The importance of administration of contrast medium in distinguishing between malignant lesions and other pleural diseases was given special attention. CT was found to be of value in diagnosing and establishing the extent of pleural lesions, especially in the mediastinal region. All malignancies exhibited high contrast enhancement following intravenous bolus injection of contrast medium. There were no features specific for mesotheliomas distinguishing these from other pleural malignancies. Some benign infectious lesions also showed high contrast enhancement. High contrast enhancement thus indicated malignancy only if an infectious lesion could be excluded. If contrast enhancement was absent or slight, the lesion was likely to be benign. Clinical follow-up may be regarded as justified in such cases. Eight patients had had surgical resection and reconstruction due to tumours of the tongue, floor of the mouth or mandible. Eleven patients had suffered from cerebrovascular disease. All patients had incoordination of tongue movements including defective initiation of pharyngeal swallow. In patients who had undergone surgical resection, oral dysfunction correlated with the extension of resection as well as type of reconstruction, and was more profound compared with the disorder present in patients with cerebrovascular disease. Moreover, muscular derangement due to surgery of the floor of the mouth interfered with the elevation of the hyoid bone, larynx and pharynx and thereby the pharyngeal stage of swallowing also became abnormal. {"title":"Mobile intraventricular cyst of the third ventricle treated with stereotactic puncture. The cyst was small so as not to appear as a significant expanding lesion on CT. The clinical history, however, suggested intermittent increase of the intracranial pressure. On CT there was some widening of the lateral and third ventricles, while the fourth ventricle had normal width. This finding in combination with the clinical history prompted further neuroradiologic examinations, including pneumoencephalography and ventriculography. The presence of a pedunculated mobile cystic lesion within the third ventricle was shown and its nature further elucidated by stereotactic puncture combined with contrast injection into the cyst. After emptying of the cyst, the patient has been free of symptoms during an observation time of 2 years. {"title":"Computed tomography in early stages of testicular carcinoma. Size of normal retroperitoneal lymph nodes and lymph nodes in patients with metastases in stage II A. Of these, 112 were in stage I (no metastases) and 44 in stage II A (metastases in normal-sized lymph nodes and in nodes with a maximum diameter in the transverse plane of less than or equal to 20 mm). The normal size of lymph nodes in young Scandinavian men was found to be less than 10 mm X 8 mm above the bifurcation, except in the area below the left renal vein to the left of the aortic midline where the normal size was found to be maximally 14 mm X 10 mm. The addition of a lymphangiographic contrast medium did not change the size of the lymph nodes significantly above the bifurcation, while changes of importance were noticed in the pelvic area. Normal size without contrast medium was found to be at the most 15 mm X 10 mm and after addition of contrast medium to the nodes 28 mm X 12 mm. The results of the CT findings in the stage II A group were not impressive but changed somewhat for the better using the new limits concerning size of retroperitoneal lymph nodes. It has been assumed that this impression implies a mechanical obstacle to the swallowed bolus. Therefore, the width of the pharyngo-esophageal segment was measured in three groups of individuals; 139 non-dysphagic volunteers, 25 dysphagic patients with a normal relaxation of the cricopharyngeus and 25 dysphagic patients with cricopharyngeal dysfunction. Compared with the other two groups of patients studied the patients with cricopharyngeal dysfunction were found to have a slightly wider gullet above and below the cricopharyngeal muscle. However, at the level of the cricopharyngeal muscle they had an equal sized or slightly narrower lumen than the patients in the two control groups. In 11 consecutive obese patients subjected to gastroplasty radiologic examinations of the stomach were performed before operation and one week, 3 and 12 months postoperatively. All stomachs were normal before operation. A significant decrease in pouch area and increase in stoma diameter were registered over the observation period. There were no statistically significant correlations between postoperative weight loss and stoma diameter or pouch area. Nor was retention in the pouch one week after gastroplasty related to weight loss. Radiographic evidence of gastroesophageal reflux was present in only one patient. The patients were examined with contrast enhanced CT and with positron emission tomography (PET). The lesions were partly calcified with a mild or no mass effect and a slight contrast enhancement at CT. There were signs of disrupture of the blood-lesion barrier also on radionuclide studies. PET with 11C-methionine and 11C-glucose showed a normal or decreased accumulation of the tracers. This combination of findings has not been encountered in intracranial tumours. As a comparison, one case of glioblastoma is described. In this patient, the CT findings suggested a cavernous haemangioma. Seven items were examined: Presence of a 'halo'; presence of cystic components; presence of calcification; hypoechogenicity; heterogeneity of the internal echo pattern: irregular or obscured margin; and massive extrathyroidal extension. Only the last-mentioned item appeared exclusively, but not very often, in malignant lesions, and the other characteristics were encountered in both benign and malignant lesions. {"title":"Quantitative metacarpal bone measurements before and after renal transplantation.","authors":"J Andresen, H E Nielsen","doi":"10.1177/028418518602700414","DOIUrl":"https://doi.org/10.1177/028418518602700414","url":null,"abstract":"<p><p>The outer (D) and inner diameter (d) of the second metacarpal bone, the combined cortical thickness (D-d), cortical area (D2-d2) and bone mass [D2d2)/D2) were measured in 74 renal transplant (RT) recipients at the time of renal transplantation and in a prospective analysis of 60 recipients after transplantation. The RT patient group was made up of recipients who after renal transplantation developed osteonecrosis or spontaneous fractures (RT-ON/SF) and an age- and sex-matched renal control group of subjects who did not develop these complications (RT-C). At the time of renal transplantation, in renal transplant recipient men and women, significantly reduced values in D, D-d and D2-d2 was noticed. These findings could be explained by a higher ratio of bone resorption than formation at the periosteal surface. Following renal transplantation, significant increases in d were seen with significant decreases in D-d, D2-d2 and (D2-d2)/D2, probably due to endosteal bone resorption, whereas D was unchanged compared with normal control persons. In the total group and in RT-ON/SF women, D decreased significantly and in ON/SF, increased significantly with significant decrease in bone mass compared with normal women whereas no significant changes in the parameters were seen in RT-C women. These findings indicate that bone loss after transplantation continues at the periosteal surface in women. {"title":"Image quality in mammography with special reference to anti-scatter grids and the magnification technique.","authors":"B Nielsen, G Fagerberg","doi":"10.1177/028418518602700420","DOIUrl":"https://doi.org/10.1177/028418518602700420","url":null,"abstract":"<p><p>Some of the parameters determining image quality in mammography are analyzed: the effects of primary photon spectra, focal spot size and screen-film systems on spatial resolution are discussed as are scattered radiation, development temperature and absorbed dose. The parameters limiting spatial resolution and contrast are evaluated for the standard and magnification techniques. Methods of reducing scattered radiation to improve contrast are evaluated. Scatter to primary ratios for different scatter reducing methods are compared, using the physical quantity energy imparted. For the standard technique the spatial resolution has been found to be limited by the fluorescent screen. With magnification technique the focal spot is the weakest link for the spatial resolution. The contrast is mainly set by the amount of scatter using the standard technique considering the use of a low tube potential (approximately 25 kVp). Using the magnification technique the amount of scatter is so small, that the tube potential is the limiting factor. We have found the optimized standard mammographic technique to be achieved under the following conditions: 25 kVp, 0.3 to 0.6 mm focal spot, film-focus distance 500 mm, anti-scatter grid, developing temperature 36 to 38 degrees C and 4 minutes total processing time with the screen-film system we have used. In magnification technique an air gap of at least 20 mm is desired. With an FFD of about 500 mm this will give a magnification ratio of 1.8 to 2.0 and a 0.1 mm X 0.1 mm focus spot is mandatory.
