Bullous pemphigoid (BP) is an autoimmune blistering disease associated with autoantibodies against BP180 and BP230. Previous studies have suggested an increased incidence of BP in patients with ALS. However, the neurological status and disease severity of patients with ALS at the time of BP onset remain insufficiently characterized. This study aimed to investigate the frequency and clinical characteristics of BP in patients with ALS.Methods In this retrospective study, we reviewed the medical records of 1,067 patients with ALS who were admitted to our institution between April 2010 and May 2025. Patients who developed BP after the onset of ALS were identified. BP was diagnosed by dermatologists based on clinical findings and, when available, by immunological or histopathological examinations. Disease severity at BP onset was assessed based on the ambulatory status and ventilatory support requirements.Results Seven of the 1,067 patients (0.66%) developed BP after ALS onset. The median interval between ALS and BP onset was 8.0 years (IQR 5.0-16.5). At BP onset, four patients (57.1%) were bedridden, four (57.1%) required tracheostomy and invasive ventilation, and two (28.6%) required non-invasive ventilation. Five patients (71.4%) required enteral feeding. The median age at BP onset was 68.0 years (IQR, 64.0-72.5). BP180 antibody testing was positive in three of the six patients. The lesions most commonly involved both the extremities and the trunk.Conclusion BP may occur more frequently in ALS patients than in the general population. Clinicians should consider BP when evaluating cutaneous lesions in patients with ALS.</p>","PeriodicalId":520650,"journal":{"name":"Internal medicine (Tokyo, Japan)","volume":" ","pages":""},"PeriodicalIF":1.1,"publicationDate":"2026-10-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149187816","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A comparison of the complementary DNA and genomic DNA sequences of UBTF-TD revealed a 60-bp insertion associated with aberrant splicing and partial retention of intron 13. Pleural fluid cell block immunohistochemistry showed cytokeratin AE1/AE3 negativity and SATB2 positivity, suggesting a mesenchymal tumor. Pelvic imaging suggested a chondroid matrix, while a biopsy revealed a high-grade osteoid-producing component with an IDH1 p.R132C mutation. We assessed the safety and clinical utility of TBLC in older adult Japanese patients with suspected interstitial lung disease (ILD). Methods We retrospectively reviewed the medical data of patients who underwent TBLC with a 1.7-mm cryoprobe for suspected ILD at the National Hospital Organization Okayama Medical Center between June 2022 and August 2024. Data on complications, diagnostic yield, and treatment after TBLC were collected and analyzed. Patients The patients were divided into two groups: an older adult group (OA group; age ≥70 years; n=31) and a non-older adult group (NOA group; age <70 years; n=19). Results The median age of the patients in the OA group was 72 years. Pneumothorax occurred only in the NOA group (p=0.017), whereas the frequencies of other complications did not differ significantly. No severe complications were noted. The severity of bleeding did not differ significantly between the groups, regardless of the hemostatic method used (p=0.690). The overall diagnostic yield was 82% (OA group: 84%; NOA group: 79%; p=0.715). Idiopathic pulmonary fibrosis was significantly more common in the OA group than in the NOA group (p=0.035), whereas hypersensitivity pneumonitis was significantly more common in the NOA group than in the OA group (p=0.013). While men have higher risks of severe acute COVID-19 and mortality, epidemiological studies consistently show a female predominance in Long COVID, especially among middle-aged women. Interpretation is complicated by variable case definitions (World Health Organization, Centers for Disease Control and Prevention, National Institute for Health and Care Excellence) and variant-era differences; however, sex disparities persist across both pre-Omicron and Omicron periods. We review evidence on sex differences in epidemiology, symptom clusters, and longitudinal trajectories, incorporating recent RECOVER findings and emerging data from Japanese cohorts. Potential mechanisms include sexually dimorphic antiviral immunity, hormonal and X-chromosome-linked immunoregulation, autoimmunity, autonomic dysfunction, and gender-related social exposures and health-care access. Recognizing sex- and gender-related determinants can improve differential diagnosis, risk stratification, and individualized management, and should be embedded as core variables in future mechanistic studies and clinical trials.</p>","PeriodicalId":520650,"journal":{"name":"Internal medicine (Tokyo, Japan)","volume":" ","pages":"2652-2657"},"PeriodicalIF":1.1,"publicationDate":"2026-10-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147358587","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
