Aim: identification and synthesis of data on posttraumatic growth (PTG) and socio-demographic, clinical and psychological factors associated with PTG in relatives, caring for patients with cancer. Methods: The review was conducted according to the statement of the PRISMA guidelines. The review used 2 search strategies: systematic electronic search in databases (Pubmed, Google Scholar, Cochrane, EBSCO, SAGE journals и eLIBRARY), manual search of articles in references lists of eligible articles. Results: a total of 23 studies were included in the review. The total amount of relatives and loved ones, caring for cancer patients was 5140. The average age of respondents was 52,2 (SD = 10,77) years. The majority of those surveyed were female - 58% and were spouses/partners of patients (52%). PTG among cancer patients' relatives is a universal phenomenon that occurs in a wide variety of socio-cultural populations and ethnic groups. The Posttraumatic Growth Inventory (PTGI) is the most popular tool used for assessing PTG in the analyzed studies. Conclusion: there is a gap in domestic research on PTG among cancer patients' informal caregivers. The complexity and multivariance of the relationships of concomitant diseases does not allow us to evaluate their complex interaction according to the concept of comorbidity. The concept of multimorbidity is convenient and promising for studying various aspects of the combination of both mental disorders and their associations with somatic pathology. However, at present, most studies in the area of multimorbidity are based primarily on epidemiological assessments, while much less attention has been paid to the mechanisms of multimorbid conditions. In this review, we consider the main recent approaches related to the study of the multimorbidity of mental disorders and somatic diseases in order to determine the optimal research strategies in this area. Further studies of the etiopathogenetic aspects of multimorbidity, the mechanisms of multimorbid clusters, and the aspects of the course and prognosis of multimorbid diseases are necessary to optimize the concept for both scientific projects and clinical practice. The most important directions for future research on the phenomenon of multimorbidity of mental disorders and somatic diseases are: 1) the study of their shared genetic and molecular mechanisms of development; 2) clinical and fundamental prospective studies of the manifestation, concomitant course and outcomes of multimorbid diseases; 3) study of the effectiveness and tolerability of psychopharmacotherapy in multimorbid patients with various somatic diagnoses, considering the complex interaction of the drugs used.","PeriodicalId":497956,"journal":{"name":"Obozrenie psihiatrii i medicinskoj psihologii imeni V.M. Experimental studies of using drinking motive phenotypes for developing personalized treatment approaches had promising results and have demonstrated the theoretical and practical relevance of their further investigation and assessment. The purpose of this paper is to explore the psychometric properties of the Russian version of the Reasons for Heavy Drinking Questionnaire. For this study 163 patients (108 men (71%), mean age 43.00 years [38.00;43.00] (Mdn [Q1; Q3]), mean disease duration 10 years [4.00;17.00] (Mdn[Q1; Q3]) undergoing inpatient treatment for alcohol use disorder were recruited. The following instruments were used: clinical interview, Reward, Relief, Habit Drinking Scale, Penn Alcohol Craving Scale, Hospital Anxiety and Depression Scale, Snaith-Hamilton Pleasure Scale, and Carver and White’s BIS/BAS scale. The results did not confirm the factor structure proposed by the authors of the questionnaire. However, two factors - \"Positive Reinforcement\" and \"Normalization and Habit\" - were identified. Multiple significant correlations were also identified between drinking motivation phenotypes and clinical and psychological characteristics (previous treatment experience and periods of alcohol remission, emotional impairment, behavioral activation and inhibition in response to reward and punishment, and another scale for assessment of reward, relief, and habit drinking motives).","PeriodicalId":497956,"journal":{"name":"Obozrenie psihiatrii i medicinskoj psihologii imeni V.M. Identification of specific patterns of atypical behavior in these disorders can improve the specificity of diagnoses and monitor response to treatment. Therefore, it would be useful to have an approved questionnaire for the assessment of pathomorphosis and pathoplasty of stereotypes in ASD and schizophrenia in childhood, aimed at parents and caregivers of children in this cohort. Objective: To develop and validate a questionnaire for assessing the manifestations and dynamics of stereotypes in ASD and ADHD in childhood, aimed at parents and guardians of children in this cohort, for further use in clinical practice. Materials and Methods: in accordance with international and Russian standards, a questionnaire was developed to assess the manifestations and dynamics of stereotypes and tested on a focus group of parents (legal representatives) of children diagnosed with ASD and PSD, followed by an assessment of the results of psychodiagnostic testing - reliability, validity and sensitivity of the results obtained. developed questionnaire. Statistical analysis was performed using Statistica, version 9.0. Results: according to the results of testing with the participation of a group of parents (legal representatives) of the selected cohort, the acceptability of the questionnaire and high validity rates were confirmed. The conducted factor analysis testifies to the adequacy of the grouping of questions of the questionnaire according to the expected factors and stable construct validity. In this regard, the use of therapeutic drug monitoring (TDM) of AP seems to be a clinically relevant method for the individualization of psychopharmacotherapy in the treatment of exacerbations of schizophrenia. The purpose of this work was to develop recommendations on the use of TDM AP for the individualization of therapy for exacerbation of schizophrenia. Materials and methods. To develop recommendations, a literature search was performed on the Medline, Google Scolar and Elibrary databases and the materials of the dissertation of Potanin S.S. \"The role of therapeutic drug monitoring of antipsychotics in the individualization of therapy for exacerbations of paroxysmal-progredient schizophrenia\" were used. Results. The conducted literature search made it possible to classify AP according to the degree of expediency of TDM, to determine the optimal therapeutic concentrations for each drug, indications for TDM, and to develop a structured decision-making algorithm depending on the results obtained. The main indications for TDM AP in the treatment of exacerbation of schizophrenia are signs of impaired drug compliance, insufficient efficacy of therapy, pronounced dose-dependent side effects, and the addition of concomitant therapy that can significantly affect the concentration of AP. According to the results obtained, TDM is strongly recommended for clozapine, olanzapine and amisulpride, recommended for risperidone, paliperidone, aripiprazole, quetiapine, haloperidol, ziprasidone, perphenazine, sertindole, trifluoperazine, sulpiride and chlorpromazine, for other antipsychotics TDM may be useful in selected cases. A detailed decision-making algorithm is presented in the form of a table and is based on both the clinical situation and the results of TDM AP. Conclusion. The presence of cognitive and affective disorders of varying severity was noted in 415 (83.6%) patients. As an addition to the neurological diagnosis was revealed: 171 patients (61.3%) had anxiety-depressive syndrome, 245 patients (87.8%) had emotional-volitional instability syndrome, and 101 patients (36.2%) had autism spectrum disorder (ASD). But further psychiatric verification of these syndromes in accordance with ICD-10 was not carried out. However, there is still a risk of drug-induced adverse events during long-term administration of antiepileptic therapy. One of the most unfavorable changes in bone tissue associated with anticonvulsant use is osteoporotic disorders, which result in a loss of bone density, making the bones more fragile and prone to fractures. The problem of decreased bone mineral density and frequent fractures in patients with epilepsy is an important and understudied issue that significantly reduces quality of life and involves significant economic costs for the treatment and rehabilitation of epileptic patients. Studying the interaction between osteoporosis and epilepsy is of great importance for the development of effective methods for timely diagnosis, treatment and prevention of bone metabolism disorders. This article presents pilot results of a study to investigate the effect of antiepileptic therapy on mineral metabolism and bone density. The aim of the study: to evaluate bone mineral density in adult patients with epilepsy long-term receiving antiepileptic therapy. Materials and methods. Thirty-eight adult patients with epilepsy taking antiepileptic drugs for a long time were examined. All patients underwent general clinical, neurologic examination and densitometric study by quantitative computed tomography at three points (L1, L2 and femoral neck). Results. Decreased bone mineral density was found in 34.2% of the patients. Of them, 29% had osteopenia and 5.2% - osteoporosis. The change in mineral density was observed at a median duration of antiepileptic therapy of 8 years. ROC analysis showed that bone mineral density decreased statistically significantly with increasing duration of anitconvulsant therapy (SROC 0.929±0.052; 95% CI: 0.827-1.000). Correlation analysis revealed a markedly close association (ρ = -0.626, p < 0.001) between bone mineral density and duration of antiepileptic therapy. Conclusion. The results of the study confirm the effect of antiepileptic therapy on bone mineral density. And show that the probability of developing osteopenia and osteoprosis with longer duration of anticonvulsant therapy is higher than in the general population. In order to determine psychometric properties and prognostic reliability, 160 patients of both sexes with persistent illegal and law-abiding behavior, average age 40 ±13 years, were examined using the Protocol. The respondents were assessed for affective rigidity, extrapunitive orientation of frustration reaction, cognitive rigidity, low self-esteem, internal locus of control, lack of empathy, egocentrism, ability to reflect on life experience. Each feature was assigned a score, the scores were summed up, the amount was compared with the threshold value. Prognostic reliability was assessed using ROC analysis (Receiver Operating Characteristics). Sensitivity and specificity were 98.1% and 88.1%, respectively. The area under the ROC curve is 0.97±0.01 with 95% CI: 0.951-0.989. The resulting model was statistically significant (p<0.001). This Protocol is not an independent tool, but serves as an addition to the methodology of Structured Risk Assessment of Dangerous Behavior (SOROP) and clinical assessment. Its integration into the risk determination procedure will ensure the implementation of a comprehensive multidisciplinary approach in making judgments about the potential public danger of the patient, will increase its objectivity and evidence.","PeriodicalId":497956,"journal":{"name":"Obozrenie psihiatrii i medicinskoj psihologii imeni V.M. Abdullaeva","doi":"10.31363/2313-7053-2023-773","DOIUrl":"https://doi.org/10.31363/2313-7053-2023-773","url":null,"abstract":"This article presents an assessment of the effectiveness of the program for providing medical and psychological care to patients with insomnia, based on psychotherapeutic and psychophysiological approaches, including using a light lamp, biofeedback devices, taking into account clinical and psychological and socio-psychological characteristics, and also evaluates the effectiveness this program. The scientific literature does not describe analogues of the totality of the use of these methods for the contingent of patients with outpatient insomnia. The uniqueness of the developed program lies in its clear algorithmization, the absence of complex techniques of psychological (psychotherapeutic) intervention, which ensures the reproducibility of the program in practical health care institutions if there is a psychotherapist or clinical psychologist on staff who knows the skills of cognitive behavioral therapy and diaphragmatic breathing. The technique has passed clinical testing approved by the Ministry of Health of the Russian Federation (protocol 2020-37-3). The quantitative evaluation of the effectiveness of the method was carried out on seven indicators: (1) Improvement in the subjective characteristics of sleep, (2) Improvement in the quality of sleep, (3) Reduction of the insomnia severity index, (4) Reduction of daytime sleepiness, (5) Reduction of the consequences of sleep disorders, (6) Decreased anxiety, (7) Improved overall clinical impression. As part of the assessment of the achievement of the target result, in accordance with Order 46n of the Ministry of Health of the Russian Federation dated February 2, 2022 (Regulations on Clinical Approbation), a threshold of 85% was set. In terms of (1) Improvement in the subjective characteristics of sleep, (2) Improvement in the quality of sleep, (3) Decrease in the severity index of insomnia, (4) Decrease in daytime sleepiness, the target result was more than 85%, which is significantly higher compared to the control group. An assessment was made of the correspondence of the distribution of indicators to the normal law using the Kholmogorov-Smirnov criterion. Comparison of groups using methods of analytical statistics was carried out using Student's t-test for dependent samples and Mann Whitney's U-test for independent samples. The assessment of the correlation of quantitative results was carried out using the Pearson coefficient (r) for interval variables with a normal distribution. The results are considered to be significant at p<0.01. All obtained empirical values were in the zone of statistical significance. Evaluation of the data obtained in the course of clinical testing shows the effectiveness of the proposed clinical and psychological method for the treatment of insomnia.","PeriodicalId":497956,"journal":{"name":"Obozrenie psihiatrii i medicinskoj psihologii imeni V.M. Authors also describe a clinical case from their own practice. In the aforementioned case, the use of Metadoxil as a part of combination therapy contributed not only to the stabilization of remission of the alcohol use disorder, but also to diminishing of symptoms of comorbid mental (obsessive-compulsive disorder) and somatic (alcoholic fatty liver disease) diseases.","PeriodicalId":497956,"journal":{"name":"Obozrenie psihiatrii i medicinskoj psihologii imeni V.M.
