{"title":"Hyperthermic intraperitoneal chemotherapy (HIPEC) in the management of ovarian cancer","authors":"Radha Graham, Nicola D Macdonald, Tim A Mould, Ioannis C Kotsopoulos","doi":"10.1111/tog.12916","DOIUrl":"https://doi.org/10.1111/tog.12916","url":null,"abstract":"\u0000Hyperthermic intraperitoneal chemotherapy (HIPEC) at the time of cytoreductive surgery is a proposed additional treatment in the management of advanced tubo‐ovarian cancer.\u0000Additional organisational approvals, costs, changes to theatre infrastructure and pre‐, intra‐, and post‐operative management considerations are required to implement HIPEC in a clinical setting.\u0000Current meta‐analysis data suggest that in the setting of interval cytoreductive surgery, HIPEC prolongs overall survival and progression‐free survival. There is currently no evidence of benefit in the setting of primary surgery or recurrent disease, but results of ongoing clinical trials are pending.\u0000\u0000To understand the mechanism of action, safety and efficacy of HIPEC as an adjuvant treatment in tubo‐ovarian cancer.\u0000\u0000Because of variation in treatment protocols and heterogeneity of reported outcomes, HIPEC has not yet been adopted into standard practice. First‐line medical management is appropriate for women with biopsy‐confirmed vulval high‐grade squamous intraepithelial lesions who have been adequately counselled on the risk of occult invasive disease. Topical treatment should be undertaken in the context of a specialist clinic because it is not suitable for all patients. Surgical management is needed for differentiated vulval intraepithelial neoplasia. To understand the clinical, histological, and pathological classification of squamous intraepithelial lesions and diagnostic challenges. To be able to contribute to the multidisciplinary investigation and diagnosis of squamous intraepithelial lesions. To understand the basis for patient‐centred counselling on the investigation, diagnosis and treatment of suspected squamous intraepithelial lesions. Indications for FGCS can be broadly divided into cosmetic and functional reasons. Counselling on the risks and cosmetic outcomes, as well as sexual and functional results, should be provided, and the woman's expectations discussed. To develop a holistic view when discussing cosmetic gynaecology concerns, including psychosexual assessment. To understand the procedures that are currently available, the evidence‐base behind these procedures, and their risks and benefits. To understand the implications of genital cosmetic surgery on patients, including their psychosexual wellbeing, and the legality of these procedures with reference to female genital mutilation (FGM). The ‘postcode lottery’ in commissioning cosmetic gynaecological procedures in the UK leads to the question: should they be available on the NHS? Who should be performing cosmetic gynaecological procedures: the plastic surgeons who do little vaginal surgery, or the gynaecologists?
