This study aimed to identify the experiences and perceptions that may have an impact in the long-term on mothers' birthing experience, considering hospital design features in the birthing environment until discharge.</p><p><strong>Methods: </strong>Inductive thematic analysis of twenty-five hospital labor testimonies employing a phenomenological research approach and utilizing a biographical method. Participants were women with a professional background in architecture, landscape architecture, engineering, or interior design.</p><p><strong>Results: </strong>The results are organized into four themes and seven subthemes. The first theme is \"First sight and long term impression\" which is subdivided into the subthemes \"Depersonalized itinerary in entrances and corridors\" and \"Instinctive search for connection with nature\". The second theme deals with \"Accompaniment and tucking in during the birthing process\", subdivided into \"Hotel-like: space for movement and personalized adaptation\" and \"Helplessness, cold and uncertainty: spaces to be against one's will\". The third theme is \"Damage in collateral rooms\", which includes \"The integration of toilets in the birthing process\", \"Operating rooms unchangeable in the face of cesarean delivery\" and \"Neonatal units that do not integrate families\". Finally, the fourth theme includes \"Improvement proposals for new designs\".</p><p><strong>Conclusions: </strong>This study contributes to the existing literature by deepening the understanding of the design features identified in hospitals in recent studies. We aimed to analyze the sense of coherence in lactating women to determine the general resistance resources during lactation and the professional interventions that promote a high sense of coherence.</p><p><strong>Methods: </strong>A search of studies in English, Spanish, or Portuguese on lactating women's sense of coherence was carried out in PubMed, PsycINFO, ScienceDirect, and CINAH databases published between May and November 2022. Study quality and risk of bias were examined according to ICROMS and STROBE criteria.</p><p><strong>Results: </strong>We identified 316 studies, of which eight -all of adequate quality- were included, three qualitative and five quantitative. A high level of maternal sense of coherence was related to longer duration, self-efficacy, attachment, and enjoyment of the breastfeeding experience. The main general resistance resources were to receive social support, particularly from partners, mothers, and health professionals. {"title":"[Occupational balance, disability and functionality in people with rheumatic disease].","authors":"Ana M Martín Pérez, Nuria Máximo-Bocanegra, Francisco Rivas Ruíz, Isabel M Alguacil-Diego, Rosa M Martínez-Piédrola","doi":"10.23938/ASSN.1053","DOIUrl":"10.23938/ASSN.1053","url":null,"abstract":"<p><strong>Background: </strong>We aimed to assess the occupational balance of people with rheumatic disease, analyze its relationship with participation, performance, and satisfaction with daily life activities, and evaluate whether age or receiving non-pharmacological treatment affects the outcome.</p><p><strong>Methods: </strong>Cross-sectional study carried out between March and November 2021; patients with non-advanced stage of rheumatic disease from the ConArtritis Association - selected through simple random sampling -, were included. Sociodemographic data and scores from the OBQ, IMPACT-S, COPM questionnaires, and a questionnaire created ad hoc for daily life activities were collected online and/or by telephone.</p><p><strong>Results: </strong>The occupational balance of the 47 participants was low (OBQ: 34.2; SD: 13.7). Despite a high participation in daily life activities (IMPACT-S: 76.8; SD: 13.1), the degree of performance and satisfaction with these activities was far from optimal (COPM-R: 3.9; SD: 2.0 and COPM-S: 4.3; SD: 2.5); 46.8% of the participants found limitations in at least four daily life activities (basic and instrumental) and 61.7% used at least one support product in their daily lives. These limitations reduced their resting time and affected their jobs, hobbies, and personal relationships. We aimed to characterize the features of these entities in our environment.</p><p><strong>Methods: </strong>We carried out a search of patients who underwent surgery for tumors and cysts in the heart between 2002 and 2022 in the registry of the Department of Cardiology and Cardiac Surgery of Clínica Universidad de Navarra (Pamplona, Spain). Sociodemographic, clinical, histological, and surgical variables were collected.</p><p><strong>Results: </strong>We identified 13 patients; mean age was 63.08 ± 15.17 years, 76.92% were female and 92.31% had at least one cardiovascular risk factor, e.g., BMI = 25 kg/m2 and high blood pressure (61.54% and 53.85%, respectively). The most common type of cardiac tumors were myxomas (69.23%). Around half (46.15%) were incidental; the most frequent symptom was dyspnea (53.85%); 30.77% of the patients were asymptomatic. The most commonly used imaging technique for the diagnosis was transthoracic Doppler echocardiography (69.23%). The agreement between the mean diameters before and after surgery was very high (ICC = 0.807, 95% CI: 0.450 - 0.943).</p><p><strong>Conclusions: </strong>We describe the features of masses and cysts in the heart (77% female patients) and provide information scarcely available in the literature, e.g., the most frequent cardiovascular risk factors for this population. Candidate patients to receive this antiviral agent are usually vulnerable, multipathological and polymedicated. The objective is to evaluate the pharmaceutical validation prior to the administration of the antiviral.</p><p><strong>Material and methods: </strong>Drug-drug interactions between nirmatrelvir/ritonavir and patients' usual treatment medications were checked in product information and in the UpToDate® and the University of Liverpool® interaction tools. We included validated prescriptions between April/2022 and April/2023 by a Primary Care pharmacist.</p><p><strong>Results: </strong>Of the 159 study patients, 168 interactions were found in 83 individuals, which may have led to changes of their usual treatment. Statins (25.6%), anticoagulants (10.7%), and antihypertensives (10.7%) were the most frequently implicated therapeutic groups. Follow-up and treatment were done in our tertiary hospital between January 1, 2014 and December 31, 2018.</p><p><strong>Results: </strong>The study included 778 patients; 62.2% female and mean age 79.83±7.94 years; 957 eyes had exudative AMD. Mean of final VA (0.65±0.45) increasing 3.2% compared to initial values. Ranibizumab was administered to 60.3% of the eyes, aflibercept to 10.2% and ranibizumab + aflibercept (mixed group) to 29.5%. Significant increase in VA was seen in the group with the mixed treatment, with no inter-group differences. Although follow-up/treatment was longer for the mixed group, they received fewer anti-VEGF injections and optical coherence tomography (OCT). The total expenditure per year and treated eye was € 1,972.7±824.5; costs were higher for visit, OCT, and treatment in the aflibercept group, and lower for fluorescein angiography, antiVEGF treatment, and total costs in the mixed group. Decimal VA gain had a cost of € 872±1,077.7 with no significant inter-group differences.</p><p><strong>Conclusions: </strong>AntiVEGF treatments (ranibizumab, aflibercept, or both) maintained VA in patients with exudative AMD.
