In Kenya, major NCDs are cardio-vascular diseases, chronic respiratory diseases, cancers and diabetes. They account for 50% of all inpatient morbidities and 39% of all hospital mortalities. Patients afflicted with NCDs go through expensive treatment regiments, restraining them from utilizing available care. NCDs deepen inequality and are major drivers of unending poverty. World leaders resolved to deal with the devastating consequences of NCDs as a developmental challenge under SDGs. Kenya successively reformed its National Health Insurer to include a package that address the blight of NCDs and transform it into a primary enabler for achieving UHC. There is however, evidence suggesting that enrolment in health insurance (HI) does not necessarily guarantee inpatient utilization of NCDs care. This study examined the effect of HI on inpatient health service utilization among households with NCDs. \u0000Methods: A quasi experimental design was conducted among eligible households with HI cover and those without, involving a representative sample of 350 households. Interviewers conducted interviews at baseline and after one year among household heads. \u0000Results: Utilization of Inpatient NCDs care improved 1.256 (95% CI= 0.965-1.634), times more among insured households, (P=0.04). \u0000Conclusion: HI improves inpatient utilization of NCDs care. To accelerate progress towards UHC, national government should expand HI program to all counties, improve awareness of cover package entitlements and remove payment preconditions for inpatient procedures. A total of 125 participants were categorized based on their prostate symptoms (mild, moderate, and severe) and vitamin D levels (deficient, insufficient, and optimal). Among those with mild symptoms (n=60), 37.93% were vitamin D deficient, 44.11% had insufficient vitamin D, and 54.83% had optimal levels. For moderate symptoms (n=55), 44.82% were deficient, 47.05% had insufficient, and 41.93% had optimal vitamin D levels. In severe cases (n=10), 17.04% were deficient, 8.82% had insufficient, and 3.22% had optimal vitamin D levels. Overall, 23.02% of participants were deficient, 27.02% had insufficient, and 49.6% had optimal vitamin D levels. Quality of life assessments revealed that 56.8% of participants reported a good quality of life, with 46.66% of these being vitamin D deficient, 57.57% insufficient, and 61.29% optimal. An indifferent quality of life was noted by 29.6% of participants, with 28.57% deficient, 27.27% insufficient, and 29.03% optimal vitamin D levels. A very poor quality of life was reported by 13.6% of participants, with 21.42% deficient, 15.15% insufficient, and 9.67% optimal vitamin D levels. These results indicate a potential association between higher vitamin D levels and improved prostate symptoms and quality of life in BPH patients. Despite the impact of breast self-examination in detecting breast cancer earlier, the vast majority of cases still present with an advanced stage. Breast cancer is the most common type of cancer among women, particularly in low income countries. \u0000Objective: This study sought to evaluate breast self-examination habits and knowledge among women in their reproductive age in the Kalio-Ama village in Okrika LGA, Rivers State. \u0000Methods: Women residing in the study area were the subjects of a cross-sectional study. A straightforward random selection technique was employed to choose 250 individuals. Questionnaires given by interviewers were utilized to gather data. Data analysis was done with SPSS version 23. \u0000Results: From the total of 250 women participated in the study with a response rate of 100%. Of these, 250 (100%) of the respondents have heard of breast self-examination, 25 (10%) heard about it at home, 180 (72%) heard about it at the health facility while 45 (18%) heard on social media, 190 (76%) said self-examination of the breast is essential for early detection of breast cancer, 180 (72%) had knowledge and skills of BSE, 190 (76%) agreed that BSE will lead to early detection of breast cancer, 250 (100) of the respondents have practiced breast self-examination, 160 (64%) practice BSE on monthly bases while 90 (36%) practice BSE when necessary, 160 (64%) will continue the practice of BSE, 160 (64%) derive pleasure in carrying out BSE. It is crucial to investigate the decline in hospitalization cases in Feira de Santana and its possible linkage with the COVID-19 pandemic and local socioeconomic factors. \u0000Study Design:  An exploratory-descriptive bibliographic study with a qualitative approach. \u0000Place and Duration of Study: Department of Health, Higher Education Unit of Feira de Santana, Feira de Santana (Bahia), during the first semester of 2024. \u0000Methodology: The bibliographic survey was conducted through searches of scientific productions on the proposed theme, from 2018 to 2024. Searches were performed using keywords in Portuguese and English with the Boolean operator \"AND\". This study aims at assessing the risk factors and causes of neonatal mortality among sick infants in the special care baby unit (SCBU); focused at identification of possible areas of improvement in care and result.  \u0000Methods: A retrospective review of medical records of sick neonates admitted into SCBU of Imo State University Teaching Hospital Orlu. Sociodemographic and clinical characteristics of 214 sick neonates that died was obtained out of the 948 admitted/treated over a five year period (2018-2022). Descriptive statistics was used to summarize data, logistic regression at 95% level of significance was used to determine association between independent variables and neonatal death. Values of p < 0.05 were considered significant. \u0000Results: A total of 948 sick neonates were admitted into SCBU, with a neonatal mortality rate of 22.6%. Complications of prematurity 78 (36.7%), severe birth asphyxia 63(29.6%) and neonatal sepsis 31(14.6%) are the leading causes of death with majority of death occurring in the first 24 hours of life 130(60.8%). Low birth weight [OR=2.59 (95% CI 1.84-3.54)], prematurity [OR=2.79 (95% CI 2.03-3.83)] and spontaneous vaginal deliveries [OR=1.85 (95% CI 1.32-2.60)] were positively associated with increased mortality. While Caesarean section as mode of delivery [OR=0.54 (95% CI 0.38-0.76)] and birth weight >2.5-4.0kg [OR=0.46 (95% CI 0.34-0.64)] significantly reduced neonatal mortality. \u0000Conclusion: Neonatal mortality rate is still unacceptably high, with complications of prematurity, severe birth asphyxia and neonatal sepsis contributing the highest percentage of poor outcome (death). Low birth weight, prematurity, and spontaneous vaginal deliveries were positively associated with increased mortality. Chowdhury","doi":"10.9734/ajmah/2024/v22i71043","DOIUrl":"https://doi.org/10.9734/ajmah/2024/v22i71043","url":null,"abstract":"The current study was set out to look into the anti-inflammatory, anti-fungal, cytotoxic, thrombolytic and membrane stabilizing activities of the methanolic extract from the leaf of Camellia chrysantha (Hu) Tuyama (MECCL). Primary evaluation of MECCL was performed via phytochemical screening.  Phytochemical analysis of the leaf extract revealed the presence of reducing sugar, flavonoids, alkaloids, and steroids. Anti-inflammatory activity test was done using egg albumin denaturation assay. In the result of MECCL’s anti-inflammatory test showed 88.88% compared to the standard acetyl salicylic acid 98.56%. The anti-fungal activity test was performed by Disc Diffusion method using zone of inhibition against four fungi. MECCL showed moderate antifungal activity compared to standard Griseofulvin against Aspergillus niger, Saccharomyces cerevisiae (yeast), Penicillium notatum. Zones of inhibition of the fungi Aspergillus niger, Saccharomyces cerevisiae (yeast), Penicillium notatum is 11mm, 14mm and 14mm at the concentration of 700 µg/disc, which is close to the standard Griseofulvin 19mm, 20mm, and 21mm respectively at 50 µg/disc concentration. Mucor hiemalis showed mild zone of inhibition in 300, 500, and 700 µg/disc (7mm). Brine Shrimp lethality bioassay method was used to test the lethality of the methanolic leaf extract. The extract showed significant cytotoxic activity against brine shrimp nauplii with LC50 value of 0.843 µg/mL when compared with the standard Vincristine sulphate (LC50 value: 0.608 µg/mL). Comparison with positive control Vincristine sulphate signifies that cytotoxicity exhibited by MECCL and further bioactivity guided investigation can be done to find potent antitumor compounds. In thrombolytic activity test, MECCL demonstrated 95.69% thrombolysis which was significant compared to standard Streptokinase (SK) which demonstrated 91.304 % thrombolysis. While doing membrane stabilizing activity test, Heat induced hemolysis assay was performed and the percent of protection was 85.35 compared to the standard DS 73.63 which was very significant. The results highlight the potential of Camellia chrysantha (Hu) Tuyama as a valuable source of phytochemicals that possess notable anti-inflammatory, thrombolytic, and membrane-stabilizing properties. Originating from the bacterium Leptospira, it is transmitted through contact with the urine of reservoir animals, such as rodents. In Brazil, it is endemic, especially in the Northeast region, where poor sanitary conditions contribute to its dissemination. The objective of this study is to analyze the main determining factors for the increase in leptospirosis cases in the Northeast region of Brazil, with an emphasis on rainfall patterns and deficiencies in basic sanitation. \u0000Methodology: The methodology consisted of a qualitative exploratory-descriptive bibliographic review of articles published between 2014 and 2024 in Portuguese, English, and Spanish. Inclusion criteria were relevance to the topic, while exclusion criteria involved duplicated articles or those that did not highlight the content of interest. The search was conducted in databases such as LILACS, SciELO, and PubMed, using specific descriptors. The analysis involved careful selection and exhaustive reading for the categorization of results. \u0000Results and Discussion: Leptospirosis impacts urban and poor areas, with over one million annual cases globally. Factors such as precipitation and disordered urbanization contribute to its spread, especially in tropical regions. Low-income populations are vulnerable due to the lack of basic sanitation. The unequal distribution reflects socio-economic disparities, with densely populated urban areas being more affected. \u0000Conclusion: Leptospirosis poses a challenge to public health, especially in regions with poor sanitary conditions. Purposive and simple random sampling techniques were used to select 134 elderly from the study area. Data was collected using questionnaires through interviews. The SERVQUAL model was employed, looking at the gap score of five dimensions (tangible, reliability, responsiveness, assurance, and empathy). Data were analysed using IBM Statistical Package for Social Sciences Statistics software. \u0000Results: Majority (57.5%) of elders are aware of free health services and 66.4% of them accessed services. Elderly’s perceptions were lower than their expectations (gap = -1.70). The management can be divided into conservative treatment and surgical treatment. Conservative treatment involves the use of intravenous fluids and antibiotics, but patients are prone for developing recurrence of symptoms. Surgical treatment involves performing a cholecystectomy either as an open or laparoscopic method. As there is no consensus on the management of acute cholecystitis in pregnancy, we have conducted this review article to look at the role of conservative treatment, the role of laparoscopic cholecystectomy and when is the best time to perform this surgery.
