This article provides an overview of the sensorimotor innervation of the upper airway structures. The upper airway should be richly equipped with receptors and sensory nerve fibers due to its critical role in life-preserving protective reflexes. It is well established that patients with obstructive sleep apnea develop sensorimotor neuropathy over time. It is an interesting hypothesis that vibrations of the upper airway soft tissues and snoring are considered as likely trigger for this sensory neuropathy. The patient had a history of tuberculosis infection treatment as a newborn. She initially presented with symptoms including fever, weight loss, and coughing. Clinical examination revealed a newly developed mass in the right breast. Diagnostic workup included an 18F-FDG PET-CT scan, followed by repeated biopsies. Microbiological culture, molecular testing, and histopathological analysis confirmed the presence of Mycobacterium tuberculosis. Tuberculous mastitis is a rare form of extrapulmonary tuberculosis. Depending on the presence or absence of comorbidities and various risk factors, patients were grouped, and their 5-year survival following the initial diagnosis of nontuberculous mycobacterial pulmonary disease was compared. In addition, independent covariates were simultaneously analyzed to assess whether they were significantly associated with an increased risk of mortality.In this cohort of 52 patients (mean age 64.2±12.2 years) with non-tuberculous mycobacterial pulmonary disease, the 5-year survival rate was 55.8%. Five-year survival was markedly lower among individuals with coexisting chronic obstructive pulmonary disease or heart failure as well as in those without culture conversion. In a multivariate Cox regression analysis of independent covariates, age and increasing Charlson Comorbidity Index scores were associated with reduced 5-year survival.A 5-year survival rate of less than 60% indicates a poor prognosis in patients with non-tuberculous mycobacterial pulmonary disease, highlighting the need for more effective management of both the disease and associated comorbidities.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":""},"PeriodicalIF":1.7,"publicationDate":"2026-07-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148407876","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This question is particularly relevant for pulmonology, given the growing importance of respiratory diseases, which are often given only secondary importance in university curricula. For this reason, the event \"PneumoNEXT - Continuing Education, Exchange, Future\" was conceived as a one-week continuing education program for medical students starting in their 10th semester and physicians in training. The program combines case-based learning with practical workshops on key topics in pulmonology, as well as opportunities for networking and exchange with experienced pulmonologists.The pilot project included 16 participants (mean age 33 ± 5.9 years, 50% women). In the accompanying evaluation via an online survey, all participants reported a positive change in their confidence in practical decision-making. Additionally, 93% stated that their professional focus had shifted more toward pulmonology and that they would like to begin or continue pulmonology training. There is still potential for improvement, including the integration of patient-centered workshop formats. PneumoNEXT can therefore be understood as a practice-oriented, structured continuing education and networking format that creates suitable conditions to promote interest in pulmonology, introduce young professionals to the field at an early stage, and support the decision to pursue further training in pulmonology.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":""},"PeriodicalIF":1.7,"publicationDate":"2026-07-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148407924","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We report an extreme presentation in a woman in her late 40s who developed acute right-sided chest pain one day prior to menstruation. Initial imaging revealed a right pneumothorax, which resolved after intercostal drainage. However, subsequent computed tomography revealed a massive defect in the right hemidiaphragm with herniation of the entire right hepatic lobe, gallbladder, and upper pole of the right kidney into the thoracic cavity. A multidisciplinary surgical strategy was adopted, involving abdominal reduction of the herniated viscera, followed by video-assisted thoracoscopic mesh repair of the diaphragm and talc pleurodesis. Postoperative hormonal suppression was initiated. The patient remained asymptomatic without recurrence at 18 months. This case highlights that catamenial pneumothorax may represent advanced chronic diaphragmatic pathology, even at the first clinical presentation.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":""},"PeriodicalIF":1.7,"publicationDate":"2026-07-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148400409","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

With a mortality rate of 35-45%, it remains a significant clinical challenge; however, current developments offer promising therapeutic options.Contemporary ARDS management continues to evolve: parameters such as driving pressure and mechanical power complement established lung-protective ventilation concepts and enable more precise therapeutic control. Transpulmonary pressure measurement supports individualized PEEP titration, while automated closed-loop systems can significantly extend time within lung-protective ranges.Veno-venous extracorporeal membrane oxygenation (VV-ECMO) has established itself as an important rescue therapy and received conditional recommendation for the first time in the new S3 guideline 2025. Ultra-protective ventilation strategies under ECMO enable substantial reduction of mechanical stress and support the concept of \"lung rest\".From epidemiology and pathophysiology to novel disease entities such as EVALI and innovative monitoring procedures, this review article provides a current overview of the syndrome's fundamentals as well as modern diagnostics and therapy of ARDS, and demonstrates how these insights can be beneficially implemented in daily clinical practice.</p>","PeriodicalId":520755,"journal":{"name":"Pneumologie (Stuttgart, Germany)","volume":" ","pages":"500-513"},"PeriodicalIF":1.7,"publicationDate":"2026-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148152996","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
