{"title":"[Results of Montenegro's reaction in patients with mucocutaneous leishmaniasis, from Espírito Santo state].","authors":"P A Sessa, A Falqueto, G C Barros, J B Varejão","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors analysed the results of Montenegro Test in 402 patients with mucocutaneous leishmaniasis from Espírito Santo State, where the Leishmania braziliensis braziliensis has been isolated from humans cases and dogs. The test was positive in 371 cases (92.2%) before treatment. The Montenegro Test given during or after treatment changed the results in 17 of 20 negative cases. The remaining 3 cases and 9 which changed the results had positives biopsies. {"title":"[Infectious endocarditis in patients on periodic hemodialysis].","authors":"C Takahashi, E A Warrak, F Ruzany","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Nineteen patients originating from four hemodialysis centers with infectious endocarditis (IE) were studied during the period of 1985-1989. It was observed high proportion of patients with apparent normal cardiac valves preceding the IE; in 68.42% (13 out of 19 cases) there was an association with vascular access infection, the dialysis treatment time had a variation from 5 days to 6 years (median 19.5 months) and the interval between the initial symptoms and the IE diagnosis was from 2 to 30 days with an average of 5 days. The echocardiogram showed vegetation in 18 cases studied. The mostly affected cardiac valves were mitral in 42.1% and aortic in 31.5%. The predominant organism was S. aureus (75%). In 8 cases the primary focus was identified as the hemodialysis catheter and in 6 others as the arteriovenous fistula, one patient presented infection in both access simultaneously. The outstanding clinical manifestations were embolic (12 out of 19 cases) neurological (13 out of 19) and of cardiac failure (13 out of 19). The median hospitalization time was 24 days (4-55 days) and the mortality rate was 68.4%, the majority of sepsis. When compared with azathioprine/prednisone, CsA contributed significantly to a better graft and patient survival, either if used associated with prednisone of with azathioprine plus prednisone. CsA was also used in substitution to azathioprine in patients with hepatopathy attributed to azathioprine toxicity. The initial results are promising. The association of CsA and azathioprine with corticosteroids withdrawal was used as an attempt to allow normal growth in children. This seems to be the best choice of treatment for children. Careful monitoring of CsA blood levels avoids, or at least, minimizes nephrotoxicity. To achieve therapeutic CsA levels, patients with liver damage need lower, while children need higher oral CsA doses. {"title":"[Hematopoiesis: I. Cytokines involved in its regulation].","authors":"S R Dalmau, C S Freitas","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The study of the direct involvement of colony stimulating factors, interleukins, and other purified factors in distinct steps of hematopoiesis has been complicated by the in vitro presence of non-hematopoietic cells which can intermediate the effects observed on hematopoietic precursors. The review covers the recent finding that the CD34 antigen is expressed on the membranes of essentially all pluripotent stem cells, but is lacking in the majority of the differentiated blood and stromal bone marrow cells. This finding allowed in vitro experiments with selected CD34+ hematopoietic precursors, and a consequent reevaluation of the participation of different factors in their differentiation. {"title":"[Chilaiditi syndrome associated the volvulus of the transverse colon].","authors":"P R de Paula, A C Bartolomucci, D G Bassi, M A Silva","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The hepatodiaphragmatic interposition of the colon (Chilaiditi's Syndrome) and associated volvulus of the transverse colon is a rare cause of mechanical intestinal obstruction. The preoperative diagnosis is extremely difficult. We report a 26-year-old- male case, symptomatic since childhood, whose diagnosis by barium enema was confirmed surgically in Hospital Universitário de Taubaté. He underwent a transversectomy, and presented an intestinal obstruction in the post-operative period. Two reoperations were required. The time required for adequate therapeutic intervention was used as the main criterion of severity. Patients considered themselves to be emergencies in 16.6% of cases, whereas physicians thought so in only 2.6% (initial evaluation) and 1.0% (final evaluation) of cases. Sixty five percent of the patients could have been taken care of at a primary care site. The main reasons that prevented the use of a primary care unit were the more rapid and more convenient emergency room facilities. {"title":"[Importance of percutaneous venous catheterization in the identification of the origin of hyperandrogenism].","authors":"D R Franco, S Faiçal, C E Kater","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Selective percutaneous venous catheterization (SPVC) was performed to ascertain the source of androgen excess (AE) in 3 virilized women (12, 50 and 58y) in whom hormonal and radiologic investigation (ultrasonography and computerized tomography) was inconclusive. In all cases, elevated testosterone levels remained unsuppressed with dexamethasone, but were adequately suppressed by normal basal DHEA-S and cortisol. SPVC for T and F determinations (and the T/F ratio) were performed before surgery in all cases and during surgery in case 3. A T/F ratio of 152.4 and 71.8 (with an ovary-peripheral vein gradient [OPG] of 637 and 1173 ng/dl) identified the right and left ovary as the source of androgen excess respectively in cases 1 and 2, whereas the T/F ratio (87.4 and 97.2) and OPG (749 and 1162 ng/dl) were increased in both ovaries in patient 3. Thus, lateralization was evident in cases 1 (R ovary Leydig cell tumor) and 2 (L ovary hemangioma and stromal luteinization) whereas bilateral production in case 3 was confirmed by biopsy (PCO). Pre-operative identification of the origin of AE by SPVC is essential in defining proper treatment for patients suspected of harboring a neoplasia.
