With the recent development of lamellar keratoplasties, eye banks play an even bigger role in the selection and preparation of donor tissues. The increasing number of keratoplasty techniques and the high demand for “ready-to-use” tissues are challenging eye banks to improve and develop new preparation techniques. Besides necessary examinations, new approaches of tissue analysis in eye banks allow a better/optimized selection of corneal tissues. Alio del Barrio, Ronald Steven II Medalle, Matteo Pederzolli","doi":"10.4103/tjo.tjo-d-23-00168","DOIUrl":"https://doi.org/10.4103/tjo.tjo-d-23-00168","url":null,"abstract":"\u0000 The purpose of this study was to describe a case and clinical course of simultaneous anterior chamber phakic intraocular lens (AC-pIOL) removal, implantation of a posterior chamber phakic intraocular implantable collamer lens (ICL), and Descemet membrane endothelial keratoplasty (DMEK). This was a case report of a 44-year-old male with a unilateral decompensated cornea from a displaced Duet–Kelman lens on his left eye that underwent pIOL extraction and implantation of a posterior chamber phakic intraocular ICL and simultaneous DMEK. After 6 months, the cornea of the left eye had regained clarity, the Descemet membrane was graft stable, and the ICL implanted was centered with good vault. Postoperative anisometropia was avoided. There was no development of cataracts or other complications. The simultaneous pIOL extraction, ICL implantation, and DMEK in our case showed good results with full restoration of anterior segment anatomy and return of transparency of the cornea. Before CXL was introduced, patients would need specialty contact lenses and possible corneal transplantation. CXL involves a biochemical reaction in which ultraviolet A light is used in conjunction with Riboflavin to form crosslinks in between corneal stromal collagen. This leads to strengthening and stabilizing of the collagen lamellae, resulting in mechanical stiffening of the cornea. Multiple protocols have been proposed including epithelium on versus off and varying light intensity and duration of treatment. All protocols appear to be safe and effective with few reported complications including infection, stromal haze, scarring, and endothelial toxicity. Overall, CXL has demonstrated to halt the progression of the disease clinically and in keratometry readings and improve the quality of life for patients. This systematic review aims to investigate the association between radiation exposure and the incidence of secondary neoplasms in retinoblastoma survivors. A comprehensive literature search was conducted using PubMed, ProQuest, and EBSCOhost databases to gather relevant studies. Studies written in English about the incidence of secondary neoplasms in retinoblastoma patients who underwent radiotherapy were included. Retinoblastoma patients who did not receive radiotherapy were regarded as the control group. Studies involving animals and those without a control group were excluded. No date restrictions were applied. The last search was performed on April 6, 2022. The study selection process was presented in a flow diagram following the Preferred Reporting Items of Systematic reviews Meta-Analysis 2020 guidelines. The risk of bias was evaluated using the Risk of Bias in nonrandomized Studies of Interventions tool. Statistical analysis was performed using the Cochrane RevMan software (version 5.4.1). Five studies comprising a total of 3324 retinoblastoma patients were included. Of these, 290 (8.72%) patients developed secondary neoplasms, with 250 (86.21%) receiving radiotherapy. The results indicated that retinoblastoma patients who received radiotherapy had a higher risk of developing secondary neoplasms, with a pooled risk ratio of 5.12 (95% confidence interval 3.56–7.36, I\u0000 2 = 14%). The findings of this review highlight the importance of considering the risk of developing secondary neoplasms when treating retinoblastoma. The used surgical techniques were the Scharioth technique for intrascleral tunnel fixation, Yamane technique (double-needle scleral fixation), and modified Yamane technique (double-needle flanged haptic scleral fixation). The IOL models and surgical instruments used as well as each patient’s ocular characteristics and complication management were recorded.\u0000 \u0000 \u0000 \u0000 Of the eight included patients, the complications of 3 (37.5%) and 5 (62.5%) were noted intraoperatively and postoperatively, respectively. Haptic-related complications, including haptic breakage, slippage, and haptic disinsertion, occurred in six eyes. Other complications included uveitis–glaucoma–hyphema syndrome, retinal detachment, and IOL tilt. For the two patients with haptic slippage, repositioning was achieved using a modified cow-hitch technique that resulted in favorable IOL centration and restored visual acuity.\u0000 \u0000 \u0000 \u0000 Most complications surgeons encountered during their early exposure to SIS IOL fixation were haptic related. Surgeons should be aware of such complications to prevent and manage them during surgery. For the two patients with haptic slippage, repositioning was achieved using a modified cow-hitch technique that resulted in favorable IOL centration and restored visual acuity.\u0000 \u0000 \u0000 \u0000 Most complications surgeons encountered during their early exposure to SIS IOL fixation were haptic related. Surgeons should be aware of such complications to prevent and manage them during surgery.
