Thirty healthy patients having plaque on all tooth surfaces were admitted for this two-week, double-blind study. Patients served as their own control by brushing manually for one week, and then for a second week with a randomly-assigned electric brush. Three plaque indexes were scored at baseline, one week and two weeks. For all these indexes, results showed that the Water Pik Automatic Toothbrush removed significantly more plaque than manual brushing. The Interplak brush removed statistically more plaque than manual brushing only with the Turesky Index. The data for all indexes have a trend toward better plaque scores for patients who used the Water Pik Automatic Toothbrush. All the parameters were evaluated at baseline and after three and six months of home use of either Plax or placebo rinse. Microbiological components of plaque were monitored. These measures were then compared with the infection control guidelines recommended by the American Dental Association for exposing and processing radiographs. During exposure procedures, it was found that an overwhelming majority of practitioners wore disposable gloves and placed saliva-contaminated film packets in a receptacle after removing them from the mouth. Very few clinicians complied with the ADA recommendations for wrapping surfaces in the X-ray operatory, but many used disinfectants to decontaminate surfaces touched by the operator. Most respondents washed or removed their gloves before leaving the operatory to process films, thereby stopping the spread of saliva. ADA recommendations for infection control during processing were generally not followed although alternate methods were frequently used. It is emphasized that infection control regimens be employed for all patients, and not only for those who are thought to be infectious. Twenty-nine first year dental students were randomly assigned to one of the two glove treatment groups. The tactile discrimination ability of each subject was assessed at baseline and three weeks after using the Roughness Discrimination Test (RDT). The subjects wore their assigned gloves when performing laboratory and clinical procedures during the 3-week period. Following this period the RDT was again administered. Subjects then switched gloves for an additional 3-week period, at which time data were collected regarding perceived discrimination ability and glove preference. Data were analyzed using a repeated-measures analysis of variance. Results revealed no significant difference (p less than 0.05) in subject's tactile discrimination ability while 29% indicated that the latex glove had an inhibitory effect. The subjects most frequently preferred the latex glove, citing specific favorable characteristics such as size, fit, comfort and durability while performing clinical procedures. One of these is flushing water through the water lines prior to use. The objective of this study was to determine the actual change over time in the number of CFUs during flushing. Four consecutive samples were collected over time from the water lines of 20 different dental units that had been in disuse for over 48 hours. Next, autoclaved handpieces were connected and two additional samples were obtained. After culturing and determining CFUs per ml, data were analyzed to determine differences between samples. There were significant reductions between the first and second, second and third, third and fourth samples prior to adding the handpiece. Flushing of the water line reduced but did not eliminate this contamination. Even after six minutes' flushing, some water samples still contained more than 10(4) viable bacterial cells per milliliter, although coliform counts were less than two per 100 milliliters as measured by a Most Probable Number assay. Sterilization of the tip or the entire syringe did not eliminate this contamination. Scanning electron microscopy revealed bacterial biofilms on the inner wall of the plastic tubing supplying water to the air-water syringe, but not on the air line or on new, unused tubing. Such biofilms probably can be found in any tubing which supplies water to components of dental units. The inoculum for these biofilms comes from ubiquitous environmental aquatic bacteria, some of which can cause disease in compromised patients. Three S. sobrinus 6715-13WT infected groups of specific pathogen-free weanling rats, consuming a diet which contains 56% sucrose, were topically treated with either demineralized water, dentifrice paste, or dentifrice powder. Both the paste and powder contained 0.22% (w/w) NaF. All topical treatments were given for one min daily per rat, five days a week. Animals' teeth were swabbed for recovery of 6715-13WT and total recoverable flora. Recoveries of 6715-13WT were 75% lower from both powder-treated and paste-treated dentifrice than from water-treated rats' teeth (p less than 0.001). Total caries scores were 45% lower for the powder-treated and 43% lower for the paste-treated rats than the water-treated ones (p less than 0.01). The major portion of the caries inhibition was evident on smooth tooth surfaces, 68% for powder-treated and 65% for paste-treated by comparison with water-treated animals (p less than 0.01), although a lesser caries reduction was also evident in tooth fissures (sulci). Patients with multiple overhangs were sent questionnaires six to 24 months posttreatment. Of the 77 patients that were mailed questionnaires, 59 returned the completed form. Analysis of the questionnaires showed that there was a highly significant increase (p less than .0001) in flossing frequency after the overhang removal treatment. This survey also found that 98% of the patients felt that overhang removal was a worthwhile dental procedure.
