as a reaction to the diagnosis of MI. Seventy-nine percent of the patients complained about fatigue and 65% felt anxious and depressed. Fatigue and nervousness were regarded by the patients as more disabling than cardiac symptoms. Emotional distress was related to a previous history of emotional complaints and to psychological factors and self-reported coronary symptoms but was unrelated to severity of the infarction, medically rated cardiac symptoms, demographic and social data. Sexual maladjustment, mainly due to fear, was frequent and associated with both emotional and somatic variables. The two groups were found to have the same characteristics. No significant differences could be demonstrated either concerning the initial duration of symptoms and sick leave or the number of recurrences and their duration during the observation year. The interpretation of this lack of positive effect is, that in this comparatively more heterogenous population the actual procedure has less influence. However, there were more patients with periods of sick-leave for different other reasons in the control group. Not surprisingly the treated group of patients were also more satisfied. The joint load was calculated from forces recorded with a force-measuring platform and pictures on cine-film. Levels of muscular activity were recorded with rectified, low-pass filtered, time-averaged and normalized EMGs. The loading moment about the hip joint was about 45 Nm during the initial part of the rising exercise, decreasing with smaller hip angle. Increase of the backward inclination of the trunk increased the load moment, which was maintained at about the same level during the rising exercise. Two adaptive adjustments lowered the joint load: foot position further backward and reduced resistance from the device. Foot position further forward increased the joint load. The levels of muscular activity in the hip extensors were low to medium and were slightly increased by posterior foot position and increased resistance from the device. Forty-six per cent of the subjects had no history of back or joint complaints. Thirty-two per cent reported current back pains while 50% stated they never experienced such pains. Back pains (past or present) were localized in the lumbar region predominantly (40%), and were almost constantly present in 8% of the subjects. The spinal mobility remained acceptable for function of the activities of daily living (ADL) in the majority of the subjects. Thirty-six per cent of the subjects had past or present complaints from the joints of the extremities, predominantly of the knee, shoulder or hip joints. Physical signs of joint disorders as deformities, swelling or tenderness were rare except for enlargement of distal interphalangeal joints (right hand: females 38%, males 14%). Restricted range of motion (ROM) in one or several joints was found in one fifth (i.e., knee joints) to two thirds (i.e., hip joints). No significant gender differences were observed with the exception of the thoracolumbar ROM that was more frequently restricted in males. Severe restriction of separate movements (to less than 50% of ROM) was, generally, infrequent (0-8% of the subjects) although advanced impairment of hip inward rotation was found in 14% of the subjects. Restricted ROM associated with joint complaints was found in 2-11% of the subjects (i.e., knee joints 2%, shoulder joints 9%, hip joints 11%). The sign consists of a V-shaped widening of the upper part of the space between the humeral head and the glenoid cavity on anteroposterior shoulder films in the erect position. Twelve of 14 patients showing this sign went on to develop chronically painful shoulders, and four of them developed radiologically evident subluxation within several months. We established the effect of physical training alone and combined with flunarizine on intermittent claudication. Twelve patients aged 48-73 years were included in the study. Pain-free walking distance on treadmill, ankle/arm pressure ratio and transcutaneous oxygen tension were measured. Walking distance increased significantly (p less than 0.05) by 130% from 75 m to 173 m during the first year when the patients were on programmed physical training. Ankle/arm pressure ratio also increased significantly (p less than 0.05) from 0.46 to 0.55 during this period. The increase in walking distance ceased when the programmed physical training was discontinued for 6 months. During the following double-blind, cross-over medication period the patients were given flunarizine 5 mg b.i.d. and placebos in randomized order for 3 months each. They also continued the same programmed physical training as during the first year. Walking distance increased, albeit not significantly, with time to 392 m after the second medication period. There was no difference, however, between flunarizine and placebo. Ankle/arm pressure ratio was of the same magnitude as at the beginning of the trial. Oxygen tension measurements did not give consistent results. We conclude that programmed physical training increased walking distance as a function of time. The study was designed as a randomized, controlled and single-blind trial. Twenty patients were assigned to two groups 10-12 days after the injury. One group was treated conventionally, the other received instructions in self-training with follow-up control of results. Objective examination and subjective assessment were made at five intervals, up to one year after the injury. The greatest improvement in function was shown in both groups between 3 and 8 weeks. The patients deemed their daily life functions to be normal 8 weeks after the injury. The objective assessment was normal at later stages. No significant differences were found between the two groups in any of the tests.
