Heesen","doi":"10.14624/nr2008005","DOIUrl":"https://doi.org/10.14624/nr2008005","url":null,"abstract":"Multiple sclerosis and physical activity: A German-wide survey on the behaviour, barriers and desires of those affected Background: Physical activity (PA) has beneficial effects on many disease related symptoms in people with multiple sclerosis (pwMS). Physical parameters – such as muscle strength, endurance, and balance – as well as fatigue, depression and neurocognitive functioning, can be positively influenced by regular PA. Nevertheless, pwMS are less physically active than the general population. The reasons for this are not well understood. Hypothesis: We hypothesized that beside personal factors, environmental factors such as transport and access to exercise facilities probably are responsible for the widespread physical inactivity among people with MS. The perception of environmental barriers could be influenced by the place of residence (city/rural area) or the degree of physical impairment. Methods: Between April and October 2017 we examined the perception of PA and exercise options among pwMS from different regions in Germany. Using an online survey, we collected data from 630 pwMS. Results: Our data indicate that pwMS in rural areas gave lower ratings for their PA and exercise options than those in cities (p<0.001). In rural areas, environmental barriers were more often mentioned as a reason for physical inactivity. In addition, MS patients with greater disability rated their PA and exercise options lower than those with less severe disability (p<0.001). The degree of physical impairment increases the perception of environmental barriers. Among physically active people with MS, the mean acceptable travel time to exercise facilities was 17 minutes, with a high variance. Surprisingly, this did not differ significantly between city and rural areas. The mean acceptable cost for sporting facilities was € 46.01 per month, also with a high variance. The three most desired activities were: swimming (13.9%, n=59), relaxation techniques (12.2%, n=52), and dancing (8.2%, n=35). Conclusion: Our study provides information on the perception of PA and exercise options among people with MS in different regions of Germany. Flachenecker","doi":"10.14624/nr2008004","DOIUrl":"https://doi.org/10.14624/nr2008004","url":null,"abstract":"Influence of fatigue and cognitive impairment on the use of health services, work performance and quality of life (utility): A study with 5,475 patients with multiple sclerosis in Germany Introduction: People with multiple sclerosis (MS) experience a number of neurological problems beyond the general outcome measures of relapses and disability that can independently affect costs and quality of life. Objectives: To investigate the effect of self-assessed fatigue and subjective cognitive impairment on direct healthcare consumption, work participation and utility in Germany. Methods: The study included 5,475 participants and investigated – in addition to all resource consumption – fatigue, cognitive impairment and the effect of MS while at work using visual analogue scales (VAS 0–10). The analysis controlled for gender, age, disease duration, education, disability, and use of disease-modifying treatments. Results: The level of fatigue and cognitive impairment was significantly and independently correlated with all resource utilisation, workforce participation and utility (p<0.0001). For each VAS point increase in severity, the probability of working was reduced by 10.6% for cognitive impairment and 4.9% for fatigue, while utility decreased by 0.03 for each point. Work hours decreased in linear fashion with each point of increasing severity for both symptoms, while sick leave increased accordingly. Both symptoms significantly affected productivity at work. Conclusions: The effect of fatigue and impaired cognition on work capacity has been shown in an international study, but demonstrating their effect on resource consumption requires a large single-country sample. Die Studie adressiert daher folgende Fragen: a) Welche kognitiven Störungen weisen Schlaganfallpatienten zu Beginn der Rehabilitation auf? b) Wie verändert sich die Mobilität im Rehabilitationsverlauf? c) Welche kognitiven Störungen beeinflussen die Verbesserung der Mobilität im Rehabilitations-verlauf negativ? Im Ergebnis zeigte sich, dass ein Drittel der Schlaganfallpatienten zu Rehabili-tationsbeginn unter kognitiven Störungen, vor allem im Bereich der Exekutivfunktionen, leidet. Dabei haben insbesondere Einschränkungen im konvergenten Denken einen negativen Einfluss auf eine Mobilitätsverbesserung im Verlauf der neurologischen Rehabilitation. Schlüsselwörter: Schlaganfall, Neurorehabilitation, Kognition, Exekutivfunktionen, Mobilität The impact of cognitive dysfunctions post stroke on the mobility during the neurorehabilitation process Abstract Cognitive impairment post stroke is common and usually persists longer than motor impairment and may influence the rehabilitation process, because cognition and motor learning are related. Therefore, this study addresses the following questions: a) Which cognitive disorders appear in stroke patients at the beginning of rehabilitation (admission)? b) How does mobility change during the rehabilitation process? Which cognitive disorders negatively influence the improvement of mobility during the rehabilitation process? The results reveal that one-third of stroke patients suffer from cognitive disorders at admission, especially in the domain of executive functions. Particularly impairments in convergent thinking negatively influence the improvement in mobility during the neurological rehabilitation process. Therefore, the question arises whether these symptoms are intrinsically associated, and if so, why. Methods: We conducted a systematic analysis of the available international literature. Specifically, we recorded and compared the number of included patients, age, gender distribution, expanded disability status scale (EDSS), disease type, study design and assessment methods for autonomic dysfunctions and fatigue. Results: A total of 14 studies were included. The results of these questionnairebased studies indicate a significant association between autonomic dysfunctions and fatigue. In addition, studies including standardized autonomic tests (e.g., the Valsava manoeuvre) showed an association between autonomic dysfunctions and fatigue. This was confirmed by studies using heart rate variability to measure the activity of the autonomic nervous system, as evidence of increased parasympathetic nervous system activity related to fatigue. Discussion: Autonomic dysfunctions and fatigue are significantly associated with each other. The results support a common third cause of this association and the central importance of the vagal nerve, which controls autonomous functions and is involved in the feeling of fatigue due to inflammatory processes.
