{"title":"Cardiac Tamponade Associated with Rivaroxaban.","authors":"Stephen Boone","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Rivaroxaban is an oral anticoagulant approved for prevention of stroke, as well as for the treatment and prevention of venous thromboembolic disease. Hemopericardium is a serious complication of anticoagulant use, which has been reported with oral vitamin-K antagonists and newer oral anticoagulants. At the time of this report, to my knowledge, there are no published reports of hemorrhagic effusion leading to tamponade associated with a Factor Xa Inhibitor. I report a case of hemopericardium with associated tamponade in a patient who developed pericarditis while being treated with Rivaroxaban. {"title":"Thrombus in Transit through a Patent Foramen Ovale: Surgical Prevention of Impending Stroke.","authors":"Zaher Fanari, Sumaya Hammami, Usman Choudhry, Asim A Mohammed, Mark Zweben, Wasif Qureshi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A Transesophageal Echocardiography (TEE) is essential in identifying thrombus in transient in patients with deep venous thrombosis (DVT) presenting with symptoms suggesting potential systemic emboli. We present a case of a 71-year-old gentleman with recent DVT who developed slurred speech and was assumed to have transient ischemia attack (TIA). TEE showed the presence of a large Thrombus in Transit (TIT) through a patent foramen ovale (PFO). Ask the patient about costume jewelry or button reactions. If it is an elective surgery, obtain a confirmatory test with the aid of a dermatologist or allergist. It is recommended to use a non-nickel implant if the surgery is urgent, the patient has a confirmed allergy, or the patient does not want to undergo testing, as these implants are readily available in 2015. {"title":"Left Internal Mammary Artery Dissection: Use of Dual Balloon Angioplasty in Treatment of Ostial Dissection Involving Subclavian Artery.","authors":"Zaher Fanari, Jhapat Thapa, Subba R Vanga, Wasif Qureshi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The left internal mammary artery (LIMA) is the preferred arterial graft to be used for the left anterior descending (LAD) artery for coronary artery bypass graft (CABG) due to high graft patency rate. LIMA dissection is a rare, but dreadful complication of graft angiography and may lead to serious complications including death, myocardial infarction (MI), and re-do CABG. Transcatheter management of LIMA dissection involves multiple stenting. However, this may leave a dissection flap at the ostium of the LIMA that may extend to the left subclavian artery jeopardizing flow to the left arm and vertebral artery.
