The cases of two patients are reported. As far as we have been able to find out these are the first descriptions in the Dutch literature. {"title":"Venous hypertension of the hand caused by subcutaneous arteriovenous fistulae established for hemodialysis.","authors":"G Kootstra, M J Slooff, S Meijer, A M Tegzess","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Side-to-side arteriovenous fistulae established in patients on chronic intermittent hemodialysis can cause venous hypertension. This gives rise to pain, pigmentation and trophic ulcers. Four patients with these complications are described. In three patients, with a fistula between the radial artery and the cephalic vein, the thumb and the second and third fingers were affected. In the fourth patient, with a fistula between the ulnar artery and the basilic vein, the fourth and fifth fingers were involved. This finding suggests segmental venous drainage of the hand. All patients were cured by ligation of the distal vein. {"title":"Compression as a treatment of cancer, a historical survey.","authors":"G T Haneveld","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Compression in medicine goes far back in history. In ancient Rome and during the Renaissance compression by means of leaden plates was a well-known treatment of cancer. Compression flourished particularly in the first half of the 19th century, in Great Britain encouraged by the publications of Young and Arnott and Walshe, in France especially by the work of Récamier. Around 1850 this treatment fell into discredit: on the one hand because of improved surgical possibilities, on the other because not all the alleged cures concerned true malignancies. The results were, on the whole, disappointing. {"title":"Case report: ischemic enteritis in a patient with Candida albicans infection.","authors":"M A Cuesta, J C van Mourik, G den Otter, Y F Felderhof","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A case is reported of a 64-year old man with Candida albicans sepsis of pulmonary origin. It was complicated by a hemorrhagic ischemic enteritis that made it necessary to resect 2 1/2 m of small bowel. The specimen contained many pseudohyphae and yeast cells in the layers of the wall, especially in the necrotic areas. The superior mesenteric artery and its branches were patent and not occluded by infected thrombotic material, as has been described in another case of necrotizing enteritis accompanying Candida albicans sepsis. {"title":"Low-dose heparin prophylaxis in herniorrhaphy? A prospective trial in bleeding complications.","authors":"M Dommering","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Low-dose heparin (L.D.H.) prophylaxis gives good protection against deep venous thrombosis (D.V.T.). In the case of subjects presenting for herniorrhaphy the literature is less unanimous regarding the chance of wound hematoma. In this prospective randomised, matched trial in 86 patients, a wound hematoma incidence of 36 percent was noted in the treatment group as against 7% in the control group. {"title":"Pulmonary circulation pressures in surgical patients. The use of the Swan-Ganz catheter.","authors":"J J de Lange, L Stamenkovic, G den Otter","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Experiences with the Swan-Ganz catheter in 'high risk' surgical intensive care patients are described. After description of the catheter and catheterization technique, normal values from the literature are presented. Attention is drawn to the occurrence of artefacts in the pressure curve, which can lead to the inference of faulty results from the measurements. Attention is also drawn to the effect of intrathoracic pressure on pulmonary vascular pressures. A review of possible complications is given, with recommendations how these may be avoided or at least reduced to a minimum. The authors' material is used to demonstrate the value of the catheter, showing that the CVP is not always a trustworthy parameter for hemodynamic monitoring. {"title":"Bladder fistulae in Crohn's disease.","authors":"E J Lubbers","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Ten patients with various stages of enterovesical fistulization in Crohn's disease are discussed. These fistulae can develop at different times in the course of the disease, but may also cause the presenting symptoms. Cystoscopy and radiologic examination rarely visualizes the fistulae. Conservative treatment by medication can sometimes suppress the symptoms, but cannot cure.
