CABG was first introduced in 1964 and it has been firmly recognized since then as a key surgical intervention for patients with intractable angina pectoris despite medications and myocardial infarction [1,2]. Percutaneous transluminal coronary angioplasty (PTCA) or percutaneous coronary intervention (PCI) established in 1979 by Andreas Gruentzig in Switzerland to treat stenotic coronary lesions of coronary arteries, the procedure then progressed technically and introduced devices like stents which improve not only long-term patency of treated lesions but also the prognosis of patients [3,4]. With the use of both revascularization methods (CABG and PCI), the relief of symptom and survival rates have increased [5]. The relative efficacy, benefits and risks of the two procedures have been compared in several studies [6-12]. They constitute a medico-surgical emergency that involves both vital and functional prognosis. Material and methods: We conducted a retrospective and descriptive study on the vascular trauma of the limbs in civilian practice supported in the department of Thoracic and CardioVascular Surgery of the National University Hospital of Fann at Dakar. The study period is 7 years, from September 2010 to August 2017. Results: The number of patients was 107 with a sex ratio of 7.92. The average age of the population was 27.8 years [5 months-82 years]. The brawls and aggressions with the domestic accidents were the most represented etiological contexts with a frequency of 38.3% and 21.5% respectively. Vascular lesions by glass breakage (40.2%) and stabbing (32.71%) were mostly represented. Eighty-seven traumas affected the upper limb (81.3%) and the lower limb (18.7%). Arterial lesions was present in 97 patients (90.6%) while venous lesions was present in 48 patients (44.8%). The most affected arteries were the radial artery (33.6%), the ulnar artery (27.1%) and the brachial artery (16.8%). Sixty-five patients (60.7%) were received within 6 hours and another 24 (22.4%) after 6 hours. The main reason for consultation or hospitalization was the occurrence of externalized bleeding in 86% of cases. The immediate or distant occurrence of swelling traumatism is also a reason in 13.1% of cases.Signs of hypovolemic shock were found in 15 patients (14%). Seventy-one patients (66.4%) presented another lesions as nerve damage in 46 patients (43%), musculotendinous lesions in 64 cases (59.8%). Arterial and venous Doppler ultrasonography was performed in 5 patients (4.67%). Computer Tomodensitometry angiography was performed in 7 patients (6.54%). The management was medicosurgical. The types of procedure performed in our series in order of frequency are: 59 sutures per termino-terminal anastomosis (55.1%), 17 single lateral sutures (15.9%), 12 venous interpositions (11.2%), 9 arterial ligations (8.4%), 3 venous patch suturent (2.8%). Venous repair was performed in 46 cases (42.9%). It consisted of 28 ligations against 25 sutures, one by venous interposition. The immediate postoperative course was favorable in 86 patients (80.4%). The other 21 patients had complications such as neurological disorders (10.3%), infectious syndrome (8.4%), early thrombosis (2.8%), compartment syndrome (2.8%), a revascularization syndrome with electrolyte disorders (1.9%) and acute renal failure (0.9%). Six patients underwent reintervention (3 aponevrotomy, one leg amputation, femoro-popliteal bypass). Early mortality was 2 cases. It affects 0.7% of the world's population and is the leading cause of death due to high blood pressure. Objectives: Determine the prevalence and study the epidemiological, diagnostic and progressive aspects of hypertensive heart disease. Patients and methods: This was a descriptive cross-sectional study over a two-year period (July 1st, 2014 to June 30th, 2016) in the Cardiology Department of Sikasso Hospital for any patient hospitalized for hypertensive heart disease. Results: One hundred and three patients were selected, representing a hospital prevalence of 7.21%. The mean age of the population was 52 years (15-82 years) with a male predominance (63%) and a sex ratio of 1.7. Smoking was the most common cardiovascular risk factor associated with high blood pressure (71.7%). High blood pressure grade III according to the World Health Organization classification was found in 52.4%. The electrocardiogram showed isolated left ventricular hypertrophy in 89.3% of patients and associated with atrial fibrillation in 9.7%. Doppler echocardiography showed an altered left ventricular ejection fraction (less than 45%) in 58.25% of cases and left ventricular hypertrophy in all patients with a predominance of the concentric type (52.9%). Therapeutically, diuretics and conversion enzyme inhibitors were used in 73.7%, beta-blockers in 17.3% and calcium channel blockers in 9%. Complications were dominated by congestive heart failure (52%), arrhythmia (21%), ischemic stroke (4%). The average length of hospitalization was 5 days. The mortality rate was 1.97%. Conclusions: Hypertensive heart disease is a common complication of high blood pressure. In our countries, they are underestimated because of the insufficiency of the diagnostic means. The purpose of our study was to evaluate the management of cardiovascular emergencies at hospital de la Paix in Ziguinchor, to identify problems related to the caring and to propose solutions to improve it. Methods: We conducted a prospective 18-month study at the Emergency Department of the hospital la Paix in Ziguinchor. We included all hospitalized patients for cardiovascular emergency during the study period. The studied parameters were socio demographic data, antecedents, field, time of consultation, means of transport, time of care, main reason for hospitalization, medical diagnosis and evolutionary modalities. Results: The average age was 57.5 ± 15 years with female predominance (52.9%). Majority of our patients were of low socioeconomic status (53.6%). Transport mode was unsafe in the majority of cases (73.3%). Average consultation time was 10 days and the average time to take care was 2 hours and 30 minutes. Hypertension was the main cardiovascular risk factor (60.1%) and acute dyspnea, the main reason for hospitalization (56%). Cardiomyopathy was the main underlying pathology. A mortality rate of 26.8% and an average ICU length of stay of 4.32 ± 2 days were noticed. Conclusion: Cardiovascular emergencies are common. Primary causes for abrupt closure include dissection, thrombus formation or combination of both. Initial treatment for abrupt closure includes balloon redilatation and deployment of stent to stabilize a dissection. In unstable patients, a quick shift to Operation theatre with an Intra-aortic Balloon Pump (IABP) support could save the lives.
