We analyse the strategies of power employed by therapy participants to promote their preferred construction of the referral problem. Focusing on the intersections of gender, psychopathology and culture, we explore how wider sociocultural discourses play out in the micro-context of participants' discursive exchanges. We also consider how the therapist's interventions privilege and marginalise client discourses in both strategic and unintended ways. This article offers some clinical techniques to address the dilemmas and difficulties identified in our analysis. Overbeek","doi":"10.1111/1467-6427.12434","DOIUrl":"10.1111/1467-6427.12434","url":null,"abstract":"<p>The effectiveness of intense specialised multi-family therapy (ISMFT) for 111 multi-stressed families, and the therapeutic alliance as a possible predictor of outcome, were examined. A repeated measures design was used, where changes in all ISMFT phases (preparation, multi-family therapy and follow-up) were assessed and compared for both mothers and fathers. Evidence was found for improved family functioning after the therapy period, which was maintained at 3 months follow-up, although the multi-stressed families still functioned in the problematic range. The therapy did however not decrease parenting stress, or did so only temporarily. Observations of the therapeutic alliance with the System for Observing Family Therapy Alliances (SOFTA) scales indicated that high therapist engagement was related to positive therapy outcomes, both at the start of therapy and later. High family engagement also predicted therapy effectiveness, but only at the start of therapy. Despite growing evidence confirming the effectiveness of systemic family therapy in child and adolescent psychiatry, this approach is not well represented in inpatient services in Greece. We present systemic principles as practiced at a child and adolescent psychiatric inpatient clinic embedded in a general hospital. The study group consisted of 40 volunteer married parents (35 females, 5 males) between the ages of 30 and 56 who resided in Central Anatolia. We conducted a descriptive study using a phenomenological design. A thematic analysis method was used in the analysis of the data. It also seeks to identify gaps in the evidence to help guide future research and intervention development. Four databases (PubMed, Web of Sciences, Scopus and PsycInfo) were systematically searched and analysed, resulting in five publications for inclusion. The main findings evidence two main themes: characteristics of multifamily interventions, and evaluation of the intervention and impact on participants. These findings suggest that multifamily interventions can provide families with extended support beyond the care usually offered by genetic healthcare services and contribute to enhancing positive change in key aspects of adaptation and coping with an inherited genetic conditions. Multifamily discussion groups are a methodology that is particularly suitable when working with families in this context. Collaboration between genetic healthcare services and accredited systemic family therapists is required to sustain the implementation of multifamily discussion groups and their feasibility. The work draws on the use of personal presence in resistance movements of the twentieth century, focusing on the role of the caregiver to increase their presence through acts of resistance and care. This paper investigates the experiences of professionals using NVR in one UK residential care home. Eight participants took part in semi-structured interviews, which were analysed thematically. Analysis identified four overarching themes: NVR is both a set of processes and a way of being, NVR and transformation, NVR and the personal–professional divide and NVR and organisational support. The findings suggest that NVR offers an effective and acceptable alternative to behavioural approaches. As part of a larger research project, this prospective cohort study explores what promotes and hinders the development of Open Dialogue in network meetings (ODNM) based on the experiences of thirty-seven clinicians and seven supervisors. Multistage focus group interviews were used to collect data and were analysed thematically. We generated two main themes: (1) togetherness and isolation and (2) challenging and evolving. Findings are organised using a framework of problems, possibilities, resources and restraints (PPRR, Neden &amp; Burnham, 2007). Despite variation in therapist confidence in online practice, all participants found new possibilities in this way of working, including connecting family members across distance, increased co-construction within therapeutic relationships and engaging clients who would not usually attend appointments. Therapeutic alliance was generally possible to establish online, though felt more challenging with whole families than individuals. Problems and restraints included therapist fatigue, risk and safety management, and attuning to nuanced expression of emotion. Implications for practice and future research are proposed.
