Therefore, we sought to investigate the association between the diagnosis of TB and the subsequent risk of uveitis in Taiwan.\u0000 \u0000 \u0000 \u0000 Data from the Taiwan National Health Insurance system were retrieved and patients with newly diagnosed TB between 2000 and 2012 were recruited. The endpoint of interest was the occurrence of uveitis. Patients without TB were randomly matched 4:1 to TB cases based on age, gender, index date, outpatient clinic visit, and index year. Univariate and multivariable Cox proportional regression analyses were performed to analyze the risk of uveitis among TB patients.\u0000 \u0000 \u0000 \u0000 A total of 6139 patients with TB and corresponding 24,555 matched control participants were recruited. The mean age was 52.9 ± 22.1 years old and 32.1% were male. The medium follow-up period was 5.81 ± 4.37 years and 7.16 ± 3.95 years in the TB and matched control cohorts. Our results showed that patients with TB had no significantly increased incidence of uveitis. After stratification by gender, age, and comorbidities, the relationship between TB and uveitis was found to be not significant. The cumulative incidence of uveitis was also found to be not significantly higher among the TB group (log-rank P = 0.84).\u0000 \u0000 \u0000 \u0000 Our nationwide population-based cohort retrospective study showed that the incidence of uveitis was not significantly higher among patients with TB. CCH are rare benign vascular hamartomas without systemic associations. We herein describe the treatment of a case of a pediatric patient with CCH associated with an exudative retinal detachment. His treatment consisted of brachytherapy with a plaque loaded with 125 Iodine seeds with a target apex dose of 40 Gy. Preoperative visual acuity (VA) in the affected right eye was 20/800. After treatment and for 24 months of follow-up, the VA improved to 20/200, with no signs of radiation retinopathy or recurrence of subretinal fluid. A 49-year-old male patient exhibited progressive right eye bulging, blurred vision, diplopia, and ophthalmoplegia. Imaging revealed a tumor involving the orbital space, nasolacrimal duct, and maxillary sinus. The patient underwent tumor excision, revealing lymphoepithelial carcinoma on histopathology. Immunohistochemistry supported the diagnosis with EBV reactivity. After then, the patient received concurrent chemoradiotherapy following orbital tumor excision. Subsequent imaging indicated tumor regression over orbit and periocular regions. Although parotid gland metastasis was detected, we arranged parotidectomy and adjuvant therapy, resulting in no recurrence and symptomatic improvement six months after treatment. This case extends the existing understanding of clinical manifestation of lymphoepithelial carcinoma. The unique presentation of lymphoepithelial carcinoma originating from the orbit emphasizes the importance of considering rare manifestations when approaching complex clinical scenarios. The correlation between EBV infection and lymphoepithelial carcinoma in the context of Asian populations was discussed, although not entirely confirmed in this case. Treatment strategies were influenced by the rarity of the tumor, combining surgical resection, induction chemotherapy, and concurrent chemoradiotherapy. This report presents a case of lymphoepithelial carcinoma originating from the orbit, adding to the understanding of the clinical spectrum of this rare malignancy. The management showcases a multidisciplinary strategy that considers the tumor’s rarity and aggressive behavior. This complex scenario encompassed simultaneous bilateral RVO occurrences and subsequent unilateral recurrences. Despite the absence of systemic comorbidities or conventional risk factors, genetic assessment unveiled notable heterozygous mutations involving Factor V Leiden (G1691A) and Factor II (PT G20210A), along with a homozygous methylenetetrahydrofolate reductase A1298C mutation. While routine hematological indicators remained largely unremarkable, the marked efficacy of anti-vascular endothelial growth factor (anti-VEGF) agents, particularly when administered promptly, emerged as a noteworthy therapeutic avenue. An intravitreal dose of 2 mg/0.08 mL was administered in the 1st week. Considering the suboptimal response, the dose of intravitreal ganciclovir was increased to 4 mg/0.08 mL. After 4 weeks of therapy (four doses of 4-mg ganciclovir), the patient complained of worsening of vision. Fundus examination showed resolving retinitis, but the optical coherence tomography (OCT) showed neurosensory detachment (NSD) at the fovea in both eyes with inner retinal hyperreflectivity in the right eye. Considering the possibility of retinitis involving fovea, a higher dose of intravitreal ganciclovir (6 mg/0.08 mL) was administered. The patient reported further worsening of vision immediately after removing the eye patch 2 hr following the injection. OCT showed a full-thickness hyperreflective vertical band with ellipsoid zone disruption at the fovea and inner retinal hyperreflectivity in the right eye. Hyperreflective vertical band with NSD was seen in the left eye. A possibility of macular toxicity due to intravitreal ganciclovir was considered. Intravitreal therapy was stopped and oral valganciclovir was continued. At 1-month follow-up, the patient reported improvement in vision. The hyperreflectivity and NSD were resolved at month follow-up. Here, we present a case of a male patient with a very unique combination of symptoms that started at the age of 12. Transient symptoms manifested after 4 days of high fever and included lethargy, a loss of appetite, and an inability to recognize faces. After that, the patient was symptom-free for 4 years before he was hospitalized again with a working diagnosis of acute disseminated encephalomyelitis. The ophthalmic symptoms at this stage were photophobia and discrete horizontal nystagmus. One year later, the patient had another seizure and during hospitalization, predominantly displayed cerebral symptoms such as dystaxia, dysmetria, generalized muscle hypotrophy, and absent myotonic reflexes. Like before, an ophthalmic examination demonstrated a best-corrected visual acuity of 1.0 and a normal anterior segment. A fundus examination revealed a slightly rarefied retina, along with areas of perifoveal and parapapillary atrophy, in addition to increased vascular tortuosity. Optical coherence tomography scans of the macula and the optic disc depicted a mild thinning of the retinal layers. Visual field testing showed an isopter contraction with a predominant loss of the temporal visual field in both eyes. A genetic serum analysis was positive for mitochondrial mutation m.3243A>G, and a diagnosis of MELAS was confirmed. The treatment included anticonvulsive drugs and dietary precautions. The ongoing international outbreak since mid-2022 has spread worldwide, including Taiwan. Ocular involvement in Mpox infection is uncommon, including external and ocular surface lesions. Here, we describe a man who developed unilateral blepharoconjunctivitis and preseptal cellulitis, followed by the appearance of skin symptoms 6 days after the ocular manifestations. Samples taken from his oropharynx and skin lesions tested positive for the Mpox virus through a polymerase chain reaction test. He was hospitalized for isolation with topical lubricant, antibiotic, and acyclovir eye ointment until the skin lesions healed. However, on the day of discharge, punctate epithelial keratitis was observed in the same eye. The corneal lesion also tested positive for the Mpox virus. His keratitis progressed to dendritic ulceration, and treatment with tecovirimat was initiated. Initially, his corneal ulcer responded well to tecovirimat, but 12 days later, it deteriorated along with cells in the anterior chamber. To treat his condition, low-dose steroid and ganciclovir eye drops were administered. The outcomes for patients with methanol-induced optic neuropathy are often unpredictable, with many cases resulting in severe and irreversible visual impairment. This case series aims to illustrate the diverse range of outcomes associated with methanol-induced optic neuropathy. This report describes three cases of methanol-induced optic neuropathy with varying outcomes despite similar conditions at presentation. One case involved methanol exposure through inhalation, while the other two cases resulted from methanol ingestion. Initially, all three cases presented with poor visual acuity, and no clear systemic manifestations were observed. The patients received high-dose corticosteroid treatment and neuroprotectors. Remarkably, the patient with the youngest age demonstrated the most significant improvement during follow-up, whereas the oldest patient, who was a smoker and alcoholic, exhibited the least improvement. This case series highlights the substantial differences in outcomes among these cases despite the identical treatment approach for all. The treatment outcomes for methanol-induced optic neuropathy may vary, even when patients present with similar severity and receive identical therapy. Age and lifestyle are considered prognostic factors affecting the outcome of therapy. This chapter intends to discuss the reported clinical results of using SMILE derived lenticules in terms of feasibility, safety and predictability; or the potential management of hyperopia, keratoconus, SMILE ectasia and presbyopia.\u0000 \u0000 \u0000 \u0000 Donor SMILE lenticules were prepared under microscope tocreate doughnut shaped lenticules. For hyperopia, this tissue was then inserted into afemtosecond laser enabled pocket created using VisuMax FS Laser at a depth of 160µm. For ectasia induced by keratoconus and post refractive procedure (SMILE), 0.23% riboflavin dye was instilled into the interface and then lenticule was inserted followed by exposure to UV-A radiation with total energy of 6.3 J.\u0000 \u0000 \u0000 \u0000 Spherical equivalent (S.E.) of within ± 0.5 D was observed in 50% (n=21) eyes and within 1 D was seen in 71% eyes treated for hyperopia. A significant increase in the K mean anterior, central corneal thickness, Q-value and corneal aberrations was seen 2 weeks post-op.
