However, intra- and inter-observer DUS reproducibility for AVF follow-up has not been thoroughly assessed. This study aimed to evaluate the reproducibility of several DUS parameters commonly employed in clinical settings.

Methods: In this prospective, single-center study, we included patients with AVFs referred to our unit for ultrasound assessment. Two operators independently performed DUS examinations. At the axillary (Ax) and brachial (Brach) artery levels, the following parameters were assessed: blood flow (Q), peak systolic velocity (PSV), resistance index (RI), and Q/RI ratio. In cases of venous stenosis, lumen diameter, PSV, and lesion length were also recorded at the stenosis site. Reproducibility was evaluated using intraclass correlation coefficients (ICCs), coefficients of variation (CVs), and Bland-Altman plots.

Results: From March to May 2025, eighteen patients (61% men, median age 63 [56-74] years) were included. Among the included population, 10 patients had a venous stenosis. For intra-observer agreements, ICCs ranged from 0.88 [0.77-0.95] to 0.99 [0.99-1.00] for both observers. For inter-observer agreements, ICCs were good to excellent for most of the parameters. CVs ranged from 5% to 34%. For Qbrach, Q/RIbrach, CVs were 14 [6-19]% and 12 [8-16]%, respectively. For the quantification of the lumen of the venous stenosis, ICC was 0.92 [0.72-0.98] and the CV was 7 [1-10]%.

Conclusion: This study showed that intra- and inter-observers can be considered as good to excellent for the commonly used DUS parameters for AVF follow-up except for the measurement of the anastomosis diameters.
","PeriodicalId":520901,"journal":{"name":"Vascular diseases (Paris, France)","volume":" ","pages":"148-154"},"PeriodicalIF":0.0,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145254416","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We report the case of a 54-year-old woman who developed a catastrophic thrombotic storm shortly after the diagnosis of advanced ovarian clear cell carcinoma. Despite therapeutic anticoagulation, she experienced multiple arterial thromboembolic events, including ischemic stroke, acute upper limb ischemia, and ST-elevation myocardial infarction, possibly due to paradoxical embolism through a patent foramen ovale. Laboratory investigations revealed markedly elevated tumor markers and features of cancer-associated coagulopathy. This case highlights the extreme thrombotic potential of ovarian clear cell carcinoma and underscores the importance of considering paradoxical embolism as a mechanism of arterial events in patients with concurrent venous thrombosis and intracardiac shunting. Early recognition and coordinated multidisciplinary management, including critical care support, are essential in this life-threatening scenario.
","PeriodicalId":520901,"journal":{"name":"Vascular diseases (Paris, France)","volume":" ","pages":"162-165"},"PeriodicalIF":0.0,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145139742","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Symptoms may improve with the use of vasodilators and may worsen with the use of vasoconstrictors. The objective was to describe the sociodemographic and pharmacological characteristics of patients with a diagnosis of primary or secondary Raynaud's phenomenon in Colombia. This cross-sectional study investigated the use of drugs by patients diagnosed with Raynaud's phenomenon between June-2021, and June-2024, collected from a population database of 9.5 million Colombians affiliated with the Colombian health care system. Descriptive and bivariate analyses were performed. A total of 567 patients were identified. The average age was 51.9±17.9 years, and 82.5% were women. A total of 58.7% were diagnosed with primary Raynaud's phenomenon and 41.3% with secondary Raynaud's phenomenon. The most frequent secondary cause was systemic sclerosis (36.8%). A total of 58.9% of patients received pharmacological treatment, with a greater proportion among patients with secondary Raynaud's phenomenon than among those with primary Raynaud's phenomenon (71.8 vs. 49.8%; P<0.001). The most used drugs were calcium channel blockers (45.7%). A total of 9.2% of patients received drugs that can worsen Raynaud's phenomenon, most of which were non-selective β-blockers (6.9%). Primary Raynaud's phenomenon was more common than secondary Raynaud's phenomenon. Systemic sclerosis was the main cause of the secondary Raynaud's phenomenon. Calcium channel blockers were the most used medications for the management of Raynaud's phenomenon. Some patients were exposed to medications that can worsen Raynaud's phenomenon, but these medications may be necessary for the management of other conditions.
","PeriodicalId":520901,"journal":{"name":"Vascular diseases (Paris, France)","volume":" ","pages":"99-103"},"PeriodicalIF":0.0,"publicationDate":"2025-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144786465","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Listening, accompanied by validated questionnaires, Doppler ultrasound under pharmacological erectile stimulation and testosterone levels are the basic elements. These are complemented by nerve tract examinations, angiography and/or cavernoscans. In the light of these assessments: This is a curable pathology, but it has a serious impact on quality of life, since 30% of our consultants consider their disability to be unbearable. Cardiovascular risk factors (diabetes, hypertension, smoking, TML, obesity) considerably exacerbate the ageing factor, which is considered to be predominant after the age of 50. When ED is of arterial origin, coronary lesions are found in almost 80% of cases, in subjects with no known cardiovascular history. This makes vascular ED the first sign of atheromatous disease, five years before more severe events (myocardial infarction, stroke). There is a specific pathology of younger subjects suffering from primary ED linked to surgically repairable venous malformations. Anamnesis reveals a delay, sometimes numbering years, before effective management of the problem. We therefore recommend that this pathology be managed by specialized multidisciplinary entities. Last but not least, a better understanding of the biochemical processes involved in erection raises hopes for the development of innovative curative therapies targeting easily accessible erectile tissue, for which trials are already underway (plasmatherapy, stem cells).
","PeriodicalId":520901,"journal":{"name":"Vascular diseases (Paris, France)","volume":" ","pages":"120-126"},"PeriodicalIF":0.0,"publicationDate":"2025-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144328492","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
