In no area of children's foot problems is there less agreement. Even the definition has become clouded. The mechanism of the deformity is explained by a variety of conflicting theories and the etiology is ill-defined. Recommendations for management are based on supposed mechanisms and, therefore, vary widely also. In the first quarter of this century, orthopedic surgeons in France, Germany, and Italy began to write about a foot deformity that seemed to be distinct from clubfoot. By 1921 Bankart' in England took note of that work, realized that he and his colleagues had been seeing a similar deformity, and undertook to establish its identity in the English literature. Unlike true clubfoot, this entity had a normal or valgus heel. It had an adducted forefoot but lacked inversion of the sole. It tries to identify both key issues and the range of probable outcomes. It draws on knowledge of the past, analysis of the future prospects, and an appreciation of the limits to extrapolation. It will be revised with the help of numerous experts in the field between now and Spring 2000. It suggests scenarios that need to be considered. Since specific predictions of future events are \"always wrong\" due to inevitable changes in circumstances, it is preferable to think about the range of probable developments rather than improbable specifics. Recognition of the possible eventualities enables strategic thinkers to build flexibility into their plans so that they can smoothly accommodate to what actually happens. The scenarios focus on three main elements: 1. The levels of traffic -- passengers, cargo and aircraft; 2. The infrastructure that will be provided to accommodate these loads; and 3. The way these facilities will be managed. Traffic: Future levels of traffic are perhaps most questionable. While many Americans rarely if ever fly and the domestic market is far from saturated, there are many reasons to think that the rate of growth of US domestic traffic may continue its long-term decline. As seemingly small differences in assumptions about the rate of growth lead over a generation to enormous differences in totals, we must be very tentative about future levels of traffic. For example, by 2025 the number of enplaned passengers in the United States could be two or even three times the current 600 million a year. The composition of the total traffic is in any case likely to be significantly different from what it is today. The international components could be larger and more pervasive. Thousands of children each year are struck by moving motor vehicles; such collisions result in numerous hospitalizations and deaths. At particular risk are young school-age children between the ages of 5 and 9 years. Using a survey methodology, we collected data regarding the method by which children in an urban setting travel to and from school, in addition to the number of streets they cross in a typical school day. This information was compared with data from police records on street intersection locations of pedestrian collisions. There is a wide variation in the number of streets children cross in 1 day, calculated as the number of streets crossed in the entire day, not only those crossed to and from school. Children whose parents own a car and home cross an average of 3.7 streets per day, whereas children whose parents do not own both a car and home cross an average of 5.4 streets per day; this difference is highly significant (P < 0.0001). The largest differences in traffic exposure are between families reporting car- and-home ownership (x = 3.70 streets) versus those who do not own both a car and home (x = 5.39 streets) (Mann-Whitney = -5.5, P < 0.0001). There is a significant correlation between the proportion of children driven home from school and the rate of pedestrian injury in different regions of Baltimore. In areas where children are driven home, rates of pedestrian injury are significantly lower, whereas in areas where children walk home, rates of pedestrian injury are high (r = -0.79, P < 0.01). This study underscores the importance of adapting the child's environment to prevent injury. Interventions that alter the nature of the hazard are indicated.
