In collaboration with the orthopaedic surgical team and the nursing colleagues in the post-anesthesia care unit (PACU), physical and occupational therapists concentrate on safe mobilization goals for patients recovering from total hip and knee arthroplasty procedures. A systematic review on the experiences and perceptions of nurses working the night shift in varied settings found the night shift work environment is different and night shift nurses juggle their sleep and deal with ongoing sleep deprivation. Data included pain site and intensity, physical function, fatigue, medication attitudes, and learned helplessness (LH) from patient self-report, and Disease Activity Score 28 (DAS28) from chart review. The overall pain intensity was 3.59 out of 10, with a median of 3. The mean number of painful joints was 8.4, with the most commonly reported painful joint groups being the fingers, wrists, and knees. Higher DAS28, impaired physical function, eight or more painful joints, higher levels of fatigue, and higher levels of LH were significantly associated with pain intensity. Our results suggest that pain in women with RA is closely associated with cognitive or psychological variables such as fatigue and LH in addition to inflammation itself. The occurrence of pain among patients with OUD is not uncommon; effective management is complex and requires a comprehensive approach developed in collaboration with patients, their families and specialists in substance use disorders. Most patients with OUD require long-term treatment. For many, this involves chronic medications including buprenorphine, methadone or naltrexone. First-line pain management therapy with nonpharmacologic or non-opioid pharmacotherapy, including multimodal strategies should be maximized first and tailored to underlying pain etiology. In some settings, patients may require additional opioid medications for pain control.
