{"title":"Neoadjuvant immunotherapy plus chemotherapy in high altitude natives with resectable esophageal squamous cell carcinoma in Tibet","authors":"Xizhao Sui , Duoji Danzeng , Ping Ni , Jiayi Geng , Pingcuo Gesang , Renqing Zhaxi , Yuling Wei","doi":"10.1016/j.asjsur.2024.11.134","DOIUrl":"10.1016/j.asjsur.2024.11.134","url":null,"abstract":"<div><h3>Backgroud</h3><div>Neoadjuvant therapy followed by surgery has been proved to improve the survival of patients with ESCC, and neoadjuvant chemoradiotherapy (nCRT) is the standard of care in most areas of the world. However, multimodality therapy including radiation therapy is actually limited in the current treatment of esophageal cancer in Tibet. The role of neoadjuvant immunotherapy in resectable esophageal cancer has been assessed in multiple phase II clinical trials, but there’s lack of evidence of applying neoadjuvant immunotherapy plus chemotherapy in Tibetan residents.</div></div><div><h3>Methods</h3><div>Patients with previously treatment-naïve, resectable ESCC were included in this study. The preoperative treatment regimen included Tislelizumab, nab-paclitaxel and carboplatin. Surgery was planned for every patient who completed neoadjuvant treatment. Surgical approaches and extent of esophageal resection was chosen depended on tumor location.</div></div><div><h3>Results</h3><div>23 patients with resectable ESCC were included from January 2022 to May 2024 in this study. Among all patients, 5 (21.7 %) had thyroid nodules or dysfunction. 19 of 23 (82.6 %) patients finished 2–3 cycles of treatment. 19 (82.6 %) patients experienced treatment-related adverse events (TRAEs), with 11 (47.8 %) patients experiencing grade 3–4 TRAEs. Thyroid toxicity of grade 1–2 was observed in 12 (52.2 %) patients. The objective response rate (ORR) was 69.6 %, and the disease control rate (DCR) was 100 %.。14 (60.9 %) of 23 patients underwent surgery. All patients underwent R0 resection. The pCR rate was 21.4 %. The median follow-up was 22.4 months. One of the laparoscopic procedures with rapid recovery, less chronic pain and less chance of infection is enhanced totally extraperitoneal (eTEP).</div><div>The eTEP technique is useful in bilateral hernias, large inguinal-scrotal hernias, incarcerated hernias, obese patients and patients with a short distance between the umbilicus and pubic tubercle. However, eTEP inherently involves extensive extraperitoneal dissection. Both bilateral hernia repair and the use of the eTEP technique reveal that the extraperitoneal space is even larger. This can be considered as a factor for mesh displacement. The aim of this study was to compare eTEP and totally extraperitoneal (TEP) techniques without mesh fixation in bilateral laparoscopic inguinal hernia repair.</div></div><div><h3>Methods</h3><div>Form January 2023 to June 2023, 40 consecutive patients with bilateral inguinal hernia were randomized into two groups; eTEP group (n = 20) and TEP group (n = 20) without mesh fixation.</div><div>Study was registered at <span><span>http://Clinicaltrials.gov</span><svg><path></path></svg></span> (NCT06070207). The meshes were marked with three radiopague clips. Pelvic radiographs was performed to evaluate the displacement of the mesh. The primary outcome of this study was mesh displacement. In addition, this is the first study in the literature to compare eTEP and TEP technique in terms of mesh displacement without fixation in laparoscopic bilateral inguinal hernia.</div></div><div><h3>Results</h3><div>There was no significant difference between the groups in terms of mesh displacement, recurrence, postoperative VAS scores, operation time, length of hospital stay, accidental pneumoperitoneum, hematoma, seroma formation and surgical site infection.</div></div><div><h3>Conclusion</h3><div>There was no more mesh displacement in the eTEP group in which more extraperitoneal space was created. Both bilaterality and without mesh fixation did not cause any difference in mesh displacement in both groups. Despite both bilaterality and larger extraperitoneal space dissection, the absence of mesh fixation in the bilateral eTEP technique is safe.
