For this reason all our efforts must be aimed at attaining two goals. First, we must replace the pressure sore by a supple, well-nourished cushion of soft tissue, which can support the weight of the body for a limited period of time. Submucous CP or its microforms were observed in an additional 30 patients (32%). Even though most of the patients have micrognathia, the high frequency of submucous clefts speaks against the role of the interposed tongue in the pathogenesis of CP in the Pierre Robin sequence. There was no hypernasality of speech in 27 of the 30 patients with submucous CP or its microforms, and the spontaneous speech of the other three was good. This is in contrast with observations in isolated submucous CP in general. No operative treatment was indicated in any case. Results after treatment of open CP and speech problems were in good agreement with results from the patients with isolated CP from the same period of time. As newborns 11 of the patients (12%) had severe respiratory difficulties caused by glossoptosis, and a Douglas operation was performed in three patients. Experiments were performed to compare the revascularization of the tibial nerve of the rabbit following classical end-to-end anastomosis and autogenous nerve grafting. The animals were divided into two groups and from each animal a 1.5 cm length of the nerve was excised. End-to-end anastomosis was then performed in the first group of 12 animals. The second group received autogenous grafts from the peroneal nerve. A microangiographic study was performed after a survival time of 30 days. The microangiograms were analysed quantitatively, and the density of the vascular network at the sites of anastomosis and at the graft was quantified. In some cases there is no stabilization on completion of normal skeletal growth. This may lead to severe malformation and functional disturbance to the face. In these cases, the patient must be followed regularly and resection of the deformed tissue must be performed repeatedly as the recurrences appear. The bone remained vital, being nourished by a slender connection to the surrounding tissue. Experience from the few cases reported in the literature shows that closed reduction is often successful. If surgery is necessary it seems that it should be limited to simple reduction of the dislocation and that there is no need for primary arthrodesis or arthroplasty. Important aesthetic guidelines are described, and techniques which the author considers unsatisfactory are listed. A simple basic technique is recommended for routine procedures, which may then be varied according to special needs. Preferred methods of raising the tip, augmentation of the tip and correction of secondary deformities are outlined. Known middle ear pathogens were isolated from 49 (53%), these accounted for 68% of all positive cultures. The frequency of occurrence of middle ear pathogens was significantly higher in the age group younger than 36 months. The most common bacteria found were S. pneumoniae, B. catarrhalis and H. influenzae. The only anaerobe was a strain of B. fragilis. We believe that by defining culture methods, more pathogens can be isolated from the middle ear of cleft children than has previously been reported.
