{"title":"Congenital partial absence of the left pericardium.","authors":"S Kostiainen,&nbsp;T J Maamies","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A 38 year old woman with congenital partial absence of the left pericardium is presented. The condition is fairly rare, usually diagnosed incidentally during intrathoracic operations or at autopsy. Eleven operatively corrected cases are reported in the literature. The present case was admitted because of transient attacks of chest pain, palpitation and dyspnoea. These attacks were brought on when she lay on her left side and were promptly relieved by a change of position. The chest radiograph revealed a prominence of the superior aspect of the left heart border and a slight laevo-position of the heart. A diagnostic pneumothorax on the left side confirmed the diagnosis of a pericardial defect. The defect was repaired by pericardioplasty. Operative correction of a partial left pericardial defect is indicated because of the danger of luxation of the heart out of the pericardium and sudden death. Diagnostic considerations and treatment.","authors":"A Hakkiluoto,&nbsp;M Lempinen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Small ulcer-simulating carcinomas accounted for 3.9% (48 cases) of the 1324 patients with carcinoma of the stomach treated at the Second Department of Surgery, University Centtive of a malignant ulcer in 52% of the cases whereas gastroscopy combined with exfoliative gastric cytology gave 73% correct results. In spite of preoperative suspicion of malignancy most patients were operated on as for a benign ulcer. Partial distal gastrectomy without splenectomy was performed in 93% of the patients. There was no hospital mortality. The thoracic duct was obstructed below the diaphragm. The lymph vessels in the left iliac and para-aortic areas were enlarged and there were lymphocysts. When the lymph loss was greatest the patient was in a state of grave malnutrition with marked hypoalbuminaemia and an absolute and relative lymphocytopenia in the blood. \"Malignant\" cells were demonstrated in the chylous fluid, but no malignancy could be found at laparotomy. It is possible that the cells were confused with immature lymphocytes. The lymphatic cysts were excised and the lymph vessels ligated. Decortication of the right lung was performed. The patient recovered. Postoperative semen analyses 2 weeks later showed azoospermia in only 2% of cases after vasectomy alone and in 25% of cases following vasectomy and irrigation, and 2 months postoperatively in 44% and 90%, respectively. Non-motile sperm cells in seminal analyses was the most common finding in both series 2 weeks after vasectomy. The problem whether the non motile sperm cells are viable or not is discussed. {"title":"Conservative surgery in acute pancreatitis.","authors":"G Fock,&nbsp;K Kyösola","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>During the years 1955-1970, 260 patients were treated for acute pancreatitis in the surgical department of the Maria Hospital. 62 of them were operated on, 44 electively, and 18 as emergency cases.
