Castelo","doi":"10.26502/fjsrs0046","DOIUrl":"https://doi.org/10.26502/fjsrs0046","url":null,"abstract":"Solitary Bone Plasmacytoma is a subtype of Solitary Plasmacytoma characterized by a mass of monoclonal plasma cells with involvement of bony structures. This pathology presents a high risk of progression to Multiple Myeloma with systemic involvement. In vertebral involvement it can lead to lytic lesions with progression to vertebral collapse and instability. The clinical case is a female patient with no medical history, with a clinic of low back pain with evolution for more than 1 month, radiography showed wedge of L4 and computed tomography a lytic lesion of that level, was hospitalized for pain control and study. The Magnetic Resonance came to discard the compression of noble parts. She was admitted for surgery due to vertebral instability and minimally invasive L2-L3-L5 posterolateral fixation was performed with transpedicular bars and screws, tissue biopsy and decompression with L4 partial facetectomy laminectomy. Surgery was uneventful and the patient was discharged 3 days after a complementary study of the disease. After 1 month, the patient showed clear improvement in pain and after half a year, the fixation was stable without instability or deformities. The patient was followed up by Orthopedics and Hematology. Plasmacytoma is an uncommon neoplasm and the main symptoms are low back pain, as in the case described. Radiotherapy is the treatment of choice, however surgery has been growing as an adjuvant and clinical improvement. We had found that young adult female mice deficient in CHL1 recovered better than their wild-type female littermates after thoracic Spinal Cord Injury (SCI). This observation was surprising, because CHL1 increases neurite outgrowth in vitro. Injury of adult mouse central and peripheral nervous systems upregulate CHL1 expression in neurons and astrocytes, consistent with CHL1's pro-active, homophilic interaction between CHL1 surface molecules in wild-type mice. After SCI, CHL1 expression was observed to increase in the glial scar, areas of axonal regrowth and remodeling of neural circuits. These observations were made only in females, and we therefore sought to analyze SCI in CHL1-deficient male mice. We now show that CHL1-deficient males did not recover better or worse than their male wild-type littermates. Primary and secondary lesion volumes were similar in the two genotypes, as seen in female mice which were studied in parallel with male mice. Assessment of peripheral leukocytes showed a significant increase in numbers of blood neutrophils at 24 h after SCI in males, but not in females. Lymphocyte numbers in mutant males increased slightly, but numbers of lymphocytes or monocytes did not differ significantly between males or females. Journal Abstract Introduction: Neck pain is one of the most common musculoskeletal disorder in the general population, second only to low back pain. Cervical radiculopathy is a dysfunction of a nerve root in the cervical spine resulting in producing radicular symptom such as pain, paresthesia, weakness, numbness in the upper extremity. Case Report: The aim of this study is evidence-based physiotherapy management of a single case of cervical radiculopathy. This single case was solved by using hypothetico deductive reasoning. Result: After 6 th week of intervention improvement was, found in pain reduction both in resting and during movement and improve range of motion. In addition, disability status also progressed from 60% to 20% Journal of Spine Research and Surgery Conclusion: Clinical reasoning is the foundation in our clinical practice. However, the false-positive fractions of these markers were 0.15-0.39 which frequencies were not negligible. These mean the delays of surgical inflammatory recoveries without SSI. This study aimed to identify and classify the factors associated with these delays. Methods: This retrospective study enrolled 320 patients who underwent spinal surgery for causes other than infectious spondylitis without surgical site infection. Demographic data, preoperative serological data, operative times, and operative methods were examined by multivariate regression for each SSI indicator and classified into related groups. Results: Nine significantly associated factors, age, malignancy, preoperative total protein, albumin, J Spine Res Surg 2022; 4 (1): 029-039 DOI: 10.26502/fjsrs0039 Journal of Spine Research and Surgery 30 white blood cell count, albumin/globulin ratio, Creactive protein, operation time, and use of spinal instrumentation were found. We classified the patients into three groups according to their preoperative nutritional status, immune-inflammation status, and surgical procedure. Malnutrition, high CRP and WBC, older age, and malignancy might be related to delays in surgical recovery without SSI; therefore, a single indicator may have limited capability in detecting SSI. Conclusions: We have to recognize the several causes to retard surgical inflammatory recovery. Habibur Rahman","doi":"10.26502/fjsrs0048","DOIUrl":"https://doi.org/10.26502/fjsrs0048","url":null,"abstract":"Objective: This study aims to evaluate the effectiveness of Dynamic Back Strengthening exercise along with conventional physiotherapy for chronic non-specific low back pain (LBP) patients over a conventional rehabilitation protocol for chronic non-specific LBP patients. Methods: Between October to December 2021 to a total of 8 patients with nonspecific LBP (5 males, 3 females; mean age: 41.12± years; ranged, 25 to 55 years) were included in this randomized - controlled pilot study. The patients were randomly assigned to experimental (n=4) and control (n=4) group. The treatment period was 3 days in a week for four consecutive weeks where pre & post assessment were done. Data were collected by using structured questionnaires related to CLBP and disability, socio-demographic data were collected through a semi-structured questionnaire including the Dallas Pain Questionnaire & Oswestry disability index (ODI). Results: In Mann Whitney 'U', the level of significance is greater than p = > 0.05, and there is no significant difference in between group analysis for all traits of the Dallas pain questionnaire and the Oswestry disability questionnaire. Conclusion: Effectiveness of Dynamic Strengthening Exercise along with conventional physiotherapy was the same in comparison to the conventional physiotherapy treatment for patients with CLBP. In these limited sessions, it has been found that the strengthening program could be started earlier but not for all kinds of patients. As the disability level has been improved by both groups, so it can be introduced earlier with the patients.
