It moves from the methodologies to the method of conducting qualitative research. It will explore three key components of the research process: developing a research question, data collection, and data analysis. It also considers some of the limitations of qualitative research.

All qualitative research processes pivot on the research question. The question will determine both the methodology and the method of conducting the research. It encapsulates the object of the research inquiry and facilitates the research process. Ideally, its focus would be the patient's experience of some aspect of mental health nursing practice. A good place to start is to identify an area of mental health nursing interest or consider those taken-for-granted aspects of practice e.g., the patient's experience of admission, treatment, a specific intervention, the relationship with nurses, and the psychiatric process. The next step is to explore the literature on the topic and what studies have been undertaken.

The research needs to contribute to new knowledge, so it is important to identify gaps in the literature. For example, your interest may be the use of observations in an acute inpatient setting as a strategy to manage risk. You would need to explore the nursing literature and the broader literature on observation and risk management and the mental health nursing literature. Most research raises further questions, and it is these unanswered questions that require further investigation. There will be relevant studies that have focused on observation and risk management, but your goal is to identify those aspects of the findings that raise further questions. Studies may have examined patients' experiences of observations from different perspectives, and your question may develop from those findings e.g., one study found that patients' sleep was disturbed by nursing observations at night (Veale et al. 2020). You may choose to develop a subsequent question around what patients think may be more helpful or the patients' experience of sleep deprivation and its impact on recovery.

Another strategy for developing a research question is to focus your exploration of the literature on systematic and integrative reviews and qualitative meta-syntheses. Observations may be regarded as a form of restrictive practice so you would need to look at the literature reviews on this topic. For example, an integrative review of patient preferences related to restrictive practices (Lindekilde et al. 2024) reported that factors such as environment, communication and duration were found to influence patients' preferences. After exploring a range of literature, a research question could be developed inquiring about the types of communication that patients would prefer when they are being observed, or what factors in the environment could impr","PeriodicalId":50076,"journal":{"name":"Journal of Psychiatric and Mental Health Nursing","volume":"32 1","pages":"256-258"},"PeriodicalIF":2.6,"publicationDate":"2024-10-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC11704982/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142373406","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

These methods do not work from pre-determined categories applied to the person's experience but rather seek to find and interpret the categories and themes from within the collected data Qualitative research fits with the skills embedded in mental health nursing practice, qualitative research because it is well-suited to the investigation of the complexities of human responses and experiences that are integral to everyday practice. Mental health nurses who practice a recovery-focused model of care have a unique perspective on the complexities of mental health distress and the patient's needs for connectedness, hope, identity, meaning and empowerment that promotes recovery (Leamy et al. 2011).

Qualitative research methods are oriented towards understanding meanings and experiences. They are therefore potentially very useful in the mental health context because they can provide new insights and knowledge in poorly understood and complex areas, such as understanding subjective experiences of mental disorders and their treatments (Fossey et al. 2002). The subjective experience is needed to inform the interpersonal, socio-cultural and political context of mental distress, treatment and service delivery. While quantitative and statistical research methods enable deduction and prediction irrespective of context, qualitative research enables contextualised understandings of subjective experiences.

Quantitative research is focused on the experimental examination of an intervention in terms of quantity, amount, intensity or frequency and is supposedly objective but the choice of what is examined and how is always a subjective process. Qualitative researchers study things in their natural setting, attempting to make sense of or interpret phenomena in terms of the meanings people bring to them (Denzin and Lincoln 2011). It does not obscure the subjective nature of the research process but acknowledges that there can be multiple interpretations of the data. An audit trail identifies how the interpretations were developed.

While qualitative research makes no claims to objectivity it does follow rigorous processes based on theoretical paradigms and epistemologies. These need to be clearly stated within the paper because they determine the characteristics of the study aim and its methods of data collection and analysis. The most commonly used methods are phenomenology, grounded theory, discourse analysis and interpretative thematic analysis. These methods are supported by explicit theoretical paradigms underpinned by ontological and epistemological assumptions of the nature of reality and the nature of experience.

The following section provides an overview of some of the core methodologies. In psychiatric healthcare specifically, conversational AI is seen as a potential solution to support psychiatric nurses in the assessment of psychiatric illnesses (Rebelo, Verboom, and Santos 2023). However, it is important that the oversight of a psychiatric nurse is needed when integrating conversational AI into psychiatric care. This editorial explores the risk of using conversational AI for psychiatric assessments and emphasises the need for human intervention in AI-driven processes.

In community mental health settings, psychiatric nurses play a crucial role in managing a wide range of conditions. The increasing demand for care, driven by factors such as heightened public awareness and reduction of stigmatisation for psychiatric illnesses, has led to longer waiting times for services (British Medical Association 2024). Conversational AI systems have been proposed to alleviate this pressure by streamlining the psychiatric assessment process (Rollwage et al. 2024). One of the key benefits of AI is its potential to triage and prioritise those with the most urgent needs, thereby ensuring that critical cases are addressed promptly (Lu et al. 2023).

Psychiatric assessments require a deep understanding of the patient's presentation, psychiatric symptoms and the context of patient behaviour. Conversational AI systems are based on language learning models (LLM) and analyse data they have been trained on previously, which is mainly in written format and often derived from patients' electronic health records initially, advises Yang et al. (2022). Electronic health records contain all patient notes relevant to the episode of care the patient is receiving. It can include quantitative data sets such as diagnoses, charts, patient demographics and patient-led questionnaires based on symptomology or more nuanced qualitative data sets, such as psychiatric nursing observations of patient behaviour and caregiver views (Goldstein et al. 2022). These data points help to construct a picture of a patient's presentation as part of an assessment before a patient is spoken to. Indeed, the psychiatric nurse will often review referrals, previous history and information to gain some knowledge of the patient before assessing the needs for that episode of care. However, an AI could face significant challenges in interpreting patient information with the depth and nuance that a psychiatric nurse could, suggests Elyoseph, Levkovich, and Shinan-Altman (2024), potentially resulting in inaccuracies in assessment, which can lead to inaccurate severity assessment and poor treatment outcomes.

The notion of bias in AI systems is a significant issue in psychiatric care, particularly because these biases often originate fro","PeriodicalId":50076,"journal":{"name":"Journal of Psychiatric and Mental Health Nursing","volume":"32 2","pages":"382-383"},"PeriodicalIF":2.6,"publicationDate":"2024-09-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://onlinelibrary.wiley.com/doi/epdf/10.1111/jpm.13119","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142331405","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
