{"title":"Altered Insulin, IRS/PI3K/AKT/mTOR Signaling, ACE2 and Angiotensin II, Correlating with Vitamin K Status and Severity in Metabolically Disordered COVID-19 Egyptian patients","authors":"Huda Ismail Abo-Elfadl, Gehan Morsy","doi":"10.21608/eajbsc.2023.321787","DOIUrl":"https://doi.org/10.21608/eajbsc.2023.321787","url":null,"abstract":"Aims: Metabolic disorders are essential risks of adverse outcomes in coronavirus disease 2019 (COVID-19). Vitamin K status might play an important role in metabolic regulation and influence the severity of COVID-19. This research shed light on vitamin K status in association with some metabolic pathways; pancreatic beta-cell function, insulin resistance, PI3K/AKT/mTOR signaling, and the angiotensin-converting enzyme 2 (ACE2)/angiotensin II in the context of COVID-19 and its metabolic effects in metabolically disordered patients. Main methods: This study included hospital-confirmed COVID-19 patients with or without obesity and/or diabetes mellitus (DM) and control subjects. The effect of viral infection on PERK double-stranded RNA-dependent protein kinase (PKR) and ACE2 was regressed against insulin resistance and blood pressure, respectively. Demographic and clinical characteristics, blood, and, clinical indices, were collected. Key findings: A total of 90 hospital-confirmed COVID-19-positive patients were eligible for our study. COVID-19 patients with obesity and DM showed poor vitamin K status which is correlated with severely compromised insulin signaling and inflammatory and immune states. It is also adversely associated with the existence of hypertension comorbidities. PKR levels significantly (P<0.05) and negatively predicted IRS (R 2 = 0.598) explaining 59.8% of the variance in IRS. Also, ACE2 is significantly (P<0.05) and negatively predicted systolic blood pressure and explained 45.6% of the variance in SBP (R 2 =0.456). Significance: Our study indicated that poor vitamin K status might be an important risk factor for severe adverse outcomes in COVID-19 patients with pre-existing obesity and type 2 diabetes. Have been utilized as an effective, affordable, secure, and ecologically friendly way to get rid of heavy metals like cadmium. Microalgae from Spirulina platensis were used to make biochar by burning it. We mix biochar with hydrogen peroxide to create functional groups on the surface by using ice bath sonication which is biochar activate (BC). Also, magnetic biochar (MBC) has been prepared by mixing biochar activated with Fe 3 O 4 (2:1 mole ratio) respectively. Two concentrations of aqueous solutions (5 ppm and 10 ppm) were mixed with two weights of biochar and magnetic biochar (0.1 and 0.5 g) for 10, 20, and 40 minutes. The concentration of cadmium (5 ppm) with 0.5 g of biochar (BC) after 40 minutes removed 97.58% of cadmium ion from the aqueous solution. On the other hand, magnetic biochar (MBC) (0.5 g) with 5 ppm of cadmium solution after 40 minutes could remove 100% of cadmium ions. We character BC, MBC, and biochar treated with cadmium ions by using FTIR, FESEM, EDX, and XRD studies. These analytical results indicate that the effective groups in biochar and magnetic biochar can adsorb heavy metals by carboxyl, hydroxyl, and carbonyl groups. The findings of the investigation on magnetic biochar revealed that, in addition to carbonyl, hydroxyl, and carboxylic groups, the substance also included magnetite. A novel analytical technique for estimating MFA 250 mg in capsule pharmaceutical formulations for different brands was established. The method suggested is based on the reduction of Cu(II)-2,9DMP complex to colored Cu(I)- 2,9DMP complex. The optimum experimental conditions have been achieved. Beer'ss Law holds true in the concentration levels 5.0-60 μg/mL with accurate molar absorptivity of 0.238 L/mol.cm at 454 nm and “detection limit” was 0.5882 μg/mL and the limit of quantification was 1.7825 μg/mL and correlation coefficient obtained has been near to one. The Effect of pH, reaction time, Buffer Solutions, Volume of Cu(ІІ), Volume of 2,9.DMP reagent, temperature, Order of Addition, and Effect of Acid on the determination of MFA, have been examined. The proposed method has been successfully utilized for quantitative assessment of commercially produced dosage modes. Our research sought to develop a fresh, accurate, and sensitive technique for measuring MFA as a pure pharmacological product and as a hard dosage. Due to their high potency and extraordinarily broad antibacterial action, Carbapenem antibiotics, which belong to the most recent generation of β-lactam antibiotics, are frequently used in clinics to treat bacterial infections. The current study aimed to shed light on Escherichia coli 's resistance to β-lactam antibiotics, especially Carbapenem, and the potential link between Carbapenem resistance and the production of β-lactamase variants bla (NDM-1, NDM-2, NDM-3, NDM-4, NDM-5, GIM-1, VIM-1, SPM-1, and IMP-1) in Karbala province, Iraq. Materials and methods: Four hundred-eight isolates have been identified according to traditional methods (colonial morphology on MacConkey and Eosin methylene blue (EMB) agar, and biochemical reactions), and confirmed by the Vitek2® system. Carbapenem susceptibility was assayed by using the Vitek2® system. Isolates were subjected to multiplex PCR targeting β-lactamase variants genes. Results: Out of a total of 408 samples randomly collected from different clinical sources, 24 (19.51%) Escherichia coli isolates were diagnosed. Four (16.67%) of the Escherichia coli isolates tested positive for imipenem and meropenem resistance. It is noted that this indicator is higher in patients with psoriasis compared to healthy people, the IL-17 level of interleukin was 765.29 ± 85.4, while the rate for healthy people is 212.37 ± 39.6.
