Case Report: Mrs. A.D, 25 years old, admitted on 9/24/2018 for fluid flow through the operative wound, abdominal pain, fever, cough. Evolution 6 days. Onset: edema, redness, blisters of the abdominal wall then fluid flow through the surgical wound of a cesarean section (breech presentation) at 38 weeks, performed on 9/15/2018 in a medico-communal center in Conakry. Patient readmitted on D6: abdominal necrosis+blisters: dressing of the operative wound, Ampicillin 1 g, Perfalgan 1 g then referred to visceral surgery at Donka National Hospital. No specific background. No known allergy. Use of dermocorticoids for skin depigmentation for the duration of 2 years. G2, P2, Cesarean section 1, Alive 1, Died 1. Patient conscious, hypocoloured in teguments and conjunctivae: BP=110/70 mm Hg, pulse=104/min, FR=24 cycles/min, temperature=37.8°C. Abdomen: wound extending from the hypogastrium up to 3 cm above the umbilical bone, also involving the flanks, with necrotic background+foulsmelling whitish pus. TV: vulva covered with sero-hematic secretions. Leukocytes 16 G/l Hb 9 g/l VS accelerated. At laparotomy: necrotic areas of the skin at the parietal peritoneum, discharge of 1 liter of foul-smelling purulent fluid. Then midline above and below umbilical incision connecting the transverse incision: deposits of fibrin throughout the cavity, intact uterine sutures: removal of fibrin. Peritoneal toilet. Aponeurotic approximation. Stripping of necrotic tissue. Parietal washing, dressing. Peritoneal fluid: Staphylococcus aureus, ceftriaxone 1 g IV twice a day. postoperative follow-up: iterative necrosectomies, complete healing on postoperative eventration. Methods: Retrograde locked intramedullary nailing was used to treat the distal femur fractures of 20 patients (15 males, 5 females; mean age 47 years; range 25 to 69 years). Two patients had fracture of the distal femur and shaft. The fractures were classified as A1 (n=12), A2 (n=6), A3 (n=2), by the AO classification. There were two Grade I open fractures and 18 closed fractures. Three fractures were managed with percutaneous technique. At the mean first follow-up time of 18 months (range 15-48 months and mean second follow up at 27 months (range 12 to 68 months), the concluding functional results were assessed by using the modified Hospital for Special Surgery (HSS) knee assessment scale. Results: The average time to achieve union was 24.2 weeks (range 14 to 42 weeks). One patient had a delayed union (44 weeks). Five knees (25%) had normal joint range of motion, ten knees (50%) were having range of motion from 100° to 110°, three knees (15%) were having range of motion of 80°, and two knees (10%) were having range of motion below the 80°. According to the modified HSS knee scale, the results were excellent in eleven cases (55%), good in six (30%), moderate in two (10%), and poor in one case (5%). In two patients (10%), the postoperative radiographic examination revealed varus angulation (10°), healing took place with severe distortion in one subject. None of the patient had any deep infection or wound issues. Conclusion: Retrograde femoral nailing is an effective method for treating distal femoral shaft fractures. The healing rate of femoral shaft fractures fixed with a retrograde nail is the same regardless of fracture location, patient age, gender, or degree of comminution. Penetrating Keratoplasty (PKP) surgeries count for the most frequent efficient surgical management of diseased corneas. The latter are usually sent as button specimens for histopathology examination for further describing the pattern of the corneal disorders. Aims and Objectives: The aims of this study are to discuss the rate of Penetrating Keratoplasty (PKP) surgeries in SMC in 6 years interval (2016-2022) along with to determine the most common histopathological and microbiological diagnosis of corneal specimens from Penetrating Keratoplasty (PKP) and to study the clinical spectrum with age and sex distribution of corneal disorders in Bahrain alongside correlation with the comorbidities and the pathological diagnosis. Patients and Methods: It is a retrospective analysis of one-hundred and one histopathology reports of corneal button specimens at Salmaniya medical complex during the last 6 years (1-1-2016 till 31-3-2022). Medical records, surgical notes and histopathology reports of patients were retrieved from ISEHA system. Statistical analysis was done using Microsoft Excel 2019. Results: Corneal scarring (44%) was the most common pathological diagnosis, followed by Keratoconus (KC) (15%), acute keratitis (14%), degeneration (9%), chronic keratitis with fibrosis (5%), decompensated cornea and failed graft (5%), pseudophakic bullous keratopathy (3%) and herpetic keratitis (3%). There was Bahraini nationality (98%) and male preponderance (66%). The most common age group affected was 70–79 years with the median age of 51.5 year. Conclusion: Our study shows the importance of determining the most common histological diagnoses of corneal button specimens along with the correlation of the latter with the age, gender, surgical complications and systemic comorbidities. The chief complaint of a patient and his parents are crowded teeth upper and lower and anterior crossbite. “@I’m looking like a crocodile” were the first words of the boy sitting on the dental chair. The main task was functional but also esthetics because it was from the first important to him. The treated malocclusion is characterized with a narrow maxillary width and prominence of the mandible. The maxilla was deficient in anteroposterior and transverse dimensions, causing a slightly concave and unpleasant profile, a crossbite relationship of lateral incisors and upper and lower anterior crowding. The use of mechanics was preferred and accepted as a better solution for the patient according to the idea to generate well directed growth. The patient was 12 year-old boy with Class I malocclusion and anterior crossbite. A molar and canine class 1 and correction of the malocclusion were obtained. They have the ability to enjoy the simple things in life, which in most cases are not financially commensurate. The connection between the beauty of the smile, positive health and longevity has been consciously sought. All aspects of the life of the Bulgarian population are outlined, which are positive and could lead them out of the black rankings for morbidity and mortality in Europe and the world. This research was conducted on young adolescents to discover their awareness and perception. The study process involved using a questionnaire-based approach to 180 students visiting a primary care center in Riyadh. The following parts were included in the survey, the participant’s basic characteristics, knowledge about CPR, and perception of CPR. The questions about understanding and perception of the CPR procedure were delivered to the participants and their feedback was recorded. Out of the 180 participants, the majority (73.4%) were aware of the Cardiopulmonary Resuscitation procedure. A minority of 40.8% could comfortably provide the correct sequence of procedures when conducting CPR as indicated by the American Heart Association, the other 59.2% either had no idea of the process or got the sequence wrong. The study revealed that a majority of the participants had never attempted CPR; some had never been in situations where CPR was performed. The closest they came to help the victim was calling for help. Comparing the student’s views revealed that those with prior training were more willing to attempt the procedure on a victim than those without training. This is influenced by the surgeon’s capacity to recreate a geometrically normal and biomechanically stable hip joint. Mal-positioining of components can lead to various complications and the surgeons have to make vital decisions intra-operatively based on their experience. Materials and Methods: This study describes a simple instrument to aid alignment of femoral stem insertion and hence achieve the normal range of ante-version between 10 º-20 º. Experiments performed on a saw bone model by twenty orthopaedic surgeons, each making six attempts to insert a prosthesis demonstrated improved positioning when compared to unaided insertion. Results: The mean average femoral stem ante-version unaided was 18.3 º (range, 4.9 º to 36.6 º). Device aided ante-version was 14.4 º (range, 9.8 º to 18.9 º) (p<0.001). We also report the practicality of the use of the device which was assessed by use on cadavers. Pancreas is rare primary location for umbilical metastases. Pancreatic adenocarcinoma associated with SMJN usually arise from the body and tail of the pancreas. We report a case of SMJ nodule’s as a cutaneous manifestation of metastatic pancreatic head cancer. Case Report: A 59-year-old man, presented with epigastric, back pain and weight loss. On examination, an umbilical nodule was noted. Investigations confirmed a nodule of sister Mary Joseph as a pancreatic head cancer metastasis. Discussion and Conclusion: SMJN nodule remains as a rare tumor with metastatic origin of cancer most commonly digestive.
