Three age groups were investigated in the districts of Almaty and Almaty oblast: middle-aged (45-59 years old), elderly (60-79 years old) and the oldest (80 years and above). A high loss of teeth was revealed, which was 79.3% in the urban and 90.6% in the rural population of the Republic of Kazakhstan, respectively. Complete absence of teeth on the upper jaw was most common in rural and city dwellers in the oldest and elderly age groups. A high frequency of complete absence of teeth was revealed in elderly rural residents (44.9%). The production of removable prostheses with a double layer base improves the quality thereof. Indications for these prostheses have been determined. In conclusion, the need for orthopedic dental care was the highest in rural areas. Complete absence of teeth on the upper jaw was most common in the oldest and elderly age groups among rural and urban dwellers. A high frequency of complete absence of teeth was revealed among elderly rural residents (44.9%). Orthopedic treatment of persons with complete absence of teeth requires improvement of this assistance. The use of removable prostheses with a two-layer base improved the quality of manufactured prostheses, providing comfort and functional efficiency for patients. The first part included demographic data (age, gender, and educational level); the second part addressed the assessment of oral health of children by their caregivers during the lockdown, the reason and influence of the pandemic on the oral health of children; and the third part was related to caregiver's attitude towards healthcare system and teledentistry. Pearson's Chi-square test and Fisher's Exact test were used for analysis using SPSS (version 21.0).</p><p><strong>Result: </strong>Around 93.5% of caregivers were not satisfied with their child's oral health. Approximately 91.5% of caregivers agree that the coronavirus pandemic has negatively affected their child's oral health. Most of them think that alteration in sleep patterns is the main reason. Ninety-three percent of caregivers have been reported to utilize teledentistry during the pandemic period, but 53.2% did not know whether teledentistry could solve their child's oral healthcare problems. About 7% disagree that teledentistry can be helpful for the oral healthcare needs of children.</p><p><strong>Conclusion: </strong>Coronavirus disease has a negative impact on children's oral health. {"title":"SIRT1720 promotes survival of corneal epithelial cells via the P53 pathway.","authors":"Hayder AbdulHasan Jalil,&nbsp;Basma Talib Al-Sudani,&nbsp;Ghaith Ali Jasim","doi":"10.47750/jptcp.2022.906","DOIUrl":"https://doi.org/10.47750/jptcp.2022.906","url":null,"abstract":"<p><strong>Purpose: </strong>To investigate the protective role of SRT1720 (SIRT1 activator) against the oxidative stress caused by H<sub>2</sub>O<sub>2</sub> in the corneal cell line.</p><p><strong>Methods: </strong>Human corneal (2.040 pRSV-T) cell lines were cultured and treated with SRT1720 (as SIRT1 activator) and nicotinamide (NAM, a SIRT1 inhibitor), and incubated with H<sub>2</sub>O<sub>2</sub>. The expression level of SIRT1, p53, and acetyl-p53 was measured by western blot. Propidium iodine/annexin V-FITC staining, and flow cytometry was used to evaluate apoptosis. The trypan blue assay was used to assess the morphological modifications that occurred after the treatment, and Pifithrin-α (PFT-α) was used to inhibit the p53 pathway.</p><p><strong>Results: </strong>The investigation revealed that under oxidative stress, SRT1720 caused a reduction in acetyl-p53 expression and increased SIRT1 expression. It was also found that under oxidative stress, SRT1720 suppressed apoptosis. In comparison, NAM promoted cell apoptosis under oxidative stress. NAM's destructive effect was eliminated by PFT-α, a suppressor of the p53 pathway. PFT-α reduced the morphological changes in 2.040 pRSV-T cell lines compared to NAM treatment and inhibited apoptosis.</p><p><strong>Conclusions: </strong>The protective effects of the SIRT1 activator (SRT1720) indicate that H<sub>2</sub>O<sub>2</sub> induces oxidative stress-associated cell damage. {"title":"Prevalence and risk factors of early childhood caries in the Middle East region: A systematic review.","authors":"Sunil Babu Kotha","doi":"10.47750/jptcp.2022.937","DOIUrl":"https://doi.org/10.47750/jptcp.2022.937","url":null,"abstract":"<p><strong>Objective: </strong>The purpose of this systematic review was to assess the prevalence and risk factors of early childhood caries (ECC) in the Middle East region in the age group 0-6 years.</p><p><strong>Methods: </strong>A systematic literature search was performed in three major databases, Web of Science, PubMed, and Cochrane database, from January 1, 1960 to December 31, 2020, to identify the published literature on the prevalence and risk factors of ECC. All preschool children of 0-6 years of age irrespective of gender and socioeconomic status from the selected geographics were included.</p><p><strong>Results: </strong>A total of 26 full-text articles with a study population of 14,479, age group 0-6 years, were included in the study. The overall ECC prevalence in the Middle East region ranged from 26.50 to 99% in the 0-6-years age group. The mean decayed missing filled teeth ranged from 0.95 to 16.9, reported in studies from Jeddah, West Province, and Al-Jouf, Northern region of Saudi Arabia. In the United Arab Emirates, the prevalence ranged from 41 to 83%. The evaluation of the included studies revealed a total of 103 risk factors for ECC. Low socioeconomic status, dietary habits, poor oral hygienic attitude and behaviors, and poor access and availability of dental care services were the major risk factors. Most studies found low socioeconomic status, gender, age of the child, parents' education, low maternal education, and type of school to be significant risk factors for ECC. Children attending public schools were at a greater risk of having ECC. Dental mercury is the main issue for suspending the use of dental amalgam. Another restoration method, the composite restoration for posterior and anterior teeth for esthetic porous, became the alternative to amalgam.</p><p><strong>Aim: </strong>To measure patient satisfaction with two different materials based on multiple criteria using an oral health impact profile (OHIP) form.</p><p><strong>Method: </strong>This is a prospective study on two groups. The patients visiting the clinic with a vital posterior tooth indicated for restoration were requested to participate in the study. The first group received composite restoration of the posterior teeth. Contrarily, the second group underwent an amalgam restoration application. Patient satisfaction was assessed using the OHIP5 to assess different aspects of patient satisfaction. The patients were asked to fill out a form before starting the procedure, and after 4 weeks, the procedure was reported. The operators were requested to fill out their forms based on the procedure done to determine the participant eligibility criteria.</p><p><strong>Results: </strong>Overall 64 subjects were involved in the study among them 35 participants who received composite restoration, 48.5% were female, whereas 51.5% were male. Under other conditions, the patients who underwent amalgam application were 29, and 41.4% were female. It is the final grade of extensive liver fibrosis, in which the hepatic architecture is modified. Cirrhosis is a common disease worldwide and can be the end stage for several reasons such as obesity, non-alcoholic fatty liver, alcoholism, viral infection such as viral hepatitis, immune disorders, bile duct obstruction, and metabolic diseases. Alpha-fetoprotein (AFP) is defined as a protein secreted by the germinal yolk sac and liver. AFP level is used as a marker to diagnose inherited disorders and chromosomal anomaly, whereas the high-sensitivity C-reactive protein (hs-CRP) has a separate correlation with NAFLD. Therefore, hs-CRP can be used as a beneficial marker for identifying liver defects.</p><p><strong>Subjects and methods: </strong>Thirty participants with liver cirrhosis and 30 healthy participants as control (male and female) were enrolled. The participants from Baghdad, Iraq, were enrolled in this study. Blood and serum samples were obtained for the estimation of hemoglobin, serum AFP, and hs-CRP levels.</p><p><strong>Results: </strong>The pooled data of participants showed that hs-CRP and alpha-fetoprotein levels in the participants with cirrhosis were significantly higher than in the control group, P<0.0001. There were no significant differences in the sexes while considering alpha-fetoprotein, whereas hs-CRP levels were higher in males compared with females.</p><p><strong>Conclusion: </strong>This research shows a significantly high level of hs-CRP and alpha-fetoprotein in patients with liver cirrhosis compared with the control participants. Dental health is integral to a holistic health care need, and a robust dental education system is necessary. Dental education in India is mainly regulated by the Dental Councilof India, setting broad guidelines. Universities having dental colleges and institutes develop fine curriculum development and evaluation details. General and Dental Pharmacology and Therapeuticsis a crucial subject taught to undergraduate dental students during the second year of a 4-year duration course. A dental graduate should be well trained in general and systemic pharmacology and rational therapeutics principles. This has been set as an objective by the Dental Council of India. Sound knowledge of the drug action mechanisms, indications, adverse drug reactions, drug interactions and contraindications, evidence-based medicine, and rational use of adrug is core to the allopathic system. The practical exercises on human simulation or computer-assisted learning are critical for understanding pharmacology. The subject of pharmacology for dental graduates has been allotted 70 hours of theory and 20 hours of practicals with almost the same syllabus as medical graduates. This article highlights the areas of concern concerning the deficiency of teaching hours and needed improvement in the curriculum to make it competent to achieve its objective. The MOT in the present study's operating room employs analytical techniques, and moreover has complicated space needs. Any hospital that wants to implement MOT in the operating room must adhere to the regulations of the Indonesian Ministry of Health, as everything must adhere to a specific criteria. Additionally, MOT must fulfill the primary components required to ensure that the operational process proceeds properly and without technological difficulties. The criteria also includes the use of insulated doors (swing or sliding) to control the pathogens, bacteriostatic floors with corner covings, the operating table must be set in such a way that the laminar airflow reaches it, the use of modern operating tables, the adequate filtration of the air conditioning system and the air distribution system, and use of sterile corridors. The MOT in the operating room must also include the medical gas pipeline system (MGPS), flexible arm light pendants, precise indications on surgeon control panels and electrical panels, scrub stations, comprehensive utility pendants, as well as static/dynamic clean/dirty boxes. The operating room MOT requirements are specified in PERMENKES RI No. 1204/MENKES/SK/X/2004, and any hospital that wishes to construct one must adhere to them. Several requirements are outlined in the study that must be adhered to while manufacturing MOT. Tyrosine kinase inhibitors (TKIs) targeting BCR-ABL have transformed fatal CML into an almost curable disease. However, TKIs lose efficacy during disease progression, and the mechanism of CML progression remains to be fully understood. Additionally, common molecular biomarkers for CML progression are lacking. Our studies previously detected ANKRD36 (c.1183_1184 delGC and c.1187_1188 dupTT) associated exclusively with advanced phase CML. However, clinical validation of this finding was pending. Therefore, this study aimed to clinically validate mutated ANKRD36 as a novel biomarker of CML progression.</p><p><strong>Materials and methods: </strong>The study enrolled 124 patients in all phases of CML, recruited from Mayo Hospital and Hameed Latif Hospital in Lahore, Punjab, between January 2019 and August 2021. All response criteria were adopted from the European LeukemiaNet guideline 2020. Informed consent was obtained from all study subjects. The study was approved by scientific and ethical review committees of all participating centers.Sanger sequencing was employed to detect ANKRD36 mutations in CML patients in accelerated phase (AP) (n=11) and blast crisis (BC) (n=10), with chronic-phase CML (CP-CML) patients as controls (n=103). Samples were processed using Big Dye Terminator Cycle Sequencing Ready Reaction kits and sequenced using ABI Prism 3730 Genetic Analyzer, and sequencing using forward and reverse primers for ANKRD36.</p><p><strong>Results: </strong>During our study, 17% of CML patients progressed to advanced phases AP-CML n=11 (8.9%) and BC-CML n=10 (8.1%). The chronic- and advanced-phase patients showed significant difference with respect to male-to-female ratio, hemoglobin level, WBC count, and platelet count. Sanger sequencing detected ANKRD36 mutations c. 1183 1184 delGC and c. 1187 1185 dupTT exclusively in all AP- and BC-CML patients but in none of the CP-CML patients. Nevertheless, mutations status was not associated with male-to-female ratio, hemoglobin level, WBC count, and platelet count, which makes ANKRD32 as an independent predictor of early and terminal disease progression in CML.</p><p><strong>Conclusions: </strong>The study confirms ANKRD36 as a novel genomic biomarker for early and late CML progression. Further prospective studies should be carried out in this regard. ANKRD36, although fully uncharacterized in humans, shows the highest expression in bone marrow, particularly myeloid cells.
