An emergency operation was performed through median sternotomy and antegrade cerebral perfusion with moderated hypothermia. During the operation we found that the ascening aorta was dilated and the intimal were teared in mid ascending aorta and the dissection was extended throughout the whole aorta. The aortic root was dilated especially in the noncoronary sinus (Figure 1C) but the aortic leaflet was well preserved. We connected arterial line via 8mm graft anastomosed into the right subclavian artery through a seperated subclavian incision, and venous cannulation was performed with a single venous cannule into the right atrium. During establishing the moderate hypothermia, the ascending aorta was clamped, and cardia arrest was induced with retrograde cardioplegic infusion. On the other hand, there is still a concern that more recurrent MR is observed in surgical eligible patients treated with Mitra Clip. As a result, trans-catheter mitral edge-to-edge repair is reserved for subjects with high or prohibitive surgical risk. In the real-world practice, majority of subjects with advanced age, severely depressed left ventricular systolic function, or in critical conditions are put in medical treatment alone rather than either surgical or trans-catheter mitral intervention. Other etiologies such as infective endocarditis, Ebstein’s anomaly, carcinoid disease and blunt chest trauma are unusual [1]. The tricuspid valve lesions caused by blunt chest trauma are rare and often widely misdiagnosed. The term “contusio cordis” is represented by the cardiac myocyte damage, fibrotic or hemorrhagic, secondary to thoracic blunt trauma. Commotio cordis, meanwhile, involves electromechanical heart disorder. In most cases, leads to ventricular fibrillation, coinciding with a critical moment in the cardiac cycleabout 20ms preceding the T wave [2]. Both presentations are usually fatal [3]. {"title":"An Account of Recent Data on Smoking-Related Buerger’s Disease and Progression to Necrosis","authors":"Muhammad Ishaq Khan","doi":"10.19080/jocct.2018.11.555809","DOIUrl":"https://doi.org/10.19080/jocct.2018.11.555809","url":null,"abstract":"Objective: The objective of this review article is to briefly synthesize the most current information and data regarding smoking-induced Buerger’s disease, its occasional progression to gangrene, and its treatment and prognosis A literature search was conducted using Pub Med and Medscape databases for article published between 2013 and 2016 using keywords relevant to the objective of the review. After eliminating irrelevant results, the authors read each article in its entirety to determine applicability for inclusion. A more general web search for synthesis articles and government documents was also conducted. Results: Buerger’s disease is much more common among males than females and exhibits a dose-response relationship with frequency and duration of smoking. Early diagnosis, successful smoking cessation, and proper nursing care are associated with good prognosis. In cases where patients develop gangrenous necrosis associated with Buerger’s disease, prognosis is poorer. However amputation is not inevitable. Amputation is associated with complex infection and evidence of occlusion in the main arteries. It is concluded particularly a may allow tobacco companies to claim that products are not harmful. Cardiac magnetic resonance is the imaging modality of choice. However, it has limited availability or clinical contraindication.
