Kajian Pengendalian Malaria di Provinsi Sumatera Utara dalam Upaya Mencapai Eliminasi Malaria

This study aim was to analyze the malaria situation in North Sumatera, subsequently taken into consideration for the preparation of a strategic plan to achieve the elimination of malaria in the region. Malaria case data, figures on Slide Positivity Rate, Annual Parasite Incidence, and plasmodium species were derived fromthe entire districts and cities of the North Sumatera province. Data were collected from 2010 - 2016. The data were analyzed qualitatively or descriptively. The results showed the Annual Parasite Incidence in North Sumatera had tended to decline from 0.91 per 1000 population in 2010 to 0.27 per 1000 population in 2016. The results showed the number of Slide Positivity Rate increased from 8.80 % in 2010 to 17.06 % in 2013 but decreased to 8.71 % in 2016. Target elimination of malaria in North Sumatera (2020) can be achieved when the discovery of cases of malaria, such as the enhanced efforts in intensification and extension, followed by improvement of malaria case management and vector control. \u0000Abstrak \u0000Provinsi Sumatera Utara termasuk daerah malaria penyumbang terbanyak kasus positif malaria di Pulau Sumatera pada tahun 2016. Penelitian ini bertujuan untuk menganalisis situasi malaria di Provinsi Sumatera Utara, selanjutnya menjadi bahan pertimbangan untuk penyusunan rencana strategis dalam tercapainya eliminasi malaria di wilayah ini. Data kasus malaria, angka Slide Positivity Rate, angka Annual Parasite Incidence, jenis Plasmodium  dan lainnya diperoleh dari seluruh kabupaten/kota di Provinsi Sumatera Utara. Data dikumpulkan dari tahun 2010-2016. Analisis data dilakukan secara kualitatif atau descriptive analysis. Hasil penelitian menunjukkan saat ini angka Annual Parasite Incidence malaria di Provinsi SumateraUtara sudah cenderung menurun dari 0,91 per 1000 penduduk pada tahun 2010 menjadi 0,27 per 1000 penduduk pada tahun 2016. Namun, angka Slide Positivity Rate dari pada tahun 2010 mempunyai kecenderungan meningkat dari 8,80 % pada tahun 2010 meningkat terus sampai 17,06% pada tahun 2013, walaupun menurun lagi menjadi  8,71 % pada tahun 2016. The aims of this study are to find out Lethal Concentration of non-polarextract from pomelo (Citrus maxima) leaf against mosquito larvae after 24 hoursexposure. Aedes aegypti and Culex quinquefasciatus larvae were tested in the study A.Larvicidal assay was using 20 larvae for each concentration in 100 ml solution with 5replications. The non-polar extract was tested at concentrations of 0 ppm, 500 ppm, 1375ppm, 2250 ppm, 3125 ppm, and 4000 ppm. The number of larvae mortality wasdetermined after 24 hours exposure. The dead larvae were counted and the data wasanalyzed using probit analyze. The results showed that non-polar extract from Citrusmaxima Leaf has potential larvicidal,  LC90 = 880 ppm for mortality of Aedes aegypti larvae, LC90 = 408  ppm for mortality of Culex quinquefasciatus larvae, the non-polar extract is more toxic against Culex quinquefasciatus than Aedes aegypti larvae. The non-polar extract from pomelo (Citrus maxima) leaf has the potential of being developed aslarvicides for mosquito control. \u0000Abstrak \u0000Penggunaan insektisida kimia untuk mengontrol nyamuk dapat menyebabkanresistensi pada populasi nyamuk. Tujuan dari penelitian ini adalah untuk menemukannilai Konsentrasi Letal (LC90 ) ekstrak non polar daun jeruk bali (Citrus maxima)  terhadap nyamuk setelah 24 jam. Larva yang digunakan dalam penelitian ini adalahlarva Aedes aegypti dan Culex quinquefasciatus. Kematian larva dihitung setelah 24 dan 48 jam.Uji larvasida menggunakan 20 ekor larva untuk setiap konsentrasi di dalam 100ml larutan ekstrak dengan lima replikasi. Ekstrak non polar diuji dengan konsentrasi 0ppm, 500 ppm, 1.375 ppm, 2.250 ppm, 3.125 ppm, dan 4.000 ppm. Angka kematianlarva dihitung setelah 24 jam paparan. Data dihitung dan dianalisa dengan analisisprobit. Hasil penelitian menunjukkan bahwa ekstrak non polar daun Citrus maximaberpotensi sebagai larvasida, Nilai LC90 = 880 ppm untuk kematian larva nyamuk  Aedesaegypti, Nilai LC90 = 408 ppm untuk kematian larva nyamuk Culex quinquefasciatus, Ekstrak non polar lebih toksik terhadap larva Culex quinquefasciatus daripada larva Aedes aegypti. Betle leaf contains tannins, terpenoids, flavonoids, and essential oils. Thepurpose of this study was to identify the effectiveness of Piper betle L as a larvicide against Musca domestica larvae. The experiment was a true experimental with a pre-test post-testwith control group design. Feeding assay method was used on 20 third instar larvae ofMusca domestica. Data were analyzed by using descriptive analysis, Kruskal Walls test,and probit analysis. The results showed the p-value for Levene test and Kruskal Walls testwere 0.028 and 0.014 respectively. Moreover, the LC50 was 0.05% and LT50 was 7.7 hours.There was a difference between negative control and all treatment concentrations. Itindicated that the betle leaf extract (Piper betle L) can be used as a bio-insecticide. \u0000Abstrak \u0000Lalat rumah (Musca domestica) sering terlihat di lingkungan rumah. Upaya yangdilakukan untuk pengendalian lalat rumah yaitu dengan penggunaan insektisida yangramah lingkungan seperti insektisida nabati. Daun sirih mengandung tanin, terpenoid,flavonoid dan minyak atsiri. Tujuan penelitian ini yaitu mengetahui efektivitas ekstrakdaun sirih (Piper betle L) sebagai larvasida larva lalat rumah (M. domestica). Jenis penelitian ini adalah true experimental dengan jenis disain  pretest posttest controlgroup. Objek penelitian ini menggunakan 20 ekor larva lalat instar III yang diberiperlakuan dengan metode feeding assay. Analisis data yang digunakan yaitu analisis deskriptif, analisis analitik dengan menggunakan uji Kruskal Walls, serta analisis probit. Penelitian ini dilakukan berdasarkan hasil uji normalitas dengan nilai p value0,002<0,05, pada uji Levene statistik nilai p value 0,028<0,05, uji Kruskal Walls nilai pvalue 0,014<0,05, dan uji Mann Whitney <0,05 pada kontrol negatif. Semua konsentrasiperlakuan, LC50 terletak pada konsentrasi 0,04827% dan LT50  pada 7,661 jam. Terdapat perbedaan antara kontrol negatif dengan semua konsentrasi perlakuan. By far, 430 anophelines have been identified but only 30-40 species have the potential to transmit malaria. A survey was carried to describe the distribution of anophelines species in seven community health centre (CHC) working areas including Caile, Ujung Loe, Bonto Bahari, Bonto Tiro, Batang, Bonto Bangun and Tanete in Bulukumba District, South Sulawesi. Human Landing Collection, Net Trap, and Light Trap was applied to collect adult anophelines mosquitoes. The Anopheles malaria vectors were observed in all areas. Various anophelines malaria vectors exist in Bulukumba District, including An. subpictus, An. barbirostris, An. hyrcanus, An. vagus, An. indefinitus, An. parangensis and An. kochi. Of which, Anopheles subpictus was the most abundant species and the majority of Anopheles mosquitoes were found in Ujung Loe area. Our study indicates that community-based programs and routine entomological surveillance, especially in high-risk areas, should be implemented to effectively control malaria transmission in Bulukumba District. \u0000Abstrak \u0000Malaria pada manusia ditularkan oleh nyamuk betina Anopheles yang berperan sebagai vektor yang membawa parasit Plasmodium. Terdapat sekitar 430 spesies Anopheles, 30-40 diantaranya berpotensi untuk menularkan malaria ke manusia. Penelitian ini bertujuan untuk menggambarkan distribusi vektor Anopheles di Kabupaten Bulukumba melalui survei entomologi (observasional) dengan pendekatan deskriptif. Sampel penelitian adalah nyamuk Anopheles di tujuh wilayah kerja puskesmas, yakni Puskesmas Caile, Ujung Loe, Bonto Bahari, Bonto Tiro, Batang, Bonto Bangun dan Tanete. Penangkapan vektor Anopheles dilakukan dengan metode Human Landing Collection (HLC), Net Trap, dan Light Trap. Hasil penelitian menggambarkan distribusi vektor Anopheles yang tersebar di tujuh wilayah kerja puskesmas yakni An. subpictus, An. barbirostris, An. hyrcanus, An. vagus, An. indefinitus, An. parangensis dan An. kochi. Anopheles subpictus merupakan spesies yang paling banyak ditemukan, sedangkan wilayah kerja Puskesmas Ujung Loe merupakan lokasi yang paling banyak ditemukan vektor Anopheles.
