Material and Methods Twenty-six patients with SLE, median age 32.5 (25–43) years, and 29 healthy individuals, median age 30 (23–32) years, were included. Exercise capacity (6-minute walk test (6MWT)), peripheral muscle strength (dynamometer), physical activity (Mi Band 4 smart band), fatigue (Fatigue Severity Scale (FSS)), anxiety and depression (Hospital Anxiety Depression scale (HAD)), quality of life (SLE-specific QOL scale (SLEQoL)) and sleep (Pittsburgh Sleep Quality Index (PSQI)) were assessed. Results Demographic characteristics were similar in both groups. 6MWT distance, peripheral muscle strength, physical activity level, SLEQoL and PSQI scores were significantly lower, while FSS and HAD scores were higher in patients with SLE (p<0.05). Moreover, pre-test and post-test heart rates, dyspnea and fatigue severity scores were higher in patients with SLE than in healthy individuals (p<0.05). Conclusion Patients with SLE had lower exercise capacity, muscle strength, physical activity, quality of life and sleep quality, along with more common fatigue, anxiety and depression. Strategies need to be developed to improve these symptoms in patients with SLE for a favourable outcome.","PeriodicalId":50831,"journal":{"name":"Aktuelle Rheumatologie","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-05-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48499757","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Im Zeitalter der Biologika läßt die medikamentöse Vielfalt multiple Variationen zu, um den Erkrankungsprozess zu verlangsamen oder gar zu stoppen [19]. Deshalb ist es wichtig geworden, bei fehlender Rückbildung der Prozeßaktivität im Sinne einer synovialen Schwellung an Sehnen oder Gelenken frühzeitig die Frage einer operativen Therapie zum Sehen- oder Gelenkerhalt beantwortet zu wissen [25]. Durch ein frühzeitiges Interagieren von medikamentöser und operativer Therapie kann dem betroffenen jungen Rheumatiker langfristig eine schmerzarme und wenig funktionsgeminderte Alltagstauglichkeit nicht nur der Hand geschaffen werden. Dabei hat der Sehnen- und Gelenkerhalt nicht nur an der Hand höchste Priorität vor den rekonstruktiven operativen Möglichkeiten.","PeriodicalId":50831,"journal":{"name":"Aktuelle Rheumatologie","volume":"48 1","pages":"203 - 211"},"PeriodicalIF":0.2,"publicationDate":"2023-04-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43685335","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods Adult female FMF patients who were followed up in our rheumatology clinic and had a history of pregnancy after the diagnosis of FMF were consecutively enrolled to this cross-sectional study between November 2021 and June 2022. Only pregnancies occurring after the diagnosis of FMF were evaluated. Patients who did not stop using FMF treatment after pregnancy were considered to adhere to treatment. Results In 50 patients, a total of 88 pregnancies and 58 live births were observed after the diagnosis of FMF. 37 (74%) had at least one completed pregnancy, and 13 (26.0) were pregnant during the study. Among the 37 patients, 34 had at least one live birth. No significant differences were observed when rates of patients with a history of at least one live birth, abortus, Cesarean section (C/S), preterm labor, stillbirth, intrauterine anomaly, congenital malformation and fetal growth retardation were compared between colchicine users and non-users. 60% of the patients adhered to colchicine treatment during pregnancy and 73.4% did so while breastfeeding. The most common reason for colchicine cessation was concerns regarding potential harm to the fetus (70%), followed by the presumption that colchicine is contraindicated in pregnancy (15%) as well as absence of FMF attacks (10%). The most common source of the patients’ opinion was a physician (60%). Conclusions Both patients and attending physicians should be informed to be more adherent to colchicine to avoid adverse outcomes and FMF-related complications.","PeriodicalId":50831,"journal":{"name":"Aktuelle Rheumatologie","volume":"1 1","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-04-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41783999","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and method Our study included 46 patients with ankylosing spondylitis (AS) and 45 healthy controls (HC). Bath Ankylosing Spondylitis Metrology Index (BASMI), Bath Ankylosing Spondylitis Functional Index (BASFI), Ankylosing Spondylitis Quality of Life Scale (ASQoL), Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Tampa Kinesiophobia Scale (TKS) scores, Timed Up and Go (TUG) test, maximum plantar pressure and spatiotemporal parameters (total weight transfer, step cycle duration, double-support phase, swing phase, step length, foot angle and cadence) were evaluated. Results Ninety-one individuals (46 AS, 45 HC) (age/sex/BMI) were included in our study. Statistically significant differences were seen between AS and HC groups in TUG, cadence, TKS, maximum pressure, step cycle duration, swing phase, step length, double-support phase, and foot angle measurements (p<0.05). Significant positive correlations existed between TKS and BASMI, BASDAI, BASFI, ASoQ, TUG and foot angle (p<0.05) in AS. Substantial negative correlations were found between TKS and maximum pressure, cadence and swing phase values (p<0.05) in AS. Conclusion Our study is the first in the literature to measure spatiotemporal parameters between patients with AS and HC and evaluate their relationship with kinesiophobia, which was done to the best of our knowledge. We found that patients with AS changed their gait phases by developing a unique pattern and created a more cautious and safe gait model to provide balance and stability. Our study revealed a statistically significant relationship between kinesiophobia and disease activity, functional status, quality of life, cadence, maximum plantar pressure, swing phase, and foot angle. The detailed data we obtained in our study will shed light on gait analysis and research on kinesiophobia in patients with AS. Kinesiophobia should be evaluated in routine gait rehabilitation protocols applied to individuals with AS, and treatment protocols for kinesiophobia should be added if necessary.","PeriodicalId":50831,"journal":{"name":"Aktuelle Rheumatologie","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-04-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47882817","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
