Her electrocardiogram (ECG) showed extensive anterior wall -ST elevation myocardial infarction (MI). She was treated with thrombolytic therapy in the form of tenecteplase and was given standard treatment for acute coronary syndrome (ACS). Her history revealed three recurrent pregnancy losses and a history of preeclampsia. Echocardiography showed regional wall motion abnormalities of the mid, distal, apex, and anterior wall with severe left ventricular systolic dysfunction. Routine blood investigations showed elevated total leukocyte count with neutrophilic predominance. With a history of recurrent pregnancy loss, she was investigated for anti-phospholipid anti-phospholipid antibody syndrome (APS) and was found to have positive lupus anticoagulant 1. She underwent coronary angiography (CAG) which showed a left main shaft 40% stenosis with an ulcerative lesion containing thrombus in the ostioproximal to mid-left anterior descending artery (LAD), other coronary arteries were normal. She was managed conservatively with glycoprotein IIb/IIIa inhibitor (Tirofiban) and switched to oral anticoagulation with Vitamin K antagonist (VKA-Acenocoumarin) along with dual-antiplatelet therapy.

The sample size was calculated as 256. First, a basic health assessment of the study population was done. This focused on basic socio-demographic data of the adult women, their health status and blood pressure reading, risk profile, treatment-seeking behavior, etc. A proforma was developed to record the data. The principal investigator did a level of prevention-based color-coded stratification for HTN patient segmentation.\u0000\u0000\u0000\u0000The majority of respondents were married (75.4%). Their literacy rate was 72%. The majority of the respondents were homemakers (58.2%). The mean age of the respondents was 38.23 years (range 18–75 years). Monthly household income ranged from Rs. 1,000 to 60,000, with a mean of Rs. 10,611.72. The average family size was 4.61 members. Some (22%) of the respondents fell into the “Health promotion” category (Green), indicating that they were free of HTN. About 39% of respondents were categorized under “Specific protection” (Blue), signifying that they were at risk of developing HTN; 14% were diagnosed with HTN cases without apparent complications, classified as “Early Diagnosis/Treatment” (Yellow). About 21% of respondents were placed in the “Disability Limitation” (Orange) category, indicating some complications, while 4.3% were identified as HTN patients needing “Rehabilitation” (Red) due to serious complications.\u0000\u0000\u0000\u0000This approach of the color-coded stratification based on levels of prevention among adult females of UHTC revealed the spectrum of HTN in the general community. Overall, 39% of the respondents had HTN. About two-fifths of respondents were at risk of developing HTN. Tammiraju, Bhuvaneswari Ramisetty, Alekhya Jasmine Gujjarlapudi
\u0000\u0000Patients with unstable angina and non-ST-elevated myocardial infarction (NSTEMI) present with a spectrum of cardiac events and death. Hence, early accurate management should be initiated. This can be achieved by risk stratifying the patients as early as possible on hospital admission. Thrombolysis in myocardial infarction (TIMI) risk score assessment is used widely for this purpose. It helps to standardize the approach of management and compare the extent and number of vessels involved in the coronary angiogram. This research aims to assess coronary artery disease (CAD) in individuals with NSTEMI by examining the extent of vessel involvement and the complexity of coronary artery blockages, using the SYNTAX score about the TIMI risk score.\u0000\u0000\u0000\u0000This cross-sectional observational study involved 60 participants who were selected based on specific inclusion and exclusion criteria. The TIMI risk score was computed for each individual, categorized as either <4 or ≥4, and the results of their coronary angiogram were analyzed to assess the degree of vessel involvement, categorizing it as either single-vessel disease (SVD) or multi-vessel disease (MVD). The Syntax score was computed for all angiograms and divided into two groups based on whether it was ≤22 or >22.\u0000\u0000\u0000\u0000The average age of the participants in our study was 56 ± 10.64, with an age range of 34. Gender distribution revealed a higher proportion of males, constituting 68.3% (41 individuals), while females accounted for 31.7% (19 individuals). The angiogram results demonstrated that among the patients with TIMI score <4, 70% (14 individuals) had SVD, 30% (6 individuals) had MVD, and among those with score ≥4, 15% (6 individuals) had SVD while 75% (34 individuals) had MVD. In the subgroup analysis of TIMI <4, a Syntax score <22 was observed in 15 cases of SVD and a score >22 was observed in cases of MVD. Likewise, in the TIMI >4 group, a Syntax score <22 was observed in 12 cases of SVD and a score >22 was observed in 28 cases of MVD.\u0000\u0000\u0000\u0000This study noted that patients with non-ST-segment elevation acute coronary syndrome who underwent coronary angiogram and had a TIMI score of ≥4 were observed to have a 13-fold increased likelihood of having multi-vessel Coronary artery disease (CAD) compared to those with a TIMI score of <4. TIMI score of ≥4 had a 7 times higher chance of getting a SYNTAX score >22.\u0000
