This tool was subsequently integrated into standard operating procedures for preparedness and response to establish a national infectious disease risk assessment framework. Beginning in 2025, the framework has been applied to domestic mass gathering events. An analysis of two events involving iterative risk assessments demonstrated that deficiencies identified during the initial assessment were successfully addressed by strengthening education, hygiene and ventilation management, medical and response systems, and risk communication. These targeted interventions reduced the overall risk level to low or very low in the final assessment.

Conclusions: The Korean infectious disease risk assessment framework for mass gatherings provides a standardized approach for systematically assessing infectious disease risks and preparedness, identifying areas for improvement during event planning, and informing preparedness and response measures. We analyzed the detection characteristics of Legionella spp. within environmental water samples submitted to the Gyeonggi-do Institute of Health and Environment, the Republic of Korea, to determine management priorities. Methods: We analyzed 9,172 Legionella spp. testing records for environmental water samples received from January 2023 through December 2025, summarized by sample type, purpose, submission route, species, and concentration. Chi-square and Cochran-Armitage trend tests were used. Results: Legionella spp. were detected in 570/9,172 samples (6.21%; 95% confidence interval, 5.74-6.73%). Detection differed significantly by sample type (p<0.001)-bathtub water: 18.30%, hot water: 11.03%, cooling-tower water: 5.53%, other water: 3.53%, and cold water: 1.28%; the ranking excluding other water was consistent across 3 years. Cooling-tower water accounted for 47.2% of positives, increasing significantly from 34.2% in 2023 to 59.6% in 2025 (p for trend<0.001); after standardization for sample-type composition, the increase was smaller (43.2% to 53.1%). Requester-collected submissions comprised 96.4% (4,691/4,865) of cooling-tower samples; detection rates were 14.94% (26/174) for officially-collected and 5.18% (243/4,691) for requester-collected samples. Cooling-tower water accounted for 55.6% of positives >10,000 CFU/L. Conclusions: Bathtub and hot water systems require preventive management given their high detection proportions, whereas cooling towers require surveillance priority given their large and increasing contribution to positives. Priorities should combine sample-type detection rates with the size and trend of positive counts. As cooling-tower testing relies largely on voluntary requests, risk-targeted official testing should complement it.
","PeriodicalId":523884,"journal":{"name":"Public health weekly report","volume":"19 39","pages":"1667-1683"},"PeriodicalIF":0.0,"publicationDate":"2026-10-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13672474/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149217948","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While no autochthonous cases have been reported in the Republic of Korea, imported cases continue to occur. This study analyzed laboratory testing data from 2020 to 2025 to support the surveillance of imported mosquito-borne infections. Methods: A total of 2,904 laboratory test requests for dengue fever submitted to the Korea Disease Control and Prevention Agency and 17 provincial Institutes of Health and Environment were reviewed. Laboratory diagnosis was performed using real-time reverse transcription polymerase chain reaction (RT-PCR) and immunoglobulin M (IgM) enzyme-linked immunosorbent assay (ELISA). The analysis focused on annual trends, presumed countries of infection, and the distribution of dengue virus (DENV) serotypes. Results: Of the 2,904 test requests, 613 cases were confirmed positive, with an overall positivity rate of 21.1%. The number of positive cases was lowest in 2021, with three cases, but increased after international travel resumed, reaching 177 cases in both 2023 and 2024. Most infections were presumed to have been acquired in Southeast Asia, particularly in Indonesia, the Philippines, and Vietnam. DENV-2 was the most common serotype, followed by DENV-1, DENV-3, and DENV-4; all four serotypes were detected. Conclusions: Imported dengue fever increased following the recovery of international travel, with infections concentrated in Southeast Asia. Presumed countries of infection were mainly associated with four Southeast Asian countries (73.1%), with cases concentrated during the summer vacation season. Because reinfection with a different DENV serotype may increase the risk of severe dengue, individuals who travel repeatedly to dengue-endemic countries should exercise particular caution.
","PeriodicalId":523884,"journal":{"name":"Public health weekly report","volume":"19 39","pages":"1651-1667"},"PeriodicalIF":0.0,"publicationDate":"2026-10-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13672475/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149217977","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
