We conducted inductive reflexive thematic analysis to develop themes and subthemes from the data, with the overarching goal of informing clinical practice and future research. RESULTS: Just over half (55%) of participants completed the combined exercise and diet program, while 45% (ineligible for the diet program) completed the exercise program only. Five themes were developed: (1) clinician guidance was paramount to success (provided accountability, provided encouragement and support); (2) ease and convenience of program (weight loss diet and meal replacements, video consultations); (3) challenges with exercise program (difficult exercises; cuff weights were difficult to use; adherence was a challenge for some; preference for some in-person contact with physical therapist); (4) useful resources; and (5) favorable outcomes (reduced hip pain and improved mobility, weight loss, increased knowledge and confidence). CONCLUSION: Participants had positive experiences with the intervention, particularly valuing clinician support, its overall ease and convenience, and the useful resources. Some found engaging with the exercise program to be challenging. JOSPT Open 2026;4(2):204-214. There were 2 studies of BFRT after distal radius open reduction and internal fixation (ORIF), and single studies of BFRT after ankle ORIF, high tibial osteotomy, total knee arthroplasty, shoulder stabilization, and distal biceps repair. BFRT was initiated within 2 weeks postoperatively in 16 studies. Quadriceps and hamstring strength, measured in Newton-meters per kilogram, was assessed at 60°/s, 180°/s, and 300°/s at baseline and final follow-up. At final follow-up, knee pain, joint effusion, and range of motion deficits were assessed. RESULTS: Of 28 patients, 23 completed the study. Study groups were comparable at baseline. Before randomization, expectations were measured using a questionnaire that captured patients' views about the effect of common OA treatments to reduce knee pain using a 5-point Likert scale ranging from \"completely disagree\" to \"completely agree.\" Responses regarding joint mobilization and massage were dichotomized into high and low expectations. The primary outcome was pain intensity at 9 weeks measured by the Numeric Pain Rating Scale. Linear regression models were used to test whether treatment expectations moderated manual therapy treatment response. Models were conducted separately for each of the two potential moderators (joint mobilization and massage) and adjusted for the same covariates associated with the outcomes of the original trials. RESULTS: Primary regression analysis showed no significant interaction effect between manual therapy and treatment expectations on pain outcomes at 9-weeks follow-up (mobilization: β = -0.17, 95% confidence interval [CI]: -0.90 to 0.56; massage: β = -0.26, 95% CI: -1.00 to 0.47). Despite the strong evidence linking sleep disturbances with pain and disability, sleep is rarely addressed systematically. Physical therapists have the opportunity to screen for sleep dysfunction, provide behavioral education, and refer patients to sleep medicine specialists when necessary. CLINICAL QUESTION: How can physical therapists effectively integrate sleep health assessment and behavioral strategies into the management of individuals with musculoskeletal pain conditions? KEY RESULTS: This commentary provides a pragmatic framework for addressing sleep dysfunction in physical therapy. A stepped approach to sleep screening is recommended, focusing on key questions and validated tools to identify insomnia, obstructive sleep apnea, and restless legs syndrome. The role of cognitive behavioral therapy for insomnia (CBT-I) is highlighted, with an emphasis on 4 domains: reducing presleep arousal, increasing sleep drive, entraining circadian rhythm, and promoting sleep hygiene. Sleep hygiene alone has limited impact, but when integrated with other strategies, it supports broader behavioral change. CLINICAL APPLICATION: Physical therapists should routinely assess sleep as part of a comprehensive pain management strategy. Integrating sleep health promotion, screening tools, and appropriate referral pathways into practice supports person-centered pain care. Enhanced training in sleep health for physical therapists will support the delivery of evidence-informed sleep support. JOSPT Open 2026;4(1):12-22. Forty studies compared sex-specific asymmetries. Fourteen studies reported significant sex differences in healthy athletes in asymmetry in hip abductor strength, explosive power, and kinetic measures commonly in females, as well as in quadriceps and hamstring strength in both males and females, while 11 studies found no sex differences. Five studies investigated asymmetries in healthy athletes with a history of injuries and reported sex differences in explosive power, and no sex differences for balance and knee muscle strength. Asymmetry in explosive power was more commonly noted in female healthy athletes (peak power [Females: 17.21 (0.07) > males: 12.81 (0.06)], P<.05, effect size = 0.33) and healthy athletes with a history of injury (asymmetry (%) [males: 95.4%, P = .003; females: 96.5%, P = .049]). A meta-analysis was not performed due to study heterogeneity. No studies assessed sex-specific asymmetry under exercise-induced fatigue. CONCLUSION: Sex-specific biomechanical lower-limb asymmetry remained inconsistent, with generally no sex differences except asymmetry in explosive power, which was more commonly reported in female athletes. High-quality studies are needed to address these gaps. JOSPT Open 2026;4(1):36-60. TMPT included six therapist-guided myofascial massage sessions; exercise training involved resistance-based neck and shoulder exercises. The primary outcome was pain intensity (100-point visual analogue scale) at 6 months. Secondary outcomes included disability, Global Rating of Change, sleep, psychological health, and adverse events. RESULTS: At the primary endpoint (6 months), there was no significant between-group difference in current pain intensity mean difference (MD; MD = 0.05 [-7.80, 7.97], P = .990) between TMPT and exercise. To date, research studies have identified numerous clinical phenotypes, but with very limited guidance on how to integrate phenotyping into physical therapy clinical practice. Phenotyping focused on distinct clinical profiles of psychological distress in patients with osteoarthritis offers a timely and relevant example of how to integrate this innovative approach into care. CLINICAL QUESTIONS: How can psychological phenotyping be incorporated into clinical practice at initial evaluation to inform decision making in the management of osteoarthritis? What are common challenges that must be overcome to successfully deliver this approach to care? How can our experiences with psychological phenotyping inform different applications of clinical phenotyping in physical therapy? KEY RESULTS: We implemented psychological phenotyping within a physical therapist-led, whole-person care model of osteoarthritis management. In this model, clinicians assigned psychological phenotypes using a standardized assessment and algorithm and are offered care pathway recommendations that address phenotype-specific psychological traits. Implementation challenges included phenotype assignment uncertainty, collection of valid and complete assessment data, avoiding stigma associated with classifying patients, and changing clinician behavior. CLINICAL APPLICATION: Phenotyping has the potential to improve the consistency and scalability of tailored care for musculoskeletal conditions, in this case related to management of psychological distress among patients with osteoarthritis. Lessons from this example can aid in implementation of phenotyping programs that consider additional biopsychosocial domains and inform translation of this framework to other musculoskeletal conditions. JOSPT Open 2026;4(1):1-11.
