Biologic drugs targeting interleukin 4 and 13 (dupilumab) receptors and blocking interleukin 5 (mepolizumab) have been approved for the treatment of the T2 endotype.Aim: Presentation of the authors' suggestions that guided the choice of an appropriate biologic agent when initiating qualification for the treatment of patients with CRSwNP, based on the authors' experience and data from the literature related to biologic therapy.Materials and methods: Patients with CRSwNP were qualified for biological treatment in accordance with the EPOS (European Position Paper on Rhinosinusitis) guidelines and the criteria of the B.156 drug program. The choice of an appropriate biologic was based on medical history data, such as the level of asthma control, presence of allergies, and the occurrence of N-ERD (nonsteroidal anti-inflammatory drug-exacerbated respiratory disease), as well as biomarker results: total IgE concentration and absolute blood eosinophil count.Results: The study included 43 patients (23 treated with mepolizumab, 20 treated with dupilumab). Groups of patients were comparable in terms of age, sex, number of sinonasal surgeries, prevalence of asthma, NSAIDs (non-steroidal anti-inflammatory drugs) sensitivity, atopy and initial SNOT-22 (Sinonasal Outcome Test-22), VAS (Visual Analog Scale), NPS (Nasal Polyp Score), and L-K score (Lund-Kennedy score). Eosinophilia total range was 250-1000 cells/microL. Baseline peripheral blood eosinophil count was significantly higher in the mepolizumab-treated group. In the dupilumab-treated group, increases in eosinophil counts up to 3500 cells/μl were observed during therapy; however, no patient developed clinical features of hypereosinophilic syndrome.Conclusions: When selecting a biologic drug for the treatment of CRSwNP, it is recommended to consider not only the values of available biomarkers, but also the degree of asthma control, the cumulative dose of oral corticosteroids (OCSs), the patient's age, and treatment convenience for the patient.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":"79 6","pages":"1-8"},"PeriodicalIF":0.6,"publicationDate":"2025-11-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145717203","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Associations were analysed using Chi-square and Fisher's exact tests (p < 0.05).Results: In proportion to the increasing number of elderly patients attending outpatient clinics, the prevalence of certain ENT conditions rise, particularly hearing loss (p = 0.00042), dizziness (p = 0.00339), chronic sore throat (p = 0.00002), \"runny nose\" (p = 0.00048), dysphagia (p = 0.0361), and dysphonia (p = 0.00765). Clinics with more seniors needed more support staff and prescriptions. Senior physicians, compared to younger specialists, reported significantly higher monthly numbers of geriatric ENT conditions, including hearing loss (p = 0.00002), dizziness (p = 0.00171), chronic sore throat (p = 0.00000), and runny nose (p = 0.00845). However, the majority across all experience levels recognized value for dedicated services (72%) and identified access barriers (78%).Discussion: Physician experience influences how the needs of elderly patients are addressed. Senior doctors show stronger clinical judgment, while younger ones focus on system improvements. Findings highlight the need for elevating the skills for tailored multidisciplinary care in senior-heavy clinics.Conclusions: Physician experience significantly affected the recognition of ENT conditions and medication side effects, while early-career physicians were more likely to support dedicated geriatric ENT services and reported more difficulties related to access barriers. In Central Europe, tree pollen plays a significant role in triggering symptoms of inhalant allergy in the spring months.Aim: This study evaluated the deposition of tree pollen grains in a 3D anatomical model of the adult nasal cavity under varying flow rates (5 L/min, 7.5 L/min, and 20 L/min per nostril).Material and methods: Original methods of analysis were developed, including macroscopic analysis of photographs reflecting tree pollen deposits on the surface of the lateral wall of a printed 3D anatomical model of the nasal cavity and nasal septum, and microscopic evaluation of pollen collected from selected parts of the nasal geometry. The macroscopic analysis was supplemented with original software for the quantitative determination of areas covered with pollen deposits.Results: The results showed statistically significant differences in the regional deposition of pollen grains of different sizes in the nasal cavity, depending on the flow conditions. The deposition of birch, alder, and hazel pollen grains in the nasal cavity exhibits similar characteristics, with significant differences between the individual segments of the lateral wall and the nasal septum. While the middle segment of the lateral wall accounted for the largest segment share of the total lateral wall deposition, the highest relative pollen deposition in relation to the segment area was found in the front segment. The posterior segment of the lateral wall showed a marginal contribution to the deposition process. The most significant percentage of the segment area of the nasal septum covered with pollen was recorded in the anterior segment. However, the posterior segment accounted for a larger share of the total septal deposition due to its larger surface area. Consistent findings describe deposits of large pollen grains from pine and spruce trees. The values of segmental pollen deposition in the anterior segment were significantly higher than in the other segments. The deposition on the nasal septum was significantly greater in the anterior than in the posterior segment. Lateral wall share % and septal share % tended to increase with increasing airflow through the nasal cavity, reaching their highest values at 20 L/min. The intensification of airflow led to a shift in the location of maximum pollen deposition towards the anterior sections of the nose, with a simultaneous reduction in pollen penetration into the posterior segments.Conclusions: The study confirms the role of inertial effects in the flow and deposition of aerosols in the nasal cavity, indicating that the large size of pollen grains and higher airflow rates promote pollen deposition mainly in the anterior regions of the lateral wall and nasal septum. However, it also shows that other deposition mechanis","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":"79 5","pages":"1-20"},"PeriodicalIF":0.6,"publicationDate":"2025-09-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145140012","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

No significant association was found between genotype and surgical outcomes or COVID-19 incidence.Discussion: Phenotypic taste testing may help identify individuals with impaired T2R38 function and poorer innate immune responses, allowing for more tailored treatment.Conclusions: The PTC taste strip test is a simple, non-invasive tool that correlates well with TAS2R38 genotype and may support clinical decision-making in CRS.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":"79 5","pages":"1-8"},"PeriodicalIF":0.6,"publicationDate":"2025-08-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144877953","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Chronic otitis mediawith cholesteatoma presents with recurrent ear discharge and hearing loss, typicallyrequiring surgical intervention. Although the coexistence of VS and cholesteatoma israre, it creates complex diagnostic and therapeutic challenges that necessitate individualized management approaches.Aim: : Considering the rare coexistence of VS and cholesteatoma, we aimed to analyze diagnostic patterns and therapeutic outcomes in our patient cohort.Materials and methods: We reviewed four patients with simultaneous VS and cholesteatoma. One patient presented with cholesteatoma in one ear and VS in the contralateral ear, and underwent middle ear cholesteatoma removal via an endaural approach, followed by translabyrinthine VS resection. The other three patients had ipsilateral lesions; two were managed with a transotic approach for the simultaneous removal of both lesions, while one underwent closed-cavity cholesteatoma surgery, with a cerebellopontine angle tumor subsequently detected and treated with gamma knife radiosurgery.Results: During a follow-up period of two to four years, no residual or recurrent cholesteatoma or tumor was observed in all patients except the one treated with gammaknife, who remained stable post-treatment.Conclusions: A tailored treatment strategy based on tumor stage, hearing status, patient age, and cholesteatoma characteristics resulted in favorable functional and anatomical outcomes. The coexistence of cholesteatoma and VS should be consideredin the differential diagnosis of hearing loss. Our findings underscore the importanceof a multidisciplinary approach in managing these rare concurrent conditions, ultimately optimizing hearing preservation and overall quality of life.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":"79 4","pages":"40-47"},"PeriodicalIF":0.0,"publicationDate":"2025-07-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144693122","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Surgical treatment of skeletal deformities of the facial part of the skull may lead to changes in the distance between the mandibular condylar processes.Aim: The aim of this study is to mathematically assess the impact of orthognathic procedures on the position of mandibular condylar processes based on measurements obtained from imaging studies.Materials and methods: A group of 30 individuals with skeletal Class II or III underwent bimaxillary orthognathic surgery (BiMax). Preoperative diagnostics and postoperative outcomes (6 months after surgery) were evaluated using computed tomography. Statistical analysis was performed on the obtained measurements.Results: The results showed that the absolute distance between mandibular condylar processes decreased following treatment. Statistically significant results were observed only in patients with skeletal Class III at a significance level of α=0.05.Conclusions: Changes in the absolute distance between mandibular condylar processes indicate an indirect impact of orthognathic surgery on these structures. The presented results may serve as a basis for further consideration of the correlation between the discussed medical procedures and changes occurring in adjacent anatomical structures not directly affected by the surgery.
","PeriodicalId":520738,"journal":{"name":"Otolaryngologia polska = The Polish otolaryngology","volume":"79 4","pages":"29-33"},"PeriodicalIF":0.0,"publicationDate":"2025-07-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144661719","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
