PBMC incubated with IL-2 (10 IU/ml) and IFN-gamma (200 IU/ml) for 4 days showed the maximum cytotoxic activity against K562, MOLT-4 and Daudi cells. Combination with IL-2 and IFN-gamma induced stronger activity than IL-2 or IFN-gamma alone. In order to investigate the sequential roles of IL-2 and IFN-gamma in the synergy of killer cell function, IFN-gamma was stimulated after washing-out IL-2 or stimulated at various timing and duration without washing-out IL-2. IL-2 was essential to induce the synergistic effect of IL-2 and IFN-gamma to killer activity. The similar augmentation of killer activity was observed by the addition of IFN-gamma at any incubation period with IL-2, compared with that of stimulations IL-2 and IFN-gamma stimulation. {"title":"[Histopathological studies on antitumor effect of tegafur administered by continuous intravenous infusion].","authors":"A Yagawa","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The effects of preoperative treatment by continuous intravenous infusion of Tegafur, the antagonist of DNA synthesis, were histopathologically studied in 34 patients with gastric cancer. Histologically the treatment was found to be effective in 41.2% of patients with cancer invasion in the mucosa, 58.8% in the submucosa, 61.3% in the muscularis propria, 59.3% in the subserosa and 86.9% of those with metastatic lymph nodes. The treatment was effective, when assessed in terms of the histological type of cancer, in 90.9% of cancers of the differentiated type (papillary adenocarcinoma, well differentiated tubular adenocarcinoma and moderately differentiated tubular adenocarcinoma) and 47.8% of those of the poorly differentiated type (poorly differentiated adenocarcinoma, mucinous adenocarcinoma and signet-ring cell carcinoma), showing a higher rate of efficacy in the differentiated type cancers. Meanwhile, even among patients with cancer of poorly differentiated type, a high efficacy rate (90.0%) was found in those with metastatic cancer of the lymph nodes. No relationship was found between the total doses of Tegafur and histological effects. There was a tendency, however, for a higher frequency of a good response in patients administered more than 4,000 mg of Tegafur. In the patients with a histologically positive effect, 5-FU concentration in the tumor tissue was higher than 0.071 microgram/g. However, some patients showed no response despite a high concentration. This finding suggested that sensitivity to 5-FU and 5-FU metabolism vary depending on the tumor. It is reported that the abscopal effect is observed occasionally in the cases of radiotherapy for malignant lymphoma, malignant melanoma and seminoma. However, pathophysiology and mechanism of the abscopal effect have not been well defined. An aim of this study was to investigate the pathophysiology and mechanism of the abscopal effect in patients with breast carcinoma.</p><p><strong>Subjects and methods: </strong>Sixty two patients entered this study. Age distribution was from 29 to 84 years old (mean 54.0 years old) and all cases were females. Their stages were as follows: stage II 12 cases, stage IIIa 16 cases, stage IIIb 22 cases and stage IV 12 cases. They were irradiated pre-operatively using less fractionated large dose irradiation. Then, they underwent mastectomy or tumor resection.</p><p><strong>Results: </strong>The abscopal effect on metastatic lymph nodes was observed in 15 out of 42 cases (35.7%) by palpation. The histopathological abscopal effect was noted in 22 out of 42 cases (52.4%). Incidence of the abscopal effect was significantly higher in patients under 55 years old as compared with that in patients over 56 years old (p less than 0.05). The abscopal effect was highly observed in the cases who had the infiltrating lymphocytes around the degenerated cancer cells in the irradiated primary tumor nests (p less than 0.01). The subsets of the infiltrative lymphocytes were analyzed immunohistologically using monoclonal antibodies. It disclosed that the infiltrative lymphocytes were CD8 and CD4 positive lymphocytes. Those findings suggest that abscopal effect was caused by activated cellular immunity in hosts. The five and ten years survival rates of stage IIIa were 71.4% and 71.4% respectively. Stage IIIb showed 62.5% in 5 years survival rate and 54.7% in 10 years survival rate. The survival rate of the cases with the abscopal effect was higher than that of the cases without the abscopal effect, however it was not statistically significant. Therapeutic efficacy and protective effect of high-dose Bismuth Subnitrate on toxicity of Cisplatin were evaluated in twenty-five courses of treatment. The Pharmacokinetics of Bismuth Subnitrate and Cisplatin were also studied in several courses. Bismuth Subnitrate was administered orally at a dose of 150 mg/kg/day for 10 days and a dose of 80-150 (Mean 108) mg/m2 of Cisplatin was administered intravenously on the day six of Bismuth Subnitrate administration. Toxicities of high-dose Cisplatin were minimal under administration of high-dose Bismuth Subnitrate and therapeutic efficacy was observed in several patients. The pharmacokinetics of Bismuth Subnitrate in plasma and urine demonstrated that 10 days administration of high-dose Bismuth Subnitrate was appropriate to maintain adequate concentration of Bismuth for preinduction of Metallothionein in organs. The pharmacokinetics of Cisplatin in plasma and urine demonstrated that deposition of total and ultrafilterable (free) platinum in blood were well described by a biphasic manner with a very rapid first phase and a very prolonged second phase, and that urine excretion of platinum was similar to the conventional manner. {"title":"[Evaluation of cardiotoxicity by radionuclide angiocardiography in the chemotherapy of gynecologic malignancies].","authors":"M Yabuta","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Cardiotoxicity was evaluated by multigated radionuclide angiocardiography using in vivo 99mTc labeling of red blood cells in the multiagent chemotherapies of gynecologic malignancies. The left ventricular ejection fraction (LVEF) was determined by computer-assisted analysis of left ventricle time activity curves (TAC) of the angiocardiograms. The mean age of fifteen patients was 52.3 (38-66) years. Ovarian carcinoma (13 patients), the endometrial carcinoma (one) and uterine leiomyosarcoma (one) were treated with a total of 69 combination chemotherapy of CAP every 4 weeks. Fifteen patients underwent a total of 76 quantitative radionuclide angiocardiograms (three to seven studies per patient). The values of LVEF before treatment were 57.5 +/- 8.3 (mean +/- SD) (48.7-75.4), which were greater than those of normal persons (45). The values of LVEF during treatment were 44.9 +/- 6.2 (mean +/- SD) (37.6-58.5), which were significantly (p less than 0.001) lower than the pretreatment values. The decrease rates of LVEF in fifteen patients were 21.0 +/- 10.4% (mean +/- SD). Over 20% decrease of LVEF was observed in eight patients (53.3%) following chemotherapies. The patients showing a decrease of the values of LVEF were treated with lower dosage of chemotherapeutic agents in three, changed in seven and discontinued in six the agents. Nine patients who changed the chemotherapeutic regimens showed improvement of the values of LVEF in six (66.7%), no change in two (22.2%) and deterioration in one (11.1%). The values of LVEF were decreased in four patients (44.4%) at 6.7 (3-11) months following chemotherapies. These patients had disease resectable with curative intent and were followed for a minimum of 5 years or until death. Among 2,749 Stage I cases, 33 had recurrence and proved to be histologically stage I. A pair matching case control study between these 33 cases and matched 33 stage I controls without recurrence proved that the preoperative CEA level was relatively higher in cases with recurrence (p = 0.079). In Stage II and III cases, an analysis of variance using a cut off level as a block factor was performed. Among 315 Stage II cases, preoperative CEA levels were significantly higher in cases with recurrence than in cases without recurrence. In 468 Stage III cases, no correlations between preoperative CEA levels and high risks of cancer recurrence were detected. In Stage I and Stage II cases, CEA levels significantly increased at the time of recurrence. We investigated effects of radiofrequency hyperthermia (RF-HT) in combination with anti-cancer drug UFT (a combination of tegafur and uracil) on activities of DNA-synthesizing enzymes, using colon carcinomas induced by DMH treatment in rats. 1) Thymidylate synthetase (TS), DNA-synthesizing enzyme in de novo pathway of pyrimidine metabolism, increased to approximately 7-fold that of normal control colon in DMH-induced colon carcinomas in activity, but was markedly reduced to 48% of that in the carcinomas by administration of UFT. 2) Thymidine kinase (TK) in salvage pathway increased to approximately 8-fold that of the control in the carcinomas in activity, but was markedly reduced to 25% of that in the carcinomas by RF-HT treatment. 3) The TK-isozyme, that was thought to be a fetal type in the intracellularly cytoplasmic fraction and closely involved in rapid DNA replication, increased to approximately 24-fold that of the control in the carcinomas in activity, but was reduced to the nearly same level as that of the control by RF-HT treatment. This isozyme was labile in thermostability and easy to be inactivated in low pH. 4) The activities of TS and TK in the carcinomas were extremely suppressed by UFT administration in combination with RF-HT treatment. The each group consisted of 10 mice. Oral medications were performed once a day for one week (6 days continuous duration) and these medications were continued during a total period of four weeks. Once a week, the general condition of the animals was checked and at the same time, the size (length x width) of the tumor was measured, and the anti-cancer effect was determined.</p><p><strong>Result: </strong>It was noted between the control and 5'-DFUR medication group that there was a marked effectiveness on the rate of decrease in size and disappear of the squamous cell cancer according to the medication of 5'-DFUR. It regarded to the amount of 5'-DFUR, that there was a significant difference in the anti-cancer effect between 90 mg/kg and 210 mg/kg groups and between 150 mg/kg or 210 mg/kg and control groups. {"title":"[Therapeutic factors related to irradiation in primary and metastatic liver cancer using multivariate analysis].","authors":"K Hatano","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Between December 1973 and August 1987, 21 patients with primary liver cancer and 41 patients with metastatic liver cancer were treated with external irradiation, intra-arterial infusion chemotherapy and/or trans-arterial embolization (TAE) at the National Medical Center Hospital, the National South Kyushu Central Hospital and the National Kure Hospital. They were all inoperable cases. We diagnosed the tumor site and the involved area with many imagings and we decided the target volume. 1. Primary liver cancer: the average survival period was 10.9 months, the 1-year survival rate was 28.6%, the 2-year survival rate was 14.3% and the 3-year survival rate was 4.7%. Using multivariate analysis, stage, cancer reduction rate, Child classification and field size were valuable factors of the prognosis in the arterial infusion group. In the TAE group, cancer reduction rate was the most valuable factor. 2. Metastatic liver cancer: The average survival period was 8.0 months, the 1-year survival rate was 19.5%, the 5-year survival rate was 2.4%. Using multivariate analysis, pre-treatment K.P.S, distant metastasis, H, Child classification were valuable factors and pre-treatment K.P.S. was most valuable factor. Lymphokine activated killer (LAK) activity in peripheral blood in administration of IL-2 showed a significant high of 13.2% at effector/target (E/T) ratio 13 compared with non-administration of IL-2 showed 4.3%. LAK activity in cervical non-metastatic lymphnodes in 6 cases of administration of IL-2 showed a significant high of 23.0% at E/T ratio 50 compared with 3 cases of non-administration of IL-2 which showed 3.7%.
