However, optimal duration of SET is discussed. This study aimed to evaluate the effectiveness of a short-duration program combining SET, nutritional advice and therapeutic education on maximal walking distance (MWD). Secondary objectives included assessing the impact of this program on smoking cessation, low-density lipoprotein cholesterol (LDL-c) levels, optimization of medical therapy, and to identify factors associated with MWD improvement.

Methods: We conducted a retrospective, monocentric study. Parameters assessed before and after the program included ankle-brachial index (ABI) at rest, MWD on treadmill, post-exercise ABI, and exercise transcutaneous oxygen pressure (TcPO2) using the Decrease from Rest of Oxygen Pressure (DROP). LDL-c levels, smoking status, and medical therapy were documented before and after the program.

Results: Forty-three patients (77% men, median age 67 [57-74]years) were included. The median program duration was 23.5 days [20.8-30.5], with 20.7h [16.8-25.1] of supervised physical activity. MWD increased significantly from 208m [139-294] to 251m [171-574] (P<0.001). Improvements in smoking status, LDL-c levels, and medical treatment optimization were also observed. Among all parameters, only the DROP TcPO2 before the program was significantly associated with MWD improvement (P<0.05).

Conclusion: A short-duration program effectively improved median MWD (+56% [10-159%]), cardiovascular risk management, and was associated with baseline ischemia severity. These findings support the utility of shorter combined programs and the potential value of exercise TcPO2 in selecting responsive patients.
","PeriodicalId":520901,"journal":{"name":"Vascular diseases (Paris, France)","volume":" ","pages":"111-115"},"PeriodicalIF":0.0,"publicationDate":"2025-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144532935","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It presents clinically as acute bilateral ischemia of the lower limbs. Its association with flaccid paraplegia remains a very rare event, which should prompt examination of the pulses for rapid diagnosis and management. There are many causes, including cardiac rhythm disorders. Acute embolic thrombosis of the aorta, secondary to a cardiac rhythm disorder in Basedow's disease, makes this event even rarer. We report the case of a 73-year-old woman with Basedow's disease, who presented with acute thrombosis of the subrenal aorta, revealed by flaccid paraplegia. The patient underwent aortic thrombectomy within 4hours, by surgical approach of both common femoral arteries. The postoperative course was marked by persistent motor monoplegia of the right lower limb despite rapid revascularization and cruralgia of the left lower limb. Knowledge of PAD in women compared to men is surprisingly low compared to literature data concerning other cardiovascular diseases and general illnesses. For example, studies comparing endovascular or surgical revascularization in men versus women have reported conflicting results.

Methods: In this retrospective cohort study based on medical records, we included adult patients (men and women) who had been hospitalized in the vascular surgery department of Nantes University Hospital Center for PAD revascularization by angioplasty with or without stent. The primary outcome was a composite of revascularization failures, collected intraoperatively, at one month (M1) and at one year (A1). Secondary outcome included various risk factors for PAD.

Results: A total of 447 patients including 315 men and 132 women were enrolled in the trial. The prevalence of tobacco was different between the two groups with 245 men (78.0%) who were smoking or had quit smoking, while only 13 women (9.8%) were smoking and 30 (22.7%) had quit smoking. Post-surgical anticoagulant, calcium channel blocker, beta-blocker and antiplatelet drugs were similarly prescribed in men and women. Differences were found for treatment with a renin-angiotensin system inhibitor, prescribed in 48.5% in women and in 60.2% in men (P=0.028). Similarly, statin treatment was prescribed in 59.1% of women and 76.4% of men (P<0.001). The 1-month incidence of the primary-outcome was higher in women than in men (27.8% vs. 19.3%), with a risk ratio of 1.43 (95% confidence interval [CI], 0.99 to 2.06; P=0.071). At one year, we found a risk ratio of 1.13 (95% confidence interval [CI], 0.94 to 1.37; P=0.20). In multivariable regression model, the adjusted odds ratio for primary outcome was 1.65 for women/men (95% confidence interval [CI], 0.95 to 2.86; P=0.075). Finally, there were significantly more cardiovascular deaths in women than in men at one month (P=0.003) and one year (P=0.005).

Conclusions: Women are more at risk of failure after lower extremity surgery, especially in the first month. After a year of hindsight, women have more cardiovascular mortality than men.
