Splenorrhaphy, with or without absorbable mesh, is a surgical procedure rarely used, although it preserves splenic function and has good outcomes. A 12-year-old girl accidentally fell from a height onto an iron bar. She complained of abdominal pain and difficulty breathing. Physical examination showed abdominal guarding and tenderness. Abdominal ultrasound demonstrated haemoperitoneum and splenic lacerations, confirmed by a CT scan, which highlighted active splenic bleeding. As she became haemodynamically unstable despite saline administration, we performed an absorbable mesh splenorrhaphy. She also received a blood transfusion. Surgical outcomes were good. She was discharged home on postoperative day 12. At a two-year follow-up, she is doing well. Absorbable mesh splenorrhaphy is an effective surgical procedure in children with haemodynamically unstable splenic trauma. Few paediatric cases are reported in the literature despite its ease of performance, its advantage of preserving spleen function, and its good outcomes.
","PeriodicalId":7519,"journal":{"name":"African Journal of Paediatric Surgery","volume":"23 1","pages":"64-67"},"PeriodicalIF":0.0,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149036072","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It presents as respiratory distress in the neonatal period and associated imaging findings of lobar hyperinflation. We report on a case from our institution to enhance diagnosis of this rare disorder. A 3‑week‑old male with acute respiratory distress, hypoxia, reduced unilateral breath sounds and pneumonia whose diagnosis was missed on the initial chest radiograph. Current foetal ultrasonography should be able to detect these cases, but they are missed and present with respiratory distress which may be due to infection or the primary disorder. Delay in diagnosis and treatment is associated with mortality from infection and the effects of compression of mediastinal structures. Surgery for symptomatic cases is preferred with good outcomes. Conservative management has been reported in asymptomatic cases, but the risk of missing a malignancy and not confirming the diagnosis by histology, especially for post‑natal diagnosed lesions remains.
","PeriodicalId":7519,"journal":{"name":"African Journal of Paediatric Surgery","volume":"22 3","pages":"161-163"},"PeriodicalIF":0.0,"publicationDate":"2025-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148954837","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While most hernias are unilateral, bilateral ones are usually recorded. In patients with bilateral hernias, the hernias on the contralateral side may not be clinically detectable at the time of the herniotomy. There has been the debate about the necessity and indications for exploring the apparently normal side during herniotomy. The aim of the study was to compare objectively the sensitivity of silk glove sign and ultrasound in detecting presence of a patent processus vaginalis (PPV).

Materials and methods: This is a prospective comparative study. A total of 79 patients with confirmed unilateral inguinal hernias were examined for the presence of patent contra‑lateral by checking for a positive silk‑glove sign. These patients subsequently had an ultrasound scan done on the same contralateral side by a different senior paediatric surgery resident trained in taking such scans with the supervision of a consultant radiologist (all blinded to the clinical findings). Patients who exhibited both positive clinical and ultrasound findings underwent herniotomies on the symptomatic side accompanied by contralateral exploration. Collated data were entered and analysed using the SPSS software version 27.0. P < 0.05 was considered statistically significant.

Results: The hernias occurred more on the right - 52 (65.8%) and 25 (34.2%) on the left. A total of 53 (67.1%) had hernias, while 26 (32.9%) had hydroceles. The sensitivity, specificity and diagnostic accuracy of the silk glove sign were 100.0% respectively, for all 4 patients that were positive, while the ultrasound scan had a sensitivity of 98.7%, specificity of 100% and diagnostic accuracy of 98.7%.

Conclusion: This study demonstrates that both the silk glove sign and ultrasound are equally effective in detecting the PPV in the contralateral groin of patients with unilateral inguinal hernia and hydrocele.
