It is the most common cause of a unilateral massive pleural effusion. Most MPE s are secondary to metastases to the pleura, most often from lung or breast cancer. Medical management includes aspiration of fluid with the insertion of a chest tube, which may be necessary to relieve dyspnea. Hypoventilation does occur in certain areas of the lungs because of pain and muscle guarding after intercostal drainage tube (ICD) tube placements in pleural effusion. Therefore, it is important to emphasize pain management and expansion of affected areas of the lungs and chest wall. SMI is the basic maneuver of incentive spirometry and is mainly given to prevent atelectasis and lung collapse in postoperative patients. TENS is a method of producing an electro-analgesic effect and is effective in providing post-operative pain control. Methods: The study design of the study was a randomized controlled trial. A total of 44 participants with a diagnosis of MPE with an intercostal drainage tube were included in this study. The study duration was 5 years with an intervention period of 2 weeks and the outcome measures were pulmonary functions and intercostal pain severity using the NRS Scale. Results: There was a significant improvement in pulmonary functions in the control group and a highly significant improvement in pulmonary functions in the experimental group after 2 weeks of interventions and pain severity (NRS) was significantly reduced in the experimental group only (p<0.001) Conclusions: This study concluded that sustained maximal inspiration (SMI) along with TENS significantly improves pulmonary functions and reduces pain at the site of ICD in patients with MPE.","PeriodicalId":505944,"journal":{"name":"International Journal of Research in Medical Sciences","volume":"49 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139150502","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: The study was a retrospective descriptive cross-sectional full population study that involved getting the total population of 7880 paediatric patients that presented to the hospital facility within 3-years (from January 2019-December 2021) and from the total population getting the patients diagnosed with diabetes mellitus. A well-structured proforma was used to collect the relevant data needed from the case files of the patients. Statistical data analysis was done with SPSS version 25. Results: The study showed that out of 7880 paediatric patients that presented to the facility, 20 of them were diagnosed and treated for diabetes mellitus giving a prevalence of 0.25 per 100 patients. The paediatric patients diagnosed with diabetes mellitus were aged 8-18years; 11 females and 9 males. This study showed that about 75.0% of the paediatric patients with DM fully recovered following treatment and was subsequently discharged; however about 95% of these patients were lost to follow-up. Conclusions: Adequate measures needs to be put in place to ensure that the management of these patients are highly optimal and does not constitute much financial burden to the caregivers, in order to improve compliance to treatment and hence reducing morbidity and mortality associated with the condition.","PeriodicalId":505944,"journal":{"name":"International Journal of Research in Medical Sciences","volume":"23 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139152183","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Improved treatments for type 2 DM are still needed, in order to slow the disease’s development. A role in the pathophysiology of type 2 DM has been suggested by the correlation between vitamin D insufficiency and several non-skeletal illnesses, including DM. The goal of the study was to determine if vitamin D supplementation may help type 2 DM patients whose glycemic status was uncontrolled even after using oral antidiabetics. Methods: 60 individuals with type 2 DM and vitamin D insufficiency participated in this 12-week open-label, before-and-after study. For 12 weeks, in addition to oral anti-diabetic medications, these patients also received 60,000 IU of vitamin D3 orally every week. HbA1c, FBS, and 25(OH)D levels parameters were included. Results: The majority of the patients were from 41-50 years of age group (48.3%) with a male predominance (60%). Most of them were having >1 year of type 2 DM duration (78.3%) with a high family history of type 2 DM (70%). After 12 weeks, there was a substantial (p<0.001*) drop in FBG levels and a significant (p<0.001*) decrease in HbA1c. 25(OH)D levels showed a high rise (p<0.001*). None of the patients had any side effects. Conclusions: Vitamin D treatment improves glycemic status, which slows the development of type 2 DM and its associated effects. As such, vitamin D supplementation is a safe and promising adjuvant treatment for individuals with type 2 diabetes who are low in vitamin D.","PeriodicalId":505944,"journal":{"name":"International Journal of Research in Medical Sciences","volume":"38 12","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139151422","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This parasite is reported from nearly one quarter of the world population, especially from less developed countries. Poor hygiene conditions are associated with trichuris transmission, usually this infection remains asymptomatic in human but heavy infection can lead to chronic bloody diarrhoea associated with complications like rectal prolapsed, appendicitis, proctitis. Very few cases of extensive trichuriasis have been reported world-wide. Usually, diagnosis is confirmed by detection of eggs in stool sample. This case report deals with a patient of trichuriasis, who was diagnosed by detection of the parasite on colonoscopy. Thus, colonoscopy might be a useful diagnostic tool, especially in symptomatic patients who are infected by only few male worms with no eggs in the stool. Hence cannot be diagnosed by conventional methods.","PeriodicalId":505944,"journal":{"name":"International Journal of Research in Medical Sciences","volume":"1 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139150736","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They may have an asymptomatic course or present with pain, abdominal distension or intestinal obstruction. The suggested management is surgical resection of the lesions by laparotomy or minimally invasive surgery. We presented the case of a 48-year-old patient with progressive abdominal distension attributed to liver cirrhosis. Ultrasound and computed tomography of the abdomen showed evidence of a giant cyst of the mesentery with no evidence of tumor activity. We therefore decided to perform an exploratory laparotomy with cystectomy and omentectomy, follow-up and pathology report negative for malignancy.","PeriodicalId":505944,"journal":{"name":"International Journal of Research in Medical Sciences","volume":"333 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139152304","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Despite a year of proton pump inhibitor therapy, the patient experienced persistent symptoms, including nausea and nocturnal vomiting. Surgical intervention involved hiatal plasty, partial Dor fundoplication, and the unexpected discovery of the cystic tumor during the procedure. The cyst, firmly adhered and delimited, ruptured during dissection, leading to successful de-roofing, cauterization, and drainage. The patient's immediate postoperative course was satisfactory, demonstrating the effectiveness of this approach in managing an enteric duplication cyst. This case underscores the importance of considering congenital anomalies in the context of refractory GERD, with the enteric duplication cyst located at the esophagogastric junction representing a unique manifestation. The study contributes valuable insights into the atypical presentation, diagnostic challenges, and successful surgical management of such anomalies.","PeriodicalId":505944,"journal":{"name":"International Journal of Research in Medical Sciences","volume":"38 5‐6","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139149307","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
