{"title":"Intra Operative Ziehl-Neelsen Test to Establish Diagnosis of Osteoarticular Tuberculosis Case: A Case Report and Literature Review from Multifocal Ostearticular Tuberculosis Case","authors":"Aryadi Kurniawan, Samuel Maruanaya","doi":"10.18231/j.ijrimcr.2019.007","DOIUrl":"https://doi.org/10.18231/j.ijrimcr.2019.007","url":null,"abstract":"Background: Multifocal osteoarticular tuberculosis, one of the various forms of extrapulmonary tuberculosis, is uncommon. The symptoms are nonspecific, and the disease often presents with an indolent clinical course, usually leading to a delayed diagnosis as well as bone and joint destruction. Till now, golden standard of diagnosis is made by confirming Mycobacterium organism from biopsy. Methods: We deliver a case of sixteen-year-old boy patient with pain on bilateral ankle and left elbow since 1 month before admission, there was no history of previous trauma or known infection. Radiologic examination has been conduct but unconcluded. He was given antibiotic and NSAIDS but still yet to improve clinically. He then went to surgically open biopsy and run examination using wet prep dye from bone biopsy sample with Ziehl–Neelsen stain, compare by Acid Fast Bacillus (AFB) culture, PCR and histopathologic test. Result: Acid fast bacillus was found on wet prep dye with Ziehl-Neelsen stain in sample from left distal tibia. Diagnosis was made multifocal osteoarticular tuberculosis, which confirmed using AFB culture, PCR and histopathologic examination. We have done debridement and the patient receive anti tuberculosis therapy for nine months. {"title":"Impact of Instructional Programme on Knowledge Regarding Protein Energy Malnutrition among the Mothers of Preschool Children from Selected Rural Areas of Karnataka: A Pilot Study","authors":"Rangappa. It is a major public health problem in India. It affects particularly the preschool children (<6 years) with its direct consequences ranging from physical to cognitive growth and susceptibility to infection. This affects the child at the most crucial period of time of development which can lead to permanent impairment in later life. According to WHO, globally 162 million under-fives were stunted among them 56% lived in Asia and 36% in Africa, 99 million under-fives were underweight among them 67% lived in Asia and 29% in Africa. 50 million under five children were wasted and 17 million were severely wasted. Approximately 71% of them lived in Asia and 28% in Africa, with similar figures for wasted children 69% and 28% respectively. Children who suffer from wasting face a markedly increased chance of death. According to United Nations International children’s Emergency Funds (UNICEF), 13% of children under five years old in the developing world were wasted and 5% were extremely wasted. UNICEF estimates that in developing world 129 million under five children were underweight, nearly one in four and ten percent of them being severely underweight. The prevalence of underweight is higher in Asia than in Africa, with rates of 27 and 21 % respectively. Methods: The quantitative research approach was used for the study. The present study adopted mixed research design in two stages, first stage uses Cross Sectional Descriptive Survey for the identification of preschool children suffering with protein energy malnutrition and second stage Randomized Pretest Posttest Control Group Design for evaluating the Impact of instructional program on knowledge regarding protein energy malnutrition among the mothers of preschool children from selected rural areas of Karnataka was adopted. Nola Penders Model was used as a conceptual framework for the study. Cross sectional survey was conducted in the selected rural areas of Dharwad to screen the preschool children to find the prevalence of protein energy malnutrition in them. A total of 50 (10% of total population of main study) mothers of preschool children were selected as per sampling criteria. 25 each mothers were distributed in experimental and control groups. Structured knowledge questionnaires were used to collect the data from study participants. The results were described by using descriptive and inferential statistics. Results: In the experimental group, Pretest knowledge score mean was 14.16, median was 15, mode was 15, standard deviation 3.27 and score ranged between 8-19. Where as in Posttest, mean was 26.20, median was 27, mode was 15, standard deviation was 2.70 and score ranged between 17-29. {"title":"Management of Carpal Tunnel Syndrome by Agnikarma-A Case Study","authors":"Samaranayake Gvp, K. The condition occurs when one of the major nerves to the hand, the median nerve is squeezed or compressed as it travels through the carpal tunnel. In most patients, carpal tunnel syndrome gets worse over time, so early diagnosis and treatment are important. If pressure on the median nerve continues, however, it can lead to nerve damage and worsening symptoms. To prevent permanent damage, surgery to take pressure off the median nerve may be recommended for some patients. Ayurveda has an advocated various treatment modalities such as Snēha, Upanāha, Agnikarma and Bandhana. Amongst these, Agnikarma seems to be more effective in providing distinct and instant relief. A 40 year-old-female patient was presented with severe pain, numbness and tingling in the hand once she is doing day to day cooking work and she complained that exacerbation of pain at night for 8 months. On examination, she was diagnosed as she is having carpal tunnel syndrome. After careful examination, patient was treated with Agnikarma without any oral medication. Superficial multiple wounds at the area of carpal flexor tunnel produced by Agnikarma healed within 5-7 days. Patient was advised not to lift any heavy objects and perform any twisting movement for further 6 months. Patient visited for follow-up for 1 month after completion of treatment–Agnikarma. The patient noticed that pain, numbness and tingling in the hand were disappeared.
