To determine the association of dyslipidemia with decreased glomerular filtration rate (eGFR) in patients with CKD KDIGO ≥3. Material and methods. Observational, analytical, longitudinal, ambispective study; was carried out at the Family Medicine Unit (UMF) No. 55 Zumpango, the sample size was calculated using the mean difference formula, with an expected mean of 5.2, obtaining n= 90 patients with CKD KDIGO ≥3 , between 20-69 years. Patients with dyslipidemia and without dyslipidemia were classified into 2 groups, in which creatinine was evaluated to calculate the estimated glomerular filtration rate (eGFR), total cholesterol and triglycerides, subjects with renal function replacement therapy, patients with pathologies or treatments that modify lipid metabolism. For the bivariate analysis, the Student’s T test and the Mann-Whitney U test were applied according to the type of distribution presented by the eGFR values for the groups with and without dyslipidemia. Result and conclusion. The most common is Chiari I malformation (CIM), which is characterised by a downward dislocation of the cerebellar tonsils through the foramen magnum. Symptomatic patients and those with syringomyelia are candidates for decompression of the Craniocervical junction. A variety of complications have been described in the literature following this procedure. We present the case of a cerebellar abscess following Craniocervical junction decompression for CIM in a patient with rheumatoid arthritis (RA), which, in our knowledge, is the first described in the literature. We believe that RA and its treatment were major contributors to this complication, mainly due to the immune impairment and the subtle infectious clinical manifestations that they cause. This case led us to modify the perioperative management of patients with RA who undergo spinal surgery in our department. In October 2012, we diagnosed a translocated ALK lung adenocarcinoma with pleural and bone metastases in a 42-year-old woman. After 16 months of chemotherapy, due to progression of the abdominal lymph nodes, the patient started Crizotinib. After 28 months, brain MRI documented progression of brain disease and a CT scan showed lung progression. The patient underwent whole-brain radiotherapy and Ceritinib was started but discontinued after six months due to grade 3 gastrointestinal toxicity. In June 2017, progression of meningeal disease was documented and the patient started Brigatinib. After 41 months the brain CT scan showed multiple lesions and Lorlatinib was started. The patient is still continuing this therapy while maintaining a good quality of life.
