Postoperative objective evaluation included whether the thumb was deflected, passive range of motion of the metacarpophalangeal (MP) joint, interphalangeal (IP) joint and the first carpometacarpal (CMC1st) joint. Radiographic examination was used to evaluate the force line of thumb, bone healing time and the development of the first metacarpal bone. Subjective evaluation was completed by the parents of the patients with functional and appearance satisfaction questionnaire. \u0000 \u0000 \u0000Results \u0000The follow-up period ranged from 2 to 36 months, with an average of 19.5 months. No deflection of thumbs occurred. The range of passive motion of MP, IP and CMC joints was not significantly decreased compared with the healthy side. Postoperative X-ray showed that the force line of the first metacarpal bone was good in 7 cases and malunion in 1 case. The bone healing time ranged from 5 to 7 weeks with an average of 6.2 weeks. There was no retardation in the development of the first metacarpal bone. The function of subjective satisfaction survey was excellent in 4 cases, good in 3 cases, fair in 1 case and the appearance was excellent in 4 cases, good in 2 cases, fair in 1 case, poor in 1 case. \u0000 \u0000 \u0000Conclusion \u0000On-top-plasty technique is an effective method to treat Wassel typeⅥ thumb duplications. Follow-up included wrist range of motion, X-ray, measurement of imaging parameters, complications, grip strength, pinch strength, PRWE and DASH scores. According to the recovery of dorsal extension angle and Gartland-Werley scoring criteria, patients with dorsal extension angle less than 45° were classified as groupⅠ (functional restricted group), and patients with dorsal extension angle more than 45° were classified as group Ⅱ (functional unrestricted group). Then, the differences of age, injury side, fracture type, imaging parameters, ulnar styloid process fracture and complications between the two groups were compared, and whether these factors were the influencing factors of different dorsal extension angles were judged. The effects of these factors on wrist PRWE and DASH scores were analyzed. \u0000 \u0000 \u0000Results \u0000There were no significant differences in age, injured side, fracture type, imaging parameters and whether there were fractures of ulnar styloid process between the two groups (P>0.05). There were significant differences in wrist flexion angle, ulnar deviation angle, pronation angle and grip strength between the two groups. The parameters of groupⅠwere lower than those of groupⅡ(P<0.05). Duan","doi":"10.3760/CMA.J.ISSN.1005-054X.2019.04.006","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1005-054X.2019.04.006","url":null,"abstract":"Objective \u0000To evaluate the clinical efficacy of the radial styloid process excision combined with bone grafting and internal fixation through radial approach of wrist in the treatment of old scaphoid fracture. \u0000 \u0000 \u0000Methods \u0000From January 2003 to September 2016, 62 cases of old scaphoid fracture nonunion were treated by the radial styloid process excision combined with bone grafting and internal fixation through radial approach of wrist. The wrist joint was immobilized with a plaster cast postoperatively. X-ray examination was performed 3 months after the operation. If the fracture was not healed, the plaster or brace should be used to continue to fix it until the fracture line was blurred, and the recovery of wrist function should be observed. \u0000 \u0000 \u0000Results \u0000All the patients were follow-up for 6 to 24 months with an average of 18 months. Bone healing was achieved in all the patients, with an average healing time of 6 months. The wrist function was evaluated according to the modified Mayo Wrist Function Score. The results were rated as excellent in 40 cases, good in 15 cases and fair in 7 cases, with the excellent and good rate being 88.7%. \u0000 \u0000 \u0000Conclusion \u0000It is simple and effective to treat old scaphoid fracture with the radial styloid process excision combined with bone grafting and internal fixation through radial approach of wrist. The surgical trauma is small. The basic situation of the mothers before pregnancy, during pregnancy and during childbirth was analyzed. \u0000 \u0000 \u0000Results \u0000The average birth weight of the children was (4 056.49±701.57) g, of which 61 cases had total brachial plexus injury. The average BMI before pregnancy of the mothers was (22.83±3.23) kg/m2 and the average weight gain during pregnancy was (13.68±3.41) kg. There were 80 cases of shoulder dystocia and 59.65% of the mothers delivered in county or township hospitals. \u0000 \u0000 \u0000Conclusion \u0000The birth weight, maternal weight before and during pregnancy, and delivery mode are all related to neonatal brachial plexus injury. The size of the flaps ranged from 1.0 cm×1.5 cm to 1.5 cm×3.5 cm. Full thickness skin grafts were used to repair the donor sites. \u0000 \u0000 \u0000Results \u0000The follow-up time ranged from 3 to 12 months, with an average of 4 months. All the flaps survived. All the incisions achieved primary healing. The appearance and texture of the flaps were good. According to the upper extremity functional evaluation criteria issued by the Hand Society of the Chinese Medical Association, the results were excellent in 11 cases, good in 4 cases and fair in 2 cases. \u0000 \u0000 \u0000Conclusion \u0000The transfer of finger lateral local flaps is simple to operate and has satisfactory clinical efficacy. The volar plate avulsion accompanied by articular surface avulsion fracture at the base of middle phalanx occurred. The volar approach was used to remove fragments and reconstruct the insertion of volar plate avulsion with mini-anchor. \u0000 \u0000 \u0000Results \u0000All the wounds achieved primary healing. The follow-up period ranged from 4 to 26 months with an average of 12 months. The flexion and extension of the proximal interphalangeal joint ranged from 50° to 85°. The joint was stable and had no lateral abnormal movement. There was no obvious pain and discomfort during the joint movement. According to the upper extremity functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association, the results were excellent in 2 cases and good in 3 cases. \u0000 \u0000 \u0000Conclusion \u0000The reconstruction of volar plate insertion with mini-bone anchor can provide the initial stability of the proximal interphalangeal joint, help early functional exercise, and help to reconstruct the basal articular surface of the middle phalanx. Among them, 1 case was accompanied with 1 tendon defect, 5 cases with 2 tendon defects, 8 cases with 3 tendon defects, 3 cases with 4 tendon defects, and 1 case with 5 tendon defects. There were 12 cases of single lobed flaps and 6 cases of bilobed flaps. 6 cases underwent secondary flap plastic surgery and tenolysis after operation. \u0000 \u0000 \u0000Results \u0000All the 18 flaps survived and the wound achieved primary healing. The donor site of thigh healed well with only linear scar. The follow-up period was 6 to 24 months with an average of 13 months. 14 cases of dorsal skin defects in hand were evaluated according to the criteria issued by the Hand Surgery Society of the Chinese Medical Association with the results being excellent in 6 cases, good in 7 cases, poor in 1 case. 4 cases of dorsal skin defects in foot had good functional recovery, no toe droop and no limitation of walking function. The appearance and texture of the flaps were excellent in 11 cases. No re-rupture or adhesion of extensor tendon occurred. 6 cases underwent second operation to thin the skin flaps 3 months after operation because of unsatisfactory shape. 4 of them had tendon adhesion after hand wound repair. The tenolysis was performed at the same time of skin flaps thinning. The appearance and function of the flaps recovered satisfactorily. \u0000 \u0000 \u0000Conclusion \u0000The single or bilobed free anterolateral thigh flaps with fascia lata in repair of dorsal skin and tendon defects of hand and foot have little damage to donor site. The defect area was 1.2 cm×1.8 cm to 2.3 cm×2.8 cm. All of them were repaired with fascial pedicle digital artery cutaneous branch chain flap. \u0000 \u0000 \u0000Results \u0000All the flaps survived after operation. And 1 of them developed swelling and blisters, and survived after lifting the affected limb and removing part of the suture. 15 cases obtained follow-up after operation and 1 case was lost. The follow-up time ranged from 6 to 24 months with an average of 14.5 months. The appearance, texture, elasticity and sensation of the flaps recovered well, with an average of 6.9 mm for two-point discrimination. The flaps carried 1 to 2 dorsal digital nerves and/or dorsal branches of one dorsal digital proper nerve. The proximal end of the flaps was designed as V-Y shape. Eight patients with middle and distal dorsal finger defects were repaired by distal advancement. The area of dorsal finger skin defects ranged from 0.5 cm×1.3 cm to 2.0 cm×1.7 cm. All the patients underwent one-stage emergency operation, and the donor sites of the flaps were sutured directly. The area of the flaps ranged from 2.0 cm×1.8 cm to 4.5 cm×1.7 cm. \u0000 \u0000 \u0000Results \u0000All the wounds achieved primary healing and the flaps survived well. The period of follow-up ranged from 3 to 9 months with an average of 4 months. The interphalangeal joint activity was normal and the sensation of dorsal finger flap recovered to S3+. \u0000 \u0000 \u0000Conclusion \u0000The V-Y advancement flap pedicled with dorsal cutaneous branch of digital artery with sensory nerve can obtain satisfactory function and shape for repairing the skin defects of the dorsal middle and distal segments of finger with sensory recovery. The operation is simple and the donor site can be closed directly. Among them, 20 cases were repaired with V-Y flaps with neurovascular pedicles (Group A), and 22 cases were repaired with dorsal branch of the terminal digital artery flaps (Group B). The operation time, complications, functional evaluation results and patient satisfaction scores were compared between the two groups. \u0000 \u0000 \u0000Results \u0000The postoperative follow-up time of group A was 5 to 25 months, with an average of 12.9 months, while that of group B was 5 to 26 months, with an average of 13.1 months. There was no significant difference in follow-up time between the two groups (P>0.05). The average operation time in group A was (27.1±1.9) minutes shorter than that in group B (38.0±4.9) minutes, and the incidence of complications in group A was 15.0% higher than that in group B 4.5%. The differences were statistically significant (P 0.05). According to the upper extremity functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association, the results were graded as excellent in 13 cases and good in 4 cases with the excellent and good rate being 85.0% for group A; the results were graded as excellent in 11 cases and good in 9 cases with the excellent and good rate being 90.9% for group B. The excellent and good rate of functional assessment for group A was lower than that for group B.
