{"title":"Parathyroid carcinoma secondary to prolonged hyperplasia in chronic renal failure and in coeliac disease.","authors":"N H Boyle, C S Ogg, R B Hartley, W J Owen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The presentation of parathyroid carcinoma in patients with chronic renal failure is rare, although with improvements in life expectancy associated with this condition there have now been 12 reported cases, including the first case we report here. It has been proposed that in these cases there has been a malignant transformation of benign parathyroid hyperplastic tissue. We also report the first case of parathyroid carcinoma associated with coeliac disease and suggest that the same mechanism may be responsible. A case of breast cancer arising during pregnancy, progressing rapidly with fungation despite induction chemotherapy. This 35-year-old woman who developed breast cancer in the 7th month of pregnancy had a Caesarean delivery, and chemotherapy started (mitozantrone as a single agent, followed by triple agent therapy). The tumour progressed rapidly to fungation during the 5-month period of chemotherapy, leading to consideration of what further treatment might be helpful. The clinical situation at this time is shown again in Fig. 1. Patient 2. A 32-year-old single woman, a professional violin player presented with Paget's disease of the right nipple. Mammography showed typical changes of intraduct carcinoma in both breasts, without clear indication of invasive cancer. A 46-year-old female presented with a huge benign lipoma of the left axilla/breast in 1977. Repeated excisions led to three recurrences, the first reported as an intramuscular lipoma, the latter two as well-differentiated liposarcoma. She presented in 1990 with a large recurrence partially fixed to the chest wall. Patient 2. A 58-year-old man with cardiorespiratory impairment due to long-standing valvular disease presented in 1982 with a large poorly differentiated sarcoma over the lower posterior rib cage. He was treated by wide local excision (not including the ribs), and cover with a latissimus dorsi flap to allow completion of a radical perioperative course of radiotherapy. Recurrences around the periphery of the excision occurred in 1984, 1985, 1986, 1987, each treated by relatively wide local excision.
