This study was conducted to determine the visual perceptual skills (non-motor) of a group of adults in Selangor, Malaysia. A total of 120 subjects from Selangor with the age ranging from 20 to 59 years were randomly recruited in this cross-sectional study. This study involved distance and near visual acuity measurements using Snellen chart and Universiti Kebangsaan Malaysia (UKM) near vision chart, cognitive screening tests using the Mini Mental State Examination questionnaire (MMSE) as well as visual perceptual skills assessment (non-motor) using the Test of Visual Perceptual Skills (Non-Motor)- Revised (TVPS-R). The mean age of the subjects was 39.41+11.81 years old. Results showed that the mean standard scored obtained for Visual Discrimination (VD), Visual Memory (VM), Visual-Spatial Relationships (VSR), Visual Form Constancy (VFC), Visual Sequential Memory (VSM), Visual Figure-Ground (VFG) and Visual Closure (VC) subtests were 14.12+1.10, 13.42+1.58, 14.53+1.26, 12.47+1.03, 12.35+2.26, 13.67+1.64 and 13.73+2.23, respectively. Analysis of ANOVA shows that the VD, VM, and VSM scores are influenced by educational level (p<0.05). Pearson correlation showed a significant negative correlation between all the sub-tests of TVPS-R scores with age variation (p<0.05) except the VFC. Linear regression showed significant negative relationship between VD, VM, VSR and VSM with age. This research provided normative data of TVPS-R test on Malaysian adults for future reference and study.","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"21 1","pages":"37-46"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78873812","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We describe the case of a middle-age male, who developed early onset capsular bag distension syndrome of the right eye after phacoemulsification with implantation of plate haptic posterior chamber intraocular lens (Zeiss CT ASPHINA 509MP). There was persistent poor vision during follow up at 1 week and 5 weeks post-surgery with a myopic shift of 2.5 diopters sphere (DS). The intraocular pressure was only elevated at 2 hours post-operative and was normal during the subsequent followup. Distension of capsular bag was confirmed with the IOP Master 700 which showed abnormal separation of posterior capsule from the intraocular lens. He was diagnosed with capsular bag distension syndrome secondary to retention of viscoelastic. Right eye removal of retained viscoelastic was performed and the capsular bag distension syndrome resolved successfully with best corrected visual acuity at 6/6. Hence, complete clearance of viscoelastic during phacoemulsification with plate haptic intraocular lens is important in order to prevent capsular bag distension syndrome.","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"82 1","pages":"306-312"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85363236","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is also associated with dissection involving the root of aorta and coronary vessels. We report a case of young male with hypertension, who presented with severe chest pain and unilateral lower limb pain. Physical examination of the left lower limb was consistent with acute limb ischemia. Electrocardiogram revealed acute anterolateral myocardial infarction together with ST elevation in aVR. Bedside transthoracic echocardiography showed a dilated aortic root measuring 4.51 cm with presence of intimal flap which raised the suspicion of dissection of root of aorta and left coronary artery. Computed tomography angiogram revealed aortic dissection from the root of aorta including the intimal flap near the origin of the left coronary artery, down to common iliac extending to the left iliac artery. Unfortunately, the patient opted for non-surgical intervention and succumbed 48 hours later. This case highlights that in case of aortic dissection, which presents with malperfusion syndrome, the presence of ST segment elevation at lead aVR should raise the suspicion for extensive aortic dissection involving the aortic root and left coronary artery which signifies unfavourable outcome","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"127 1","pages":"297-305"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"74571772","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to translate the English version of the original Chicago Lead Knowledge Test (CLKT) into the Malay (local) language systematically and to suit the background culture and language of this country. The steps of systematic translation followed the International Society for Pharmacoeconomics and Outcome Research (ISPOR) standard which consists of preparation, forward translation, reconciliation, backward translation, backward translation review, harmonisation, cognitive debriefing, review of cognitive debriefing and finalisation, proofreading, and final report. The total number of items remained at 24, where one item was deleted, one item was revised, and one item was added into the instrument. All four themes; general knowledge, exposure, prevention and nutrition in this questionnaire were maintained. The response scale, answer scheme, and the scoring system remained the same. Most of the English items had direct translation to Malay language except for items 5, 7, 12, 20 and 22. The mean (+SD) and interquartile range (IQR) for total knowledge score was 9.50 + 2.45 marks and 5 marks, respectively. The lowest score was 4 marks and the highest score was 13 marks. This was be the first known study to utilise the systematic and standardised approaches in the cross-cultural translation and adaptation of the CLKT in Malaysia. The CLKT’s cultural and linguistic adaptation in Malaysia may contribute to other current research particularly on lead exposure among children.","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"10 1","pages":"187-214"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76803299","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The number of tuberculosis (TB) cases in Malaysia appears to be increasing and there were 500,000 Malaysians diagnosed with COPD in 2016. A substantial number of PTB patients go on to develop post tubular airway disease. Past infection with TB has been shown to contribute to the aetiology of COPD. We conducted a one year cross-sectional study in outpatient setting involving adult subjects in Universiti Kebangsaan Malaysia Medical Centre (UKMMC) with a history of pulmonary tuberculosis three years prior to the study. We excluded subjects with bronchial asthma, COPD, interstitial lung disease and bronchiectasis. Airflow obstruction was defined as FEV1: FVC ratio <0.70. The COPD Assessment Test (CAT) questionnaire was used. Eighty-two subjects were recruited with a median age of 52.5 years (IQR 36-62). Male predominance (56.1%) with 29 (35.4%) smokers. Eighteen (22%) subjects had airflow obstruction. Six (33%) out of these subjects were smokers. Fourteen(17%) had restrictive pattern and 50 (61%) had normal spirometry results. Five subjects (22.22%) had mild obstruction, 7 (44.44%) moderate obstruction, and 6 (33.33%) had severe obstruction. There was a positive correlation between chest X-ray abnormalities and age with airflow obstruction (P<0.05). Abnormal spirometry pattern was found in 39% of subjects. Post TB-COPD incidence was 22% in those who completed TB treatment. There appeared to be an association between abnormal chest radiograph findings with airflow obstruction in subjects with history of PTB.","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"208 1","pages":"85-95"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89103275","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This research analyses the health-related quality of life and the relative factors of type 2 diabetics in Tam Binh District, Vinh Long Province, Vietnam. A cross-sectional study was administered from July-August 2019 at seventeen communes in Tam Binh district. All total of 500 patients diagnosed type 2 diabetes and aged 35 years and older, comprised this investigation. The exploration questionnaire encompassed the Vietnamese Diabetes Quality of Life. Diabetics had an average age of 59.77 + 9.81 years. The health-related quality of life score was moderate at 60.15 + 21 points. The inter-personal relationship had the lowest score. However, the highest score was detected for the physical endurance. The significant differences could be distinguished at different individual components like age, marital status, location, family type, education, occupation, income, duration, glycaemia, HbA1c, hypoglycemia, alcohol-using, treatments, and other issues. The study highlighted that type 2 diabetes had a negative impact on health-related quality of life. Fifty-six patients with Body Mass Index (BMI) <35 kg/m2 who underwent lower abdominal gynaecological surgery were randomly recruited into two groups. Group I received one ampoule (2.47 g) of magnesium sulphate and Group II received 50 mg/kg magnesium sulphate (based on body weight), pre-operatively. Pain score and patient controlled analgesia (PCA) morphine requirement were compared at 30 minutes, 12 hours and 24 hours post-operatively. The pain score was comparable at all intervals between the two groups (30 minutes, p = 0.450; 12 hours, p = 0.402; and 24 hours, p = 1.000). Post-operative PCA morphine requirement was not statistically significant between the two groups at 30 minutes, 12 hours, and 24 hours (2.7 vs 2.4 mg, p = 0.545; 12.5 vs 9.8 mg, p = 0.154; 7.7 vs 6.4 mg, p = 0.323). The side-effects of magnesium sulphate on blood pressure, heart rate and sedation were not statistically significant between the two groups. In conclusion, the analgesic effects of pre-emptively administered intravenous MgSO4 of 2.47 g (one ampule) was comparable to 50 mg/kg in patients with BMI less than 35 kg/m2 following lower abdominal gynaecological surgery under general anaesthesia with negligible side effects.","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"80 1","pages":"164-174"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81233137","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Critical thinking dispositions development is supported with sufficient self-esteem. The present study aimed to measure the level of critical thinking disposition and self-esteem among undergraduate nursing students in a public university in Malaysia. A cross-sectional study was carried out in a university involving 99 students. Data were collected by self-report questionnaires and analysed quantitatively. A total of 97% of the nursing student had a high level of critical thinking dispositions. Almost¾3/4 of the students had a moderate level of perceived self-esteem (74.7%). The critical thinking dispositions were significantly related to perceived self-esteem (p<0.044). Overall, the nursing student of this study had high critical thinking disposition and moderate self-esteem, which is very important for them to become a good nurse with critical thinking skill in the future.","PeriodicalId":53946,"journal":{"name":"Medicine and Health","volume":"7 1","pages":"9-16"},"PeriodicalIF":0.2,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89300455","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
