To experimentally study the effect of light of different ranges on the proliferation and migration of mesenchymal stem cells of human MSCs and to select its optimal parameters for treatment. \u0000Materials and methods. The experiment was conducted in vitro on 260 cultures of human MSCs isolated from peripheral blood by magnetic separation. Cells were treated with pulsed LED light: 475 nm, 516 nm, 635 nm or left unstimulated. All LED devices had a peak radiation intensity of 80 mW/cm2. The average radiation intensity reached 40 mW/cm2. Irradiation was carried out at room temperature for 10 minutes at a distance of 2 cm from the cells. Cells embedded in a 2D fibrin gel matrix to study cell proliferation and a 3D fibrin gel matrix to study cell migration were studied. Hereby, cells used for 2D experiments were stimulated on day 0, while cells embedded in 3D arrays were stimulated on day 0 and then every 24 h until quantification. The effect of different wavelengths on both proliferation and cellular metabolic activity of MSCs from peripheral blood was evaluated after initial light treatment at 24 hours, 48 hours and 72 hours. \u0000Results. During the first 48 hours after stimulation, cells proliferated in all studied groups (stimulated and non-stimulated). At the same time, there were no significant differences between the groups at 24 hours and 48 hours. For 2D experiments, cells were stimulated only on day 0, whereas for 3D experiments, stimulation was performed every 24 h. Quantification of cells migrating into the surrounding fibrin gel matrix showed that red and green light stimulation significantly affected 3D migration after 4 days. Irradiation with blue light had no significant effect on migration. \u0000Conclusions. Thus, exposure of MSCs to red and green light increases cell proliferation in 2D and 3D environments, while exposure to blue light decreases their metabolic activity. In our opinion, this fact should be used to modernize laboratory equipment and improve treatment regimens for patients using MSCs. The aim of the research is to study epistemological, philological, medical, social, economic aspects for pain in order to improve the awareness at the medical environment and develop effective forms, means and methods of medical care to patients with pain syndromes. \u0000Materials and methods. Bibliosemantic, comparative analysis, systems analysis. \u0000Results. Pain is dualistic both in its origin and in its mechanisms. Pain is both a physical sensation and an emotional response to it including cognitive and social components. In essence, chronic and recurrent pain is a separate health care issue, an independent disease. Convincing data were obtained on the presence of functional, structural and chemical changes in the brain due to chronic pain. The ability to modulate a response to pain is crucial for adaptation and takes place through cortical, stem, and cerebrospinal level. The response to pain is modulated by three mechanisms: gate control, descending modulation system, neuromodulators. \u0000Discussion. Chronic pain is one of the most burdensome state and one of the most common chronic diseases as the frequency of such disorders reaches the epidemic level, and the economic loss for health care provided for patients with chronic pain syndrome in terms of the total costs of society outweigh the costs of cardiovascular diseases, cancer, diabetes. Chronic pain is accompanied by anxiety, depression, fatigue, sleep disturbance, functional disability and reduced quality of life. This is an urgent issue especially nowadays during the warfare in Ukraine. The frequency and severity of the course of chronic pain and psychosomatic syndrome will certainly increase not only during the war but also in the post-war years, possibly in even more severe forms. Only specialized multimodal medical care for patients with chronic pain would benefit in terms of treatment outcomes, economic efficiency and social consequences. \u0000Conclusions. The environment of the oral cavity is a complex electrolyte environment with variable pH values, in which metal restorations release metal ions. These ions can induce cell apoptosis through intrinsic and extrinsic pathways. Dental alloys require excellent wear resistance and biocompatibility, which can extend the service life of removable prostheses. \u0000Aim. To study the composition of microelements and pH in the oral fluid in the presence of fixed cast prostheses made of cobalt-chromium and nickel-chromium alloys Remanium GM 700 and Remanium CSe of certified delivery and after multiple remelting. \u0000Materials and methods. 120 patients aged 25 to 60 took part in our study, including 79 women (65.9%) and 41 men (34.1%), who were divided into 4 groups of 30 people each. In the presented research studied the composition of microelements and pH in the oral fluid in the presence of fixed cast prostheses made of cobalt-chromium and nickel-chromium alloys Remanium GM 700 and Remanium CSe of certified supply and after multiple remelting. Depending on the type and topography of the defect in the dental row, patients were made permanent cast dentures. \u0000Results. After 7 days, the pH value shifted to the acidic side in people who were made solid-cast constructions with the specified alloys, and after 3 months it reached a neutral value. The dynamics and degree of expression of the change in pH depend on the timing of the study, the quality of the alloy, and the technological process of manufacturing solid-cast structures of dental prostheses. \u0000Conclusions. We determined that the composition of microelements of oral fluid in the presence of solid-cast prostheses changes under the influence of random factors. Marital codependency is a common form of family relationships that has significant socio-psychological consequences due to the persistent influence on the personal characteristics of codependent persons. In the sources of scientific literature, the gender-individual-psychological features of marital codependency are insufficiently covered, which determined the relevance of the topic of this study. \u0000Aim. The purpose of the study is to determine the individual psychological characteristics of men and women in codependent marital relationships. \u0000Materials and methods. 39 men and 46 women who are in codependent family relationships took part in the study. The following psychodiagnostic techniques were used: The Codependency Self-Inventory Scale (CSIS); Scale for measuring the level of codependency (D. Spann and D. Fisher); R. M. Hirschfeld Interpersonal Dependency Inventory (IDI); Test-questionnaire for determining self-esteem (S. V. Kovalev); Diagnosis of emotional intelligence (N. Hall); Coping test (R. Lazarus and S. Folkman). \u0000Results. It was established that the formation of codependent marital relationships is influenced by self-esteem, which is low in both codependent men and codependent women. It was found that codependent men and women equally have a low level of emotional intelligence. \u0000Conclusions. Gender differences in the manifestations of interdependence have been revealed. The hypothesis that the individual psychological characteristics of men and women influence the formation of codependent relationships in married couples has been confirmed. It is shown that a low level of the ability to manage emotions and to self-motivate, in combination with greater empathy, makes the coping strategies of codependent women more adaptive, determining their adaptation to codependent relationships and their inability to actively resist, while men are more prone to confrontation and distancing. \u0000Prospective research. An interesting direction of prospective research may be to find out the gender characteristics of the conflict behavior of spouses in codependent relationships. The study of the serum level of PCT and sTREM has been proposed as markers of bacterial infection, which have been applied for the early differentiation of processes of viral and bacterial etiology. \u0000Aim. Determination of express markers for predicting the effectiveness of empiric antibiotic therapy in patients with IPD. \u0000Materials and methods. Study groups with viral pneumonias, as well as bacterial etiology caused by Haemophilus influenzae, groups of patients with ChOPd I-II III- IV stages were formed. Controls for the studied groups were patients at the stage of remission and a group of healthy individuals without lung pathology and infectious diseases. The specific identification of microorganisms in the clinical material of patients with pneumonia was carried out in two stages: the first stage in the laboratories of the respective hospitals was carried out by the method of HIR and CFT; the second was carried out with the help of specific antibodies by the indirect fluorescence method using a fluorescent microscope and qualitative analysis test kits («Respiratory Tract Profile 1 BIOCHIP Mosaics for infectious serology», Medizinische Labordiagnostika AG «EUROIMMUN»). \u0000Results. In our study, the serum level of sTREM-1 was used as a criterion for assessing the probability of bacterial infection and, accordingly, increasing the degree of reliability of the prognosis provided by the PCT level study. The expression level of sTREM-1 and PCT were used as markers of bacterial infection for the early differentiation of processes of viral and bacterial etiology. \u0000Conclusions. The study of serum levels of PCT and sTREM, which are markers of bacterial infection and are used for early differentiation of processes of viral and bacterial etiology, has demonstrated the prognostic importance of the express assessment method of the infectious process etiology. Stetsenko","doi":"10.31612/2616-4868.2.2024.01","DOIUrl":"https://doi.org/10.31612/2616-4868.2.2024.01","url":null,"abstract":"The aim: To study the effectiveness of the enhanced recovery after surgery (ERAS) protocol for laparoscopic hernioplasty (LH) in patients with ventral hernias (VH). \u0000Materials and methods: 190 patients with VH after laparoscopic prosthetic hernioplasty with intraperitoneal mesh placement (IPOM) were included in the study and divided into two groups. The study group (ERAS group) included 92 (48.4%) patients to whom the ERAS protocol was applied, the control group (preERAS group) – 98 (51.6%) patients. The width of the hernia was more than 10 cm in 25 (13.2%) patients of the ERAS group. For them botulinum toxin type A (BTA) was injected into the muscles of the anterior abdominal wall 4 weeks before the operation. In the postoperative period, the duration of the operation, hospital length of stay, the intensity of the pain syndrome and well-being, the level of C-reactive protein (CRP) and interleukin-6 (IL-6) on the first postoperative day were evaluated. \u0000Results: After the introduction of BTA in 25 patients of ERAS group, the hernial defect decreased by an average of 4.6±0.62 mm and in all cases became less than 10 cm (р<0.001). The average duration of LH in the study and control groups did not differ statistically (ERAS; 91.2±37.41 min vs preERAS 88.9±30.05 min, p=0.76). In 2 hours after the operation, it was possible to activate all patients of the study group and 78 (79.6%) of the control group, within 4-6 hours – the other 20 (20.4%) patients of the control group. Using the ERAS protocol demonstrated significantly less intensity of pain syndrome according to visual analogue scale (VAS), complaints of nausea (р<0.001), bloating (р=0.017), feelings of hunger, thirst and general weakness (р<0.001). At the same time, there was no statistically significant difference in the presence of defecation (р=0.31). The average level of CRP after surgery was significantly higher in the control group compared to the study group (preERAS; 43.63 ± 13.90 vs ERAS; 16.55 ± 9.97, p<0.001). The level of IL-6 similarly increased more significantly in the control group (pre ERAS; 34.03 ± 18.18 vs ERAS; 11.44 ± 5.30, p<0.001). The length of hospital stay after surgery did not differ statistically between the groups (р=0.21). \u0000Conclusions: The use of the ERAS protocol during laparoscopic hernioplasty IРOM for patients with VH can reduce the intensity of the pain syndrome in the postoperative period and increase the patient's subjective assessment of their condition. Patients with type 2 diabetes mellitus (T2D) have an increased risk of cancer of different localizations. Identification and correction of diabetes-associated factors of oncogenesis can be important in cancer prevention. \u0000The aim of the study. To investigate the influence of diabetes-associated factors of oncogenesis on the formation of the risk of breast cancer (BC) and endometrial cancer (EC), as well as on the survival of women with the indicated localization of cancer, and to develop a method of calculating the predicted risk of BC and EC in women with T2D. \u0000Materials and methods. The study includes the results of a retrospective epidemiological analysis of cancer cases in patients, residents of Ivano-Frankivsk region with T2D during 2012-2016, with an analysis of 5-year survival of patients. Statistical processing of the results was conducted using STATISTIKA-12 (StatSoft Inc., USA). The impact of the pathogenetic factors of T2D on the development of BC and EC was evaluated through multifactorial analysis and ROC-analysis in the Medcalc v.19.1.6 program. The coefficient of predicted cancer risk was determined using mathematical modelling and a logistic regression equation. Kaplan-Meier cumulative survival analysis and Cox-Mantel Test were used to assess patient survival. \u0000Results. BC and EC were most often diagnosed in women of postmenopausal age, with obesity, with a duration of T2D > 5 years, on combined antidiabetic therapy. Among women with BC, who take secretagogues 67% had obesity and 24% were overweight, with EC – 54% and 27% respectively. A new method to calculate the predicted risk of BC and EC (Y) in women with T2D has been introduced. The accuracy of the mathematical model for calculating the Y index is 76.24%. It was proved that coefficient Y increases in women with obesity (p<0.001), duration of T2D > 5 years (p<0.001), on combined therapy with non-secretagogues and secretagogues (p<0.05). It was found that T2D increases the risk of death within 1 year in women with both types of cancer (p<0.05). A worse 5-year survival rate was found in women with EC stage I treated with drugs that increase blood insulin levels (p<0.05), as well as in women with EC stage II with HbA1c > 8.0% (p<0.05). \u0000Conclusions. Obesity, duration of diabetes > 5 years, and use of secretagogues in obesity increase the risk of breast and endometrial cancer in women with T2D. The use of antidiabetic drugs that contribute to iatrogenic hyperinsulinemia may negatively affect survival in obese women with breast and endometrial cancer, both by increasing the risk of cardiovascular events and by hyperactivating insulin signaling. Decompensation of diabetes reduces the 5-year survival of patients with breast cancer and endometrial cancer. Inflammatory diseases of the pelvic organs (IDPO) remain the most relevant issue in contemporary gynecology. In modern diagnostics of reproductive organ pathologies, a highly effective and accessible method is ultrasound examination. Recent studies in this field, conducted using high-frequency digital transvaginal echography, have significantly contributed to reevaluating the potential of ultrasonographic diagnosis of the cervix; however, further research is warranted. \u0000The aim of this research is to enhance the diagnosis of cervical changes in cervicitis. \u0000Materials and methods. The study included 68 women with chronic cervicitis. The diagnosis was established based on clinical, colposcopy, and bacterioscopist examinations. During transvaginal ultrasound examinations, the cervix was visualized in both longitudinal and transverse projections for all patients. The patients with chronic cervicitis were divided into two groups: Group I consisted of 37 patients with visual changes in the cervix, while Group II comprised 31 women without any changes. A comparison group included 35 healthy women. \u0000Results. Chronic cervicitis is characterized by an increase in the thickness and volume of the cervix, as well as the heterogeneity of the stroma due to cystic cavities, with elevated ratios of cervix thickness to length and endocervix thickness to cervix thickness. Significant sonographic signs of the inflammatory process in the endocervix also include certain structural features, such as indistinct contours, increased echogenicity, small cysts, and multiple microcalcifications in the endocervix, uneven dilation of the cervical canal, and an area of reduced echogenicity. \u0000Conclusions. To determine the role and place of endothelial dysfunction in the pathogenesis of comorbid gastroesophageal reflux disease (GERD) and obesity by studying the levels of nitric oxide (NO) metabolites in blood and urine. To evaluate the efficacy and benefits of the prescribed therapy, which included the additional inclusion of a peptide compound analogous to leu-enkephalin tyrosine-2-alanine-glycine-phenylalanine-leucyl-arginine diacetate (Dalargin). \u0000Materials and methods. Clinical manifestations were assessed using the GERD-HRQL questionnaire, levels of stable nitric oxide metabolites NO2 and total metabolites NO2 + NO3 were determined by spectrophotometric method, leptin levels were determined by enzyme-linked immunosorbent assay, pH-metry and motor function was studied by ultrasound. 130 patients were examined, including 70 patients with GERD with comorbid obesity of the first degree, 40 patients with GERD without concomitant pathology and 20 healthy individuals. The study was divided into 3 groups. \u0000Results. It was determined that the levels of stable metabolites NO2, NO2 + NO3 in blood and urine in the group of GERD with obesity are significantly lower than in the group of isolated GERD (p<0.001). A direct correlation between the level of nitric oxide metabolites in blood and urine and acidity in the stomach and oesophagus was established; an inverse relationship between NO metabolites and the degree of gastrointestinal motor and functional disorders, the severity of clinical manifestations, and the level of hormonal secretion by adipocytes. The supplemental inclusion of Dalargin improved the effectiveness and quality of pathogenetic therapy and achieved a more positive disease dynamics and is recommended for prescription in practice. \u0000Conclusions. Obesity is a predictor of a more severe course of GERD and more severe endothelial dysfunction. The study involved 80 patients. The range of motion in the shoulder joint affected by capsulitis was assessed. A numerical pain scale was used to assess pain sensations when pressing on trigger points. Functional limitations in the shoulder were measured by means of the Oxford Shoulder Score (before and 1.5 months after the end of the physical therapy). Participants were divided into two groups: home-based group (HG) and outpatient group (ОG). All the patients were consulted by a physical therapist. ОG patients received end-range mobilization with the assistance of a physical therapist 5 times a week to improve shoulder joint mobility. Most of ОG participants received 15 procedures (3 weeks). Patients HG performed therapeutic exercises without any assistance at home. (twice a day, 3 weeks). In terms of trigger points therapy, the patients performed ischemic compression without any assistance. \u0000Results. Patients of both groups demonstrated positive dynamics of the studied indicators. Comparison of the short-term results of the therapy according to range of motion and pain showed better indicators in the ОG. Comparison of the initial results of the groups in Oxford Shoulder Score items did not reveal any significant differences. Total score was 18.2±6.5 points in ОG and 17.6±7.5 points in HG (p=0.715). Me (25%; 75%) values were 17.5 (12.5; 23.8) and 18 (13; 23.5) points, respectively. Comparison of the final results of the groups in Oxford Shoulder Score items revealed significant differences. Total score was 47 (47; 48) points in ОG and 33 (31; 35.75) points in HG (p<0.001). x̅±SD values were 46.5±2.6 and 32.8±4.4 points, respectively. \u0000Conclusions.
