Materials and methods: Data collected and analyzed retrospectively from COVID-19-positive pregnant women admitted to our hospital between April and September 2020. Results: 78.85% of women below 30 years of age, 60.57% multigravidae, 53.71% above 37 weeks of gestation. 76.85% no comorbidities, 73.42% no history of travel/contact with COVID-19 cases. 84.57% asymptomatic, two had a severe acute respiratory infection (SARI), requiring intensive care. 74.57% diagnosed using reverse transcriptase-polymerase chain reaction (RT-PCR). 91.42% turned negative within 6–10 days. The SARI patients required intensive care unit (ICU) care, one expired. 81.42% discharged between 8 days and 14 days of admission, returned for delivery at term. 83.42% required no medication. 96.28% had normal investigation reports. 83.14% of patients discharged, 16.57% required emergency obstetric intervention. Cesarean section rate 52.28%, the previous cesarean section most common indication (43.16%). Two babies were stillborn. 99.20%, Apgar > 7 at 5 minutes. Two required resuscitations. Nine required neonatal intensive care unit (NICU) admission. Two hundred and seven babies isolated from their mother, four turned positive. Forty-four were roomed in with mothers, 30 turned positive. One hundred and sixty-seven were breastfed, included 34 positive babies. Affected babies were asymptomatic. Conclusion: COVID-19 runs a milder course in pregnancy, possibly due to altered immune status. However, severe cases can deteriorate rapidly. There is evidence of community spread. COVID-19 seemed to increase cesarean rates. Neonatal transmission appeared higher following vaginal deliveries, rooming-in, and breastfeeding. Clinical significance: There is a dearth of adequate data to formulate guidelines about COVID-19-positive pregnancies despite numerous studies. The causative virus was identified from throat swab samples in the Chinese Centre for Disease Control and Prevention (CCDC) on January 7, 2020, and it was named severe acute respiratory syndrome coronavirus (SARS‐CoV‐2). Subsequently, World Health Organization (WHO) renamed it COVID‐19. COVID-19 was a highly infectious newly discovered coronavirus, which led to a worldwide pandemic. It has shown an equal number of cases between men and women, but a lower mortality rate in women. Materials and methods: A retrospective observational study was conducted in the Department of Obstetrics and Gynaecology of GMC, Srinagar, comparing the total number of emergency admissions including obstetric emergencies, such as ectopic pregnancy, intrauterine device (IUD), severe anemia, eclampsia, abruption, obstructed labor, and postpartum hemorrhage (PPH) during a period of 6 months from March 2020 to August 2020 with that from March 2019 to August 2019. Results: A total of 13,784 patients were admitted over a period of 6 months from March 2020 to August 2020, and there was a significant decline in the admissions as compared to pre-COVID-19 period of March 2019 to August 2019 (p-value, 0.007). Conclusion: The COVID‐19 lockdown greatly reduced the rate of admission to gynecological and obstetric emergencies. To identify maternal and neonatal risks associated with COVID-19 in pregnancy and to describe outcome a matched case–control study was done. Materials and methods: In this study, COVID-19 reverse transcription polymerase chain reaction (RT-PCR) positive women who delivered from May 1, 2020, to August 31, 2020, were included. Cases were classified according to their severity and investigations. Controls were selected from COVID-19 negative women in a 1:1 ratio who delivered in the study period. Data were collected, analyzed in terms of maternal and neonatal outcomes. Results: Of the total 181 COVID-19 cases delivered during the study period, there were 97.23% mild, 2.76% moderate, and 0.55% severe cases. A total of 178 (98.34%) were diagnosed in the third trimester. More COVID-19 cases were from urban areas. Mean gestational age in the mild category was 38.1 weeks and the moderate/severe category was 37.5 weeks. Around 154 (85.08%) were asymptomatic. Cases had more medical and pregnancy morbidity than controls, which was statistically significant. About 52.49% of cases and 33% of controls had cesarean. Length of hospital stay was more in cases. Mean birth weight was 2.7 ± 0.59 kg in cases and 2.5 ± 0.56 kg in control. There were 7 stillbirths in each group. A total of 14 in cases and 16 in controls were transferred to the newborn intensive care unit (NICU). Conclusion: The majority of COVID-19 infected women who are asymptomatic are in the mild category and there are no adverse maternal and neonatal outcomes due to the disease. The aim of the present study was to explore whether hospital isolation and containment policies among women giving birth in COVID‐19 Level 3 facility enhanced psycho‐emotional distress in the immediate postpartum period. Methodology: The study was designed as an observational study. All women giving birth at Santosh Hospital, a Level 3 COVID facility in Ghaziabad, from June 2020 to October 2020, were studied. Data collection was done by a pretested structured questionnaire which was administered to the participants in isolation ward on the second postpartum day. The women were interrogated regarding the experiences of the COVID isolation ward with special reference to the problems faced during their stay there, the anxiety for the baby, loneliness, and other factors. The women were evaluated using Edinburgh Postnatal Depression Scale (EPDS). Result: The study group comprised 61 women who delivered in our hospital. The mean age of the subjects was 24.2 ± 1.2 years. In the study, majority of the patients were distressed with loneliness (54%) and anxiety for the baby (43%). As the policy of the institution was to hand over the newborn baby immediately to the attendants, the mother was bound to be distressed. Sleeplessness, loss of appetite, and boredom were other problems faced by the COVID-positive patients. The incidence of postnatal depression was 24.5% during COVID-19 pandemic in this institution. In the study, we found that patients with postnatal complications, patients who had symptoms of COVID, whose baby was handed over to the attendants, postpartum stay more than 7 days, and inability to connect to the family had a statistically significant correlation with those who had EPDS score >13. To study evidence of the vertical transmission in pregnant women infected with COVID-19. Materials and methods: A prospective study was performed in BJ Medical College and Civil Hospital, Ahmedabad, one of the biggest designated (1200 beds) COVID hospitals in India. It was carried out from June 2020 to mid-October 2020. A total of 50 pregnant patients who were COVID-19 positive and admitted to the labor room were studied. Results: The mean age of the mothers was 27.40 ± 4.76 years (range: 19–36 years) and mean gravidity was 1.67 ± 1.41 (range: 1–4). The mean gestational age was 38.31 ± 1.67 weeks. The main complaint was fever in 22 (44%), followed by dry cough in 9 (18%), sore throat in 4 (8%), headache in 2 (4%), malaise in 7 (14%), and diarrhea in 2 (4%) patients. Twenty-two (44%) patients showed an elevated C-reactive protein level, 17 (34%) had lymphopenia, 19 (38%) had mild elevation of the D-dimer level, and 16% of the patients had increased interleukin 6 levels. Most of the patients had normal renal function test (RFT) and liver function test (LFT) results. A majority of them (86%) had normal chest X-ray, whereas rest of them (14%) had haziness in bilateral lung fields. Twenty-six (52%) patients underwent C-section, and 23 (46%) delivered vaginally. Four (8%) babies required the admission in the newborn intensive care unit for mild birth asphyxia but were discharged well. Five babies developed physiological jaundice and required single surface phototherapy (SSPT). Forty-eight (96%) out of 50 had negative vaginal swab by reverse transcription polymerase chain reaction (RT-PCR), 48 (96%) did not show any evidence of the virus in the expressed breast milk by RT-PCR. We collected the cord blood sample of 35 patients after delivery which was found to be negative in all (100%). The nasopharyngeal swab of all the babies was negative on the very first day. Day 7 repeat swab was found positive only in one baby. This transmission was mostly via respiratory droplets during intermittent assisted breastfeeding. Most of the cases were having mild disease without acute respiratory distress syndrome (ARDS), which responded to the supportive treatment. Four patients (8%) had severe anemia and required blood transfusion. Conclusion: Most of the cases were having mild disease without ARDS, which responded to supportive treatment. Reassuringly, there was no evidence of poor fetal outcome, intrauterine fetal deaths, or premature deliveries. Khan","doi":"10.5005/jp-journals-10006-1810","DOIUrl":"https://doi.org/10.5005/jp-journals-10006-1810","url":null,"abstract":"Background: The coronavirus disease-2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is a global public health emergency Physiological and mechanical changes in pregnancy increase susceptibility to infections in general, particularly when the cardiorespiratory system is affected, and encourage rapid progression to respiratory and heart failure in the gravida Case description: We present a case report of 30-year-old primigravidae with 36 5 weeks by date and 37 week by scan with diagnosed preeclampsia 2 weeks back on medication with 11 × 10 4 × 10 9 cm single intramural anterior wall fibroid with breech presentation COVID nasopharyngeal sample reported as SARS-CoV-2 real-time PCR positive The patient was started on medication defining the Stage 1 Group A Asymptomatic Category Chest X-ray showed mild bilateral pulmonary infiltrates On Day 6 of admission, the patient went in labor, and hence was taken for emergency LSCS in view of primigravidae with breech with preeclampsia with fibroid with PROM under spinal anesthesia with all precautions Intraoperatively, the LSCS was uneventful Postoperatively 6 hours postpartum, the patient developed sudden onset breathlessness NYHA Grade 4, with saturation of 56%, and patient was intubated and shifted to ICU Chest X-ray revealed viral pneumonia, and further 2D ECHO was suggestive of cardiomyopathy Conclusion: We suggest an echocardiogram in pregnant women with coronavirus disease 2019 pneumonia, in particular those necessitating oxygen or those who are critically ill Clinical significance: Viral myocarditis and cardiomyopathy have been reported in nonpregnant COVID-19 patients Information on COVID-19 in pregnancy is currently limited No specific literature is available of viral cardiomyopathy reported in pregnant women with COVID-19 infection © The Author(s) 2020","PeriodicalId":53559,"journal":{"name":"Journal of SAFOG","volume":"5 1","pages":"243-244"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"88359365","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
