{"title":"[Oviductal physiological alterations and endosalpingiosis (author's transl)].","authors":"A Ernst, E Aguilera, A Dabancens","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A study was made of the oviducts from 110 patients salpingectomized between 1943 and 1976, after endosalpingiosis had been diagnosed. All of the patients were infertile women in whom histopathology performed after surgery revealed diverticula located mainly in the isthmic and interstitial portions of the oviductal wall. By either continuity or invasion, these configurations spread the endosalpingeal epithelium into the peritoneum and/or the ovary. Endosalpingiosis affects the muscular layer of the oviduct, which becomes thickened and rigid, thus accounting for the severe alterations of tubal physiology. {"title":"[Inhibition of menstrual uterine motility with four beta-adrenergic drugs (author's transl)].","authors":"R Cifuentes, E Cobo","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Effects of the sublingual administration of four beta-adrenoceptor drugs on the uterine motility in 40 normal menstruating women were studied. The drugs and total doses tested were: orciprenaline (40 mg), Partusisten (10 mg), salbutamol (8 mg) and isoxsuprine (40 mg). The uterine and antidiuretic activities were studied before and after administration of each one. All those drugs employed reduced greatly the uterine contractions in all the patients. The cardiovascular side-effects were minimal and well tolerated. When hormonal therapy is administered, the selection of the adequate pill is very important. The effect of D-(-)-norgestrel 300mg was studied on uterine contractility values during puerperium is seven patients breast feeding and was compared with a control group of 26. The 33 patients had the following characteristics: multiparous during puerperium without recent episiotomy, with healthy cervix, absence of genital septic focus, uterine tumours or malformations; all of them breast feeding. In the hypothesis, it was considered that the endogenous oxytocin increases and stimulates the mammary mioepithelium and uterine contractilities. For recording uterine contractility, the technique of Jaumandreu and Hendricks was used. The recordings were made during 24 hours postpartum, and at 5, 10, 20, 30, and 40 days with a duration 2 to 3 hours. All the studies were longitudinal. The changes of the human uterine contractility during normal puerperium were estimated. The range of the tonus was 22--41 mm Hg, the intensity 5--18 mm Hg, the frequency 17--23 contractions in 10 minutes, and the uterine activity 102--223 Montevideo Units. In the control group the following results were obtained: The range of the tonus was 24--34 mm Hg, the intensity 9--16 mm Hg, the frequency 17--37 contractions in 10 minutes, and the uterine contractility 137--524 Montevideo Units. The initial treatment was always 10 mg/day for 10 days followed by a modification of the daily dose and/or length of treatment needed. Ovulation was induced in all 4 AS patients with an adequate luteal phase (ALP) in 3 of them. In all other patients (with ILP and SLP) an ALP was induced; 2 of them became pregnant. {"title":"Correlation between incremental changes of oestradiol-17 beta and prolactin during the menstrual cycle by means of multiple plasma sampling.","authors":"P M Kicovic, F Franchi, M Lusi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Incremental changes of circulating oestradiol-17 beta (E2) and prolactin (PRL) at midcycle and in the luteal phase were studied in 7 healthy young women by means of multiple plasma sampling over a longer period of time. The mean E2 and PRL levels were significantly higher at both times of assessment than in the follicular phase (p less than 0.001). Rapid fluctuations of both E2 and PRL levels were observed; amplitude of E2 fluctuations was increased at higher mean E2 plasma levels. Regression analysis showed a significant correlation (r = 0.62; p less than 0.01) between the incremental changes of E2 and PRL levels at midcycle and in the luteal phase. {"title":"Reconstructive microsurgery of the fallopian tube with the carbon dioxide laser - procedures and preliminary results.","authors":"J H Bellina","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Unlabelled: </strong>In 1974 the carbon dioxide laser was adapted to the operating microscope for reconstructive pelvic surgery. A protocol was designed to test the efficacy of this surgical modality and a new study begun. Complete documentation of laparoscopic findings, surgical technique, and pre- and post-operative hysterosalpingograms are kept on videotape. One hundred cases will be entered into this study. Patency and pregnancy failures will be compared with patency and pregnancy successes to determine, if possible, the reasons for failure. In this paper detailed descriptions of the surgical techniques employed in the first 61 cases are presented. PRELIMINARY RESULTS in terms of patency and pregnancy experience to date are reported.</p><p><strong>Preliminary results: </strong>Eighty-two couples have been evaluated at the Reproductive Biology Unit. Sixty-one cases have undergone fertility enhancement laser microsurgery and/or interim medical management prior to surgery for infertility. Bilateral patency or patency of the only existing fallopian tube was demonstrated post-treatment in 93% or 57 of the cases. Eliminating those cases who are not at risk of pregnancy due to elective contraception or medical prohibition during Danocrine therapy (28), conception has occurred in 10 of 33 patients. This represents approximately one of every three patients at risk. The obtained fractions were lyophilized and tested against human sperm agglutinating and sperm immobilizing sera, in order to detect their antigenicity. Positive and negative controls revealed intensive precipitations and no precipitations respectively. Fraction A4 reacted intensively with sperm immobilizing sera. The final objectives of these sperm antigens solubilization studies are twofold. The analysis has established five characteristic stages which permit the individualization of the maturation stage of the oocyte in the ovary.
