{"title":"[Antibiotic Resistance and Serotype Pattern of Streptococcus pneumoniae Isolated from Children in St. Petersburg in 2010-2013].","authors":"O S Kalinogorskaya,&nbsp;S S Belanov,&nbsp;M O Volkova,&nbsp;V V Gostev,&nbsp;S V Sidorenko","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The surveillance of the serotype pattern and antibiotic resistance of S. pneumoniae in various geographical regions is required for the validity of rational etiotrophic therapy of pneumococcal infections and the choice of the optimal vaccines for their prophylaxis. 250 S. pneumoniae isolates from children with acute otitis or pneumonia and healthy carriers in St. Petersburg in 2010-2013 were investigated. The analysis of the serotype pattern of the pneumococci showed that 13-valent conjugate vaccine was the most active (86.1% of pneumococci causing pneumonia and 86.4% of pneumococci causing acute otitis). The isolates were higly resistant to beta-lactams and macrolides. By the EUCAST criteria, the decrease in the susceptibility to penicillin, cefotaxime, erythromycin and ceftarolin was observed in 32.4%, 14%, 33.2 and 6% of the isolates respectively. 22.4% of the isolates showed associate resistance to penicillin and erythromycin.. No resistance to moxifloxacin was detected. The frequency of resistance to tetracycline, co-trimoxasole and chloramphenicol in various patients ranged within 30-50%. According to the timing of the CT-verification the patients were divided into 2 groups: \"early\" visualization (EV 50.2%) and \"late\" visualization (LV, 49.8%). In the EV group the lesion was visualized by CT on the admission (3-6 hours after the admission), in the LV group it was visualized only when CT was repeated 2-3 days after the admission. The patients in the LV group demonstrated increased severity of the underlying pathology, increased mortality and unfavourable functional outcomes correlating with the stroke lesion volume vs. the EV group. The maximum position of MRSA was recorded in the ICU (38 to 75% of the strains). The MRSA strains were characterized by higher methicillin resistance, mainly from the cardiosurgical units (60 to 91% of the isolates). All the variants of the laboratory and clinical ERCC1 assessment in the ovarian cancer tissue (single nucleotide polymorphisms of the ERCC1 gene, levels of mRNA or protein) are considered. Data on the prognostic and predictive value of ERCC1 as a marker of the response to platinum-based therapy in ovarian cancer are systematized. The authors discuss the possible causes of heterogeneity of the results and emphasize the necessity of a unified and integrated approach to evaluation of ERCC1 in the tumor. Genetic polymorphism due to the activity of N-acetyltransferase (N-AT) is common in the majority of human populations. Consequently, persons with \"slow\" or \"fast\" acetylation phenotype should be identified. N-AT catalyzes acetylation of a number of medical products. Efficiency of pharmacotherapy is mostly associated with the specific features of medical products biotransformation. The processes of biotransformation with participation of acetyltransferase, monooxygenase or other ferment systems are under the gene control. The aim of the study was to characterize the features of the clinical course of acute respiratory infection complicated by pneumonia as dependent on the acetylation phenotype to predict the character of the disease and optimize the used antibiotic therapy among the native population (Yakut) and the arrived (Russian) in the Far North Regions (Sakha, Yakutia). 112 children with acute respiratory infections complicated by pneumonia and 49 practically healthy ones were examined. For the children with low N-AT activity (less than 30%) it was recommended to be treated with gentamicin which directly takes part in the acetylation and provides the antibiotic therapy efficiency in 80% of the cases. Marine algae and in particular their extracts and obtained from them sulfated polysaccharides are significant sources of natural antioxidants. The recent data on the effect of the extracts and sulfated polysaccharides of seaweed on the functional activity of the liver with injuries induced by CCl4, some drugs (paracetamol, diclofenac), N-nitrosocompounds, aflatoxin are presented in the review. Particular attention is paid to the effect of sulfated polysaccharides and in particular fucoidans on the functional activity of the liver in patients with chronic viral hepatitis C. Fucoidan is highly safe and active not only as an antioxidant but also as an inhibitor of HCV replication, has antiinflammatory and immunomodulating effects. The effect of the concentrate on the mycoplasmas growth was investigated for the first time. Two representatives of Mycoplasmafaceae, i.e. Mycoplasma hominis and Mycoplasma fermentans and one representative of Aholeplasmataceae. i. e. Aholeplasma laidlawii were used. It was shown that the fermentation broth inhibited the growth of Mycoplasma hominis after the preliminary exposure.
